Plasma sprayed coating of the hydroxyapatite(HA) are biocompatible and may contribute to the early fixation of total joint prosthesis. We studied 64 hips(58 patients) who had hydroxyapatite coated primary total hip arthroplasty during the period from January 1992 to June 1993 about clinical and radiological result prospectively. The components have been followed for a minimum of 26 months and an average 31 months. Harris hip score averaged 92 at most recent follow up intervals. At the last follow up, the pain in 7 cases(11%) respectively and only slightly to mild in mostly. Although thigh pain has been occurred in 7 cases(11%), all patients had not trouble in daily activity. In radiological evaluation, there was a rapid and good endosteal bone formation of implants with bone apposition on the Gruen zone 2 and 6 of HA coating area. No radiolucent line formation was detected around HA coated area. In acetabular component, the endosteal bone formation was found in 12 cases and the radiolucent line was occurred in 4 cases. No revision were done for the femoral and acetabular component. The result of the study indicate that this hydroxyapatite coated component can provide satisfactory clinical and radiological result. Further prospective follow up is necessary to determine whether the favorable early result of this implant are maintained over longer period.

Transient hyperkalemia has been reported to occur in eletric burn patients following intravenous administration of succinylcholine. We have studied the origin of the elevated serum K+ concentration after injection of succinylcholine from both the non-burned area and from the burned area in electirc burn patients. Blood samples for serum K+ measurement were drawn 1,2,3,4,5 and 10 minutes after administration of succinylcholine from both the burned are and the non-burned area in 6 electric burn patients. The following results were obtained: 1) Serum K+ levels increased in the burned group in 3 minutes after injection of succinylcholine. 2) The serum K+ concentration of venous samples from the electric burn area were more increased than from the non-burned area injection of succinylcholine.

A case of leiomyoma of the vaginal wall causing voiding difficulty is presented. Although vaginal leiomyoma is rare, it is actually the most common benign tumor found in vagina and the most frequent location is the anterior vaginal wall. Because of the close anatomical relationship with the female urethra and the anterior vaginal wall, the tumor is often responsible for urologic symptom and excision of the lesion is usually performed by urologist.

A clinical observation was made on the 16 cases of the ureteropelvic junction obstruction of the in patient in the Dept. of urology, Fatima hospital during the period from Jan. 1981 to 1985 and the followings were obtained. 1. The patient in this series were distributed from 4 month old to 61 years old and the male female ratio was 3: 1. 2. The obstruction was on the left side in 14 cases, on the right side in 2 cases. 3. The pathologic lesion encountered at ureteropelvic obstruction, are intrinsic ureteropelvic obstruction in 10 cases adhesion and fibrosis in 2 cases, high insertion in 1 case, aberrant vessel in 1 case and unknown in 2 cases. 4. The most presenting symptom was frank pain, affecting 11 cases and palpable mass with urinary tract infection was found 5 cases. 5. The degree of hydronephrosis was determined by urography, retrograde pyelography and ultrasonography, Grade 4 was found in 5 cases, Grade 3 in 5 cases, Grade 2 in 6 cases. 6. On laboratory examination, microscopic pyuria was found in 11 cases, microscopic hematuria in 3 cases, microscopic pyuria and hematuria in 3 cases and BUN was increased in 4 cases. 7. Dismembered pyeloplasty was done in 10 cases, spiral flap operation in 1 case, simple ureteropelvic anastomosis in 1 case and nephrectomy in 2 cases. Preliminary nephrostomy was done 1 case. Of the 12 repairs 10 were protected by nephrostomy drainage plus ureteral stent.

In spite of vigorous efforts to enhance appropriateness of blood usage in surgery, it is recognized that there are still not a few overuse and misuse of blood products in Korean hospitals. To assure appropriate use of blood, continuous monitoring and controlfling blood orders, particularly for surgical operations, should be implemented. The indicator of 'ratio of crossmatching to transfusion' (C/T ratio) has been focalized on in this regard. The authors investigated C/T ratio for eight hospitals, evenly distributed in their geographical location as well as size. Only elective surgeries operated from March 1 to May 31, 1995 were included for analysis. Standardized survey format was distributed, and retrospective reviews of medical records were performed by volunteer surveyors from each hospital after two sessions of pre- survey education. The results were as follows. Average C/T ratio, for all hospitals and all surgeries, was 1.76. Differences in C/T ratio by sex, months, clinical departments, blood components were not significant. However, the ratio showed increasing tendency with ages. The C/T ratio on the average was not higher, compared with the recommended guideline. However, due to study' s limitations ini standardization of survey method and verification of data, we could not conclude that current status of appropriateness of blood use is satisfactory. In addition, to pervasively use the C/T ratio as a quality indicator for blood use management, supplementary measures, such as standardization of data, should be adopted.
