2.Percutaneous Catheter Drainage of Liver Abscess: Clinical Outcome.

PURPOSE: We studied to evaluate the effect of percutaneous catheter drainage (PCD) in liver abscess. MATERIALS AND METHODS: We retrospectively analyzed clinical data (presence of fever, WBC count), radiological findings (computed tomography, ultrasonography and abscessogram) and correlated them with treatment period. Percutaneous drainage of liver abscess were performed in 19 patients under fluoroscopy guide during recent three years. RESULTS: Eighteen patients (95%) were cured, and one patient underwent surgery because of peritonitis caused by PCD procedure. Average treatment period was 17 days. CONCLUSION: In the treatment of liver abscess, PCD is thought to be safe and effective.

4.A case of pemphigus erythematosus.

We present here, a case of pemphigus erythematosus developed in a 31-year-old woman, who had been suffering from repeated episode, of remission and exacerbation with several erythematous, and somewhat oozing patche accompanying a mild pruritus or her face, upper part of the back and anterior chest. In serological studies, she had antinuclear antiboc@lies at. a titer of 1:160. Histopathological finding showed subcorneal bullae. Immunofluorescent study revealed IgG and C, deposition in the intercellular space of the epidermis and along the de rmoepidermal junction.

8.Extracorporeal shock wave lithotripsy as monotherapy of staghorn renal calculi.

Between May, 1987 and July, 1991, 27 renal staghorn calculi were treated with EDAP LT-01 extracorporeal shock wave lithotriptor as monotherapy. Of the staghorn calculi 18 were complete and 9 incomplete staghorn. Of the patients 5 had urinary tract infection before treatment. Stone size in longest diameter was up to 4cm in 37.0% of the cases, 4 to 9cm in 59.2% and longer than 9cm in 3.7%, including complete or partial staghorn stones. Complete response rate was 59.3%, including complete staghorn in 44.4 % and partial staghorn in 14.8%. In the complete response cases, the number of shocks used varied from storage 60 to storage 2770 at 5/sec frequency, 100% adjust power. Complete response rate was not related with hydronephrosis, size of stones and configuration or the stones, but it was related with chemical composition of the stone. In the cases of struvite stones, they needed fewer number of shocks than any other composition for complete response. Of the stones 25.9 % were treated after double-J stents were inserted. Over aI1 25.9% of the patients needed ancillary measures, including ureteroscopic removal in 11.1%, repeated ESWL for steinstrasse in 11.9% and litholapaxy in 3.7%. Of the cases 11 developed post treatment fever and they were treated with antibiotics without clinical problems, therefore urinary tract infection is no longer contraindication of ESWL. The ESWL monotherapy is one of the recommendable treatment method for staghorn calculi.

10.Measurement of the Morphologic Changes of Intercondylar Notch by Magnetic Resonance Imaging.

The purpose of this retrospective study was to evaluate the morphology of the intercondylar notch of the knee in 72 anterior cruciate ligament (ACL) intact group and 30 acute and chronic ACL tear group by plain radiographs and MRI, and to find the predisposing factors of ACL tear. The ACL tear group was divided into acute and chronic ACL tear group. In plain lateral radio-graphs, beta angle, angle between extension line from anterior cortical line of distal femur and from Blumensaat s line, was measured. In magnetic resonance imaging, the width of intercondylar notch (NB), the widest width of both femoral condyle (NW), intercondylar notch width from lower one third point of notch basal line (NB1), intercondylar notch width from upper one third point of notch basal line (NB2), depth of intercondylar notch from notch basal line (ND), and intercondylar angle which made from both end point of notch basal line and apex of intercondylar notch (alpha) were measured. Three groups were compared with each other by statistical analysis. Stastistically, the acute ACL tear group had narrow upper portion of intercondylar notch than chronic ACL tear group and more acute angle of roof of the intercondylar notch than intact ACL group. So in notchplasty, the upper one-third of intercondylar notch should be carefully widened. And after fixation of femoral interference screw, arthroscopic observation should be done whether reconstruced graft touchs the roof of the intercondylar notch during the extension of the knee.
