Sleep Medicine and Psychophysiology 2004;11(2):89-94
OBJECTIVES: In the past, latent dreams were emphasized in the psychiatric field, but these days the interest in manifest dreams is increasing as ego psychology develops. Hall and Nordby proposed that there are similarities between manifest dreams and real life. The Hall/Van de Castle System is a method of dream content analysis, which considers both the quantitive and qualitive analytic aspects of manifest dreams. METHODS: The dreams of 232 males and females (M: F=127: 105; mean age=21.02.7) were collected through the Most Recent Dream Method. Collected data were analyzed using the Hall/Van de Castle System. RESULTS: Female subjects tended to be more detailed and meticulous in reporting their dreams. The dreams of male subjects showed a higher percentage in self-negativity (2=6.64, df=1, p=0.004), and the dreams of female subjects showed a higher percentage in group character (2=6.64, df=1, p=0.0099), dreamer-involved success (2=3.12, df=1, p=0.048), and good fortune (2=4.52, df=1, p=0.034). CONCLUSION: This study suggests the norm of dream content of Korean college students, and it presents the differences between Korean males and females, and between Korean college students and American college students. This study may contribute to further studies on dream content analysis.

An automated percutaneous lumbar discectomy(APLD) have been apphed for contained lumbar disc herniation. But suggested that more exclusion criteria than disc containment was needed to improve success rate. The purposes of this study are to evaluate cIinical outcome of more than 4 years follow up of APLD, to analysis the cause of failure for longer follow up period, and to define prognostic factor of APLD. The l04 patients with contained lumbar disc herniation were treated with APLD from March 1990 to November 1992 in National Medical Center. The surgical candidates were contained focal disc herniation in MRI, sciatica than back pain, failure to conservative management at least 6 weeks and clinical and radiological correlation. And patient were excluded from this data if they had history of previous lumbar surgery and compensation claims. Among them 74 cases were followed up for over 4 years (Mean: 5.3 years). The results were accessed by questionnaire using telephone or OPD follow up. The overall success rate was 84% on 3 months follow up, but 68.9% on more than 4 years follow up by four subjective criterias (Onik, 1987). Causes of decreasing success rate were reoperation, recurrence without specific cause or after sprain and heavy work. 76% of failed cases occurred within 1 year and 40% of them underwent open discectomy. The patient sex, treated level, duration of symptom were not influenced on success rate, but age was factor related to success rate. In this study we can assess the effectiveness of APLD within 3 months in most cases and then change treatment option according to patients status, and it seems that we need more exclusion criteria than disc containment in MRI and refined patient selection in order to decrease the failures.

The Journal of the Korean Orthopaedic Association 1977;12(4):839-842
Maroteaux and Lamy coined the term Pycnodysostosis derived from the Greek, Pycnos (thick or dense), Dys (defective), and Ostosis (bone). The principal characters of this syndrome are short stature, dysplasis of the skull, obtuse manibular angle, dysplastic clavicles, partial or total aplasia of the terminal phalanges, and generalized increased roentgenographic density of the skeleton. Patient so affected are predisposed to fracture. Pycnodysostosis seems to be a genetic disorder, inherited as an autosomal recessive trait. Here, it is reported a case of incidental finding of pycondysostosis with chronic renal failure in 12 years old girl.

10.A case of Neuroblasoma with multiple skeletal metastasis

The Journal of the Korean Orthopaedic Association 1977;12(4):805-809
Neuroblastoma is a malignant tumor of sympathetic nerve origin. They are derived from sympathetic formative cells, the primitive neuroblasts that have failed to mature. They may be located wherever sympathetic nerve tissue is found, The majority of cases occured in the suprarenal medulla, posterior mediastinum, and orbit. The tumor metastasized early by the hematogenous or lympathic route, or both to skeletal system, liver, and skin, etc. Since the vast majority of children with neuroblastoma have local or distant metastases at the time of original diagnosis, the clinical manifestation and the prognosis are variable. The age distribution is from birth to 19 years old. Many methods of treatment of neuroblastoma have been advocated such as surgical excision, roentgen thrapy, and its combipation therapy but the effect is questionable. Here, it is reported a case of neuroblastoma with systemic skeletal metastasis originated from posterior mediastinum in 8 years old girl.
