Trichilemmal carcinoma is a rare tumor that is located predominantly on sun-exposed skin of the elderly. It is thought to be related to the external root sheath of the hair follicle and the malignant counterpart of a trichilemmoma. We report a case of trichilemmal carcinoma in a 88-year-old woman who had a 10 year history of a single, dark brown tumor on the right preauricular region. In her past medical history, it had been excised twice at a private clinic without a histopathological evaluation and recurred 2 years later. Histopathological findings showed atypical clear cells resembling those of the outer root sheath. Tumor cells showed PAS-positive cytoplasm. Immunohistochemistry revealed positive high molecular weight cytokeratin expression. To our knowledge, this is the second case of trichilemmal carcinoma in the Korean literature. This study was performed to compare the amount of blood flow from left and right side heart using Doppler echocardiography and Fick method. The pulmonary blood flow (Qp), aortic blood flow (Qs) and Qp/Qs ratio were noninvasively evaluated by two dimensional Doppler echocardiography in 10 patients with ventricular septal defect in children, aged 2 years to 15 years (mean+/-SD=7.1+/-4.9 years). Cardiac catheterization was performed within 2 days after examination of Doppler echocardiography in all patients. Systemic and pulmonary blood flow was calculated by using Fick principle, based on measured oxygen consumption and measured blood oxygen saturation. The Doppler frequency shift was analyzed by computer system combined with Meridian Echocardiography System. Comparison of Qp, Qs and Qp/Qs ratio obtained by invasive methods and by two-dimensional pulsed wave Doppler echocardiography showed a correlation of 0.679 in Qp, 0.710 in Qs and 0.639 in Qp/Qs ratio (p<0.05).

We report two cases of linear focal elastosis. They are a 16-year-old girl and a 14-year-old boy who had had asymptomatic, several, yellow-red, slightly elevated, palpable, striae-like lesions on their middle and lower back since their early childhood. Light microscopic examinations of skin biopsy specimens demonstrated dermal thickening and focal increases of thin wavy fibers in the upper and mid-dermis but no changes in the epidermis. Verhoeff-van Gieson stains for elastic fiber revealed aggregated, clumped, curled, or fragmented elastic fibers.

The authors did clinical analysis for 37 cases, ranging from 5 to 13 year old of age and following result obtained. 1. In group I partial involved type had brightful prognosis even in the neglected the cases. 2. Ischeal brace did not give any help to group II & III but in group I, it gave some help to the prognosis. 3. In untreated cases of old age group developed early arthritic changes. 4. The inital radiological bony changes in early L-C-P. cases was the demineralization of the subchondral area and subsequently subohondral vaccum phenomena followed.

The knee is a weight-bearing joint which is subjected to a great variety of static and dynamic loads. The comminuted fracture of the knee is caused by severe violent trauma, which requires well planned therapeutic procedures because of restoration of joint surface and avoidence of subsequent joint stiffness. authores have experienced 8 cases of various types of comminuted fractures of the knees at the department of orthopedic surgery, The National Medical Center, during the years from 1967 to 1972 They were analized and concluded as following. 1. The mode of the injury was considerably higher in falling type of trauma as 4 out of 8 cases Type III of comminuted fracture of the tibial plateau had higher incidence, 4 out of 5 cases and Grade III type of comminuted fracture of the femoral condyle was 2 out of 3 cases 2. 2 cases out of 8 had complicated injuries, such as peroneal nerve injuries in the Type III of comminuted fracture of the tibial condyle and meniscus injury in 1 case. 3. Conservative treatment was given to 2 cases in 1 of Group 1 and 1 of Group III type of comminuted fracture of femoral condyle, and open reduction & internal fixation was given to 6 cases with satisfactory result in all cases Average healing time was 73.5 days. 4. 1 case was complicated after operation by superficial wound infection but healed up without difficulty.

Partial closure of an epiphyseal plate can reduce growth in length and cause angular deformationof the bone. The bone bridge between the epiphysis and the metaphysis, which inhibits growth, can be removed operatively, leaving a cavity, and improvement of the deformity by normal growth is then possible. Previous study have shown that silastic or bone cement diminished both the angular deformity and growth retardation to a highly significant degree. In the present study we wanted to test whether indomethacin and iliac apophysis as an interposition material could prevent bone bridge recurrence following operative removal in adolescent rabbits. It was designed by the concept that cartilage(iliac apophysis) has a specific inhibitory effect on bone formation and indomethacin inhibits bone resorption and bone formation in both heterotopic and orthotopic bone in rabbits. The following experimental groups were designed. Group 1 (N = 7) control group; no interposition material Group 2 (N = 7) iliac apophysis as an interposition material Group 3 (N = 7) only bone wax as an interposition material Group 4 (N = 7) bone wax with administration of indomethacin 10mg/kg/day The results of the study are as follows: 1. In control group, an osseous bridge were revealed as early as 2 to 3 weeks, and this bridging was found consistently in all control groups for the duration of the study. 2. The gmup using iliac apophysis as interposition material was found ineffective for prevention of the formation of epiphyseometaphyseal bone bridge. It was probably resulted from difficulty of its taking from iliac bone. 3. The group using administration of indomethacin after application of bone wax reduced both the angular deformity and growth retardation to a highly significant degree. From the results, it is recommended administration of indomethacin after insertion of silastic or bone cement as an interposition meterial in the elimination of transphyseal closure after physeal damage.

Six cases of lunate dislocation were treated in the departments of orthopaedic surgery, Hangang Sacred Heart Hospital and Kangnam Sacred Heart Hospital, Hallym College during the period from March 1, 1978 to August 31, 1982. The results were as follows; l. Among the total 6 cases, two cases had lunate dislocation only and the other four had associated injuries of the same wrist joints. 2. Marupulative reduction was successful in only one case. Five cases were treated by open reduction, and for four of them internal fixation was tried. 3. Postoperative roentgenograms showed acceptable reductions in all cases except one which redislocation occurred. This case showed lunate dorsiflexion instability and was treated with extensor carpi radialis longus tenodesis. 4. The cases with pure lunate dislocation yielded better functional recovery than those with associated carpal injuries.

Acute tubular necrosis has been rarely reported as a complication of typhoid fever in the literature. We experienced four cases of acute tubular necrosis associated with typhoid fever in children. Patients showed significant titer of widal reaction associated with acute renal failure and one of them rised in 2 months after onset of clinical symptoms. Renal biopsy findings were compatible with acute tubular necrosis and immune complexes were not deposited in the glomeruli. Clinical and urinary findings were normalized by hemodialysis and antibiotic therapies. In conclusion, close longterm follow up of widal titer is mandatory in acute tubular necrosis, especially when associated with high fever.
