4.Comparision between Treatment Methods of Simple Bone Cyst

Simple bone cyst is a benign bone tumor occurring most frequently in the long bone. The pathogenesis is not yet fully known and there is no established mode of optimal treatment. In 1979, Scaglietti reported favorable results in the treatment of simple bone cyst with steroid injection. We have studied 23 cases of curettage with bone graft and 13 cases of steroid injection in 39 cases of histologically proven simple bone cyst in the department Orthopedic Surgery at College of Medicine Yousei University dating from Jan, 1970 to Jan. 1989. In 1 case out of 23 cases, there was no improvement in curettage with bone graft, and this cases was treated with steroid injection. There were 4 cases which did not receive any treatment except cast immobilization. In this case, the patient refused further treatment. The mean follow up period was 20.1 months and the results and conclusions were as follows:1. The average age of patients was 17.9 years, and incidence under 20 years was 74.4% The male to female ratio was 1.6 to l. 2. Twenty three cases were treated with curettage with bone graft. The average age of patients was 16.1 years, and there were 4 cases of recurrence of simple bone cyst (17.3% ). 3. Thirteen cases were treated with steriod injection. The average age of patient was 12.1 years, and there was 1 case of recurrence of simple bone cyst(7.6%). There were 6 cases who were completely healed after one steroid injection, 3 cases after 3 injections, 1 case after 4 injection, and 3 cases who had a maximun of 5 injections. 4. The difference was statistically significant from equality (p<0.05). The steroid injection method is a more simple and a more safe procedure than curettage with bone graft, and can be done at the Out-Patient Department. Steroid injection may be repeated in persisting and recurrent simple bone cyst.

7.Clinical study of Marjolin's ulcer.

Marjolin's ulcer is the malignant lesion which develops in a burn scar or chronic fistula. Due to the low prevalence of this lesion, there has been disagreement regarding its clinical features, methods of treatment, and prognosis. We evaluated 19 cases of patients who had been admitted to Severance Hospital from Jan. 1970 to Dec. 1985. The results were as follows: The previous lesion was a burn scar in 52% of the cases and a fistula of chronic osteomyelitis in 32%. The mean latent period was 31.5 years. The initial symptoms were increased pain (74%), discharge with foul odor (68%) and bleeding (58%). Upon histological examination, all of the cases were squamous cell carcinoma. The rate of metastasis at the time of diagnosis was 32%. Of the 16 patients treated by surgery, local recurrence was noted in 4 cases. Three of these cases were patients who had been treated by excision and split thickness skin graft. The time interval for local recurrence ranged from 6 months to 11 months (average 8.8 months). In conclusion, the squamous cell carcinoma of marjolin's ulcer seems to have a worse prognosis than other squamous cell carcinomas and it requires aggressive treatment. The burn scar or chronic fistula that occurs in elderly patients especially requires more adequate treatment and close observation.

8.Cementless Total Hip Arthroplasty Using Circumferentially Porous Coated Femoral Stem: Minimum Five-Year Followup Study.

PURPOSE: The result of more than 5 year follow up after cementless total hip arthroplasty using circumferentially porous coated femoralstem was evaluated. MATERIALS AND METHODS: Eighty five patients (95 hips) were followed more than 5 years after a primary total hip arthroplasty using multilockfemoral stem, which was circumferentially porous coated at the proximal 1/3 of femoral stem. The average age at the time of operation was 48 years. The average duration of followup was 6.9 years. The Harris hip score was used for the evaluation of clinical results. The canal fit of the stem at the proximal canal and isthmus level was evaluated from the postoperative radiographs. The component stability, endosteal bone formation, radiolucent line, osteolysis and polyethylene wear were evaluated from serial followup radiographs. RESULTS: The mean Harris hip scores increased from 60.1 points preoperatively to 91.3 points at the time of the most recent followup. All hips had a press fit at both the proximal canal and isthmus level. On the lastest followup radiograph, endosteal bone formation was observed in all cases (100%). The radiolucent line around the femoral stem was observed in 92 cases (97%), mainly Gruen zones II, III, IV, V and VI. The radiolucent line was thinner than 1 mm and there was no progressive widening. The acetabular osteolysis developed in 31 cases(33%) and the femoral osteolysis in 31 cases (33%) mostly in Gruen zones I, II, and VII. There was no distal osteolysis in the femur. There was no loosening or migration of cup and stem. The mean linear wear was 1.29 (0-5.69) mm and the mean rate of linear wear was 0.18(0-0.81) mm/year. Seventeen hips (18%) were revised for the dissociation of polyethylene liner in 10 hips (11%), excessive wear in the polyethylene liner and progressive osteolysis in 6 hips (6%) and recurence of tuberculous arthritis in 1 hip (1%). CONCLUSION: Cementless total hip arthroplsty using circumferentially porous coated femoral stem showed good medium-term results clinicallyand radiologically, it is likely that circumferential porous coating in the femoral stem induces bone ingrowth and prevents distal osteolysis.

9.Effects of Steroid and Nonsteroidal Anti-inflammatory Drugs on Acute Wound Healing and Collagen Synthesis in Rat Skin.

PURPOSE: We studied short-term effects of steroid and anti-inflammatory drugs on collagen synthesis in rat skin. MATERIALS AND METHODS: The 18 rats were divided equally into three groups: group I; piroxicam, group II; triamcinolone, group III; saline. All drugs were given by intraperitoneal injection. Drugs were administered to rats for 4 days after dorsal skin incision. One day after operation and at 24 hour interval there after, skin tissues containing subcutaneous fat were randomly harvested. Measurement procedures were composed of three steps: the first was Masson's trichrome stain for collagen tissue identification; the second involved transporting the image to a computer and the third the analyses of the collagen area using a morphometry program. RESULTS: Statistically significant differences were found in the mean values of the relative collagen contents of the triamcinolone and saline injection groups on the third day (p<0.05). Relative collagen content of the saline injection group was greater than those of the other groups. The triamcinolone group showed the least collagen content. Collagen content of groups increased in time. CONCLUSION: Steroid had an inhibitory effect on the synthesis of collagen. Anti-inflammatory drugs also reduced the amount of collagen synthesis, but without statistical difference.

10.A Case of the Bleomycin-Induced Bronchiolitis Obliterans Organizing Pneumonia.

A 34-year-old man was admitted to our hospital due to fever and cough. He received the combination anti-cancer chemotherapy for testicular tumor, including bleomycin. The chest X-ray showed consolidation and ground glass opacity on the right upper lobe and subpleural areas of other lobes. This condition was initially misdiagnosed as a pneumonia, but consolidation did not disappear after antibiotics treatment. We performed transbronchial lung biopsy and bleomycin induced pulmonary toxicity was confirmed. The bleomycin induced lung injury is the most common chemotherapeutically induced pulmonary disease. Bleomycin induced Bronchiolitis Obliterans Organizing Pneumonia(BOOP) is less common than interstitial pneumonitis and respond well to corticosteroid treatment.
