1.A Case report of Generalized Pustular Psoriasis Associated with Spondylitis, ARthritis and Glomerulonephritis.

Generalized pustular psoriasis is a severe form of paoriasis and is frequently associated with arthritis and nail deformity. We present a case of generalized pustular psoriasis in a 40 year-old man, with nail change, arthritis of both knee and ankle joints, spondylitis, and glomerulonephritis. We found a positive results of HLA A-9, B-27 in this patient. Skin lesions, arthralgia and hematuria improved marked1y by plasmapheresis and application of topical corticostertoid.

2.An In-vitro Experimental Study on Antibacerial Effects of Antibiotic-mixed Plaster of Paris Pellets

Various kinds of bone cements mixed with antibiotics have been widely used for treatment and prevention of infections, especially combined with bone defects. But bone cements have some disadvantages in that they are rather expensive and capable of incurring some complications such ss fibrosis of surrounding soft tissue or local erosion of bone, and furthermore, they must be removed after infection is controlled for bone graft to fill up the defects because they do not have properties of osteoconduction or osteoinduction. So we have come to the idea of using theoretically advantageous plaster of paris to take place of bone cements, and performed in-vitro experiment upon the effectiveness of plaster of paris pellets containing antibiotics, then obtained good results comparable to those from many experimental studies using bone cements. By this, we think that it would be possible in the future to treat and prevent infections supervening bone defects with plaster of paris mixed up with antibiotics. The results are as follows : 1. Sustained release of large amount of antibiotics from plaster of paris enough to exert inhibitory activity on abcterial growth, and there was a direct relationship between concentrations of eluted antibiotics and diameters of inhibitory zones. 2. Tobramycin was capable of inhibiting the growth of S. aureus, E. coli, and Ps. aeruginosa for 7 days at maximum. 3. Cephradine inhibited the growth of S. aureus and E. coli but did not affect that of Ps. aeruginosa. 4. The maximum antibacterial activity wss reached within first 24 hours from the start of elution of antibiotics from plaster of paris.

3.The Clinical Study of Scabietic Patients.

We had observed 60 patients with scabies in Kangwon province around Wonju area from June to Aug. 1981. The purpose of our present study was the evaluation of the clinical manifestations and laboratory findings in patients with scabies. The results were summarized as follow; 1) The sex ratio of male to female patients was 1. 9: 1 and the most frequent affected age was 10-29 year-old group. 2) Family history was positive in 72.4% of all patients. 3) Ninety per cents of the patients had a history of previous treatment for their kin lesiona, but only 30. 0% of the patients were treated under the diagnosis of scabies. 4) We found the itch mite and it's eggs in 16. 7% of patients and the most frequent detection site was the burrow of interdigital webs, 5) Secondary infection and dermatitis were the most frequent associated complications. 6) The severity of thedisease was correlated with the rate of peripheral eosinophil count. 7) Oniy 4(22.2%) of 18 patients were sensitized to DNCB. 8) The result of VDRL test of the 40 patients whose age were over 17 years old was non-reactive in all.

4.A Case of Carcinoma Erysipelatoides Associated with Carcinoma Telangiectaticum.

Carcinoma of the breast can produce cutaneous metastases which sbow highly diversified clinical pictures. The cutaneous metastases produce four definite clinical types: inflammatory carcinoma, telangiectatic carcinoma, nodular carcinoma, and carcinoma, en cuirasse. We present a case of 78-year-old nulliparous woman who had several painful dark purplish colored zosteriform papulovesicular eruptions, suggestive of herpes zoster, on the right breast, right axilla, scapular area, and upper arm for three month. Two month later, well dermacated linearly arranged erysipeloid eruptions appeared around the previous skin lesions and the right chest wall. Histopathological findings showed metastatic ductal carcinoma with massive vascular permeation but the primary focus was undetermined.

6.The Expressions of Matrix Metalloproteinase-9 and Tissue Inhibitors of Metalloproteinase-1 in Scar Endometriosis and Full Term Endometrium.

OBJECTIVE: Endometriosis in a surgical scar following cesarian section is rare condition. And its pathogenesis has not been well established. The purpose of this study is to observe the expressions of matrix metalloproteinase-9 (MMP-9) and tissue inhibitors of metalloproteinase-1 (TIMP-1) in scar endometriosis and full term endometrium during cesarian section. METHODS: Nineteen scar endometriotic tissue and ten full term pregnant endometrium during cesarian section were investigated by immunohistochemical stain for MMP-9 and TIMP-1. RESULTS: The MMP-9 was expressed in eleven of nineteen scar endometriotic tissue (57.9%) and eight of ten full term pregnant endometrium (80.0%). The TIMP-1 was not expressed in both scar endometriotic tissue and full term pregnant endometrium. CONCLUSION: These results suggest that the relationship between MMP-9 and TIMP-1 might play a role of pathogenesis of scar endometriosis.

9.A Case of Annular Pustular Psoriasis.

Annular pustular psoriasis is a rare variant of the generalized pustular psoriasis characterized by annular lesions with rapidly evolving pustules, associated mild toxicity, and rapid response to therapy. We present a case of annular pustular psoriasis in a 17-year-old male, in whom no previous history of psoriasis was found. The skin lesions were characterized clinically by annular to gyrate erythematous lesions with pinpoint pustules near the periphery on both shoulders, upper arms, and back areas. The histology showed large intraepidermal spongiform pustules. Most of the skin lesions disappeared within 1 week with topical corticosteroid cream.
10.A Case of Left Atrial Myxomoa Presented as Pseudovasculitis.

Left atrial myxoma is well-known mimicker of systemic vasculitis. In general, however, these patients showed some symptoms and signs of cardiovascular disease. We experienced a patient with left atrial myxoma who first presented as vasculitis without any symptoms and signs suggesting cardiovascular disorders. A 45 year-old lady showed tender skin nodules, livedo reticuiaris-like skin lesions, severe constitutional symptoms, multiple cerebral infarctions, and elevated ESR and globulin. Skin biopsy findings were thought to be consistent with polyarteritis nodosa. Echocardiography was performed as a routine test for evaluating suspected vasculitis patients and it revealed left atrial myxoma. However, we were not able to detect any auscultation abnormality, even after echocardiography, After echocardiography, we found the tumor emboli in skin biopy specimen with deeper section. This case demonstrated the importance of suspecting the possibility of left atrial myxoma when performing diagnostic work-up for vasculitis.
