2.A Case of Umbilical Omphalomesenteric Duct Polyp.

A umbilical omphalomesenteric duct polyp is the result of incomplete closure of the omphalomesenteric duct, which connects the midgut with the yolk sac of the embryo. It may be associated with underlying embryologic anomalies such as Meckels diverticulum and umbilical enteric fistula, the complications of which may at times be fatal. This rare malformation should be clinically discerned from persistent granulation tissue or pyogenic granuloma. Histologically, it shows a polypoid lesion consisting of ectopic gastrointestinal epithelium with the appearance of gastric, intestinal, or colonic mucosa. We report a case of an umbilical omphalomesenteric duct polyp in an 8-year-old male patient, who had had a bright-red polyp on the umbilicus from the age of 1 month and had not had any other types of underlying abnormalities.

3.INFLUENCE OF TOOTH SURFACE ROUGHNESS AND TYPE OF CEMENT ON RETENTION OF COMPLETE CAST CROWNS.

Bond strength of luting cements to dentin is a critical consideration for success of complete cast crowns. This study was performed to evaluate the relationship between surface characteristics of teeth prepared for complete cast crowns and retention of cemented restorations. Eighty artificial crowns were cast for standardized complete crown tooth preparations accomplished with the use of a special device on recently extracted human teeth. Coarse diamond(#102R, Shofu) and superfine finishing diamond(#SF102R, Shofu) burs of similar shape were used. Crowns in each group were randomly subdivided into four subgroups of 10 for luting cements selected for this study: zinc phosphate cement (FLECK'S), polycarboxylate cement (Poly-F), reinforced glass ionomer cement (Fuji PLUS), and adhesive resin cement (Panavia 21). Retention was evaluated by measuring the tensile load required to dislodge the artificial crown from tooth preparations with an Instron testing machine, and analysed by one-way ANOVA and Student's t-test. The obtained results were as follows ; 1. When tooth preparation was done with coarse diamond bur, retentive force was diminished in order of Panavia 21, Fuji PLUS, FLECK'S, and Poly-F. Retentive forces showed the significant difference between Fuji PLUS group and FLECK'S group(p<0.001). 2. When tooth preparation was done with superfine diamond bur, retentive force was diminished in order of Fuji PLUS, Panavia 21, FLECK'S and Poly-F. Retentive forces showed the significant difference between Panavia 21 group and FLECK'S group(p<0.001). 3. Retentive force in coarse tooth surfaces was significantly higher than that in superfine tooth surface with all luting cements(p<0.001), and cement residues were almost retained with in the cast crown in all groups.

4.Two Cases of Segmental Neurofibromatosis.

Neurofibromatosis(NF) is a disorder characterized by its relative commonness, variability, and heterogeneity. It is usually expressed as a generalized form, but is rarely localized in a limited part of the body as a segmental form. In 1981, Riccardi classified NF into 8 types and a segmental form (type V) was defined by limitation of cafe-au-lait spots and/or neurofibroma on a given unilateral segment of the body. Recently we came across two cases of typical segmental neurofibromatosis. The First case was a 53-year-old woman with localized neurofibromatosis on the right T8,9 dermatome. The Second case was a 34-year-old man with localized neurofibromatosis on the right T5,6 dermatome. There were no cafe-au-lait spots, axillary frecklings, Lisch nodules or a family history in both cases. We report these rare cases with a literature review.

5.A Study on Changes of Serum HDL-Cholesterol Level in Some Diseases.

A study on changes of serum HDL-cholesterol level in various diseased states was performed. Diseased groups were nephrotic syndrome, liver cirrhosis, hyperthyroidism, hypothyroidism and chronic alcoholism. Following results were obtained. 1) Compared to normal control group, levels of HDL-cholesterol in nephrotic syndrome were lower but the difference was significant statistically. 2) In liver cirrhosis, the result showed much lower levels than normal control group with high significance. 3) In both hyper-and hypothyroidism group, the levels of HDL-cholesterol were significantly lower than normal control group. 4) In chronic alcoholism, the levels were significantly higher than normal control group. 5) Except for the group of chronic alcoholism, the levels of HDL- cholesterol were higher in female groups than male groups, but was of no significance statistically.

7.Understanding Nutritional Support in Digestive Diseases.

The prevalence of hospital malnutrition is still high in patients with digestive diseases, especially for those suffering from cancer and bowel diseases which cause malabsorption. It is well known that malnutrition is associated with delayed wound healing, impaired immunity, infection, increased complication, and poor convalenscence. Recently, nutrition screening and assessment by nutrition support team has become essential for nutrition management, and gastroenterologists comprise a dominant member of the nutrition support team. In critically ill patients and older people with chronic disease, nutritional support with enteral feeding and early feeding contributes to recovery and rehabilitation of patients. Securing enteral feeding routes, such as feeding tube insertion and placement of percutaneous endoscopic gastrostomy/jejunostomy, is an essential part of nutrition care that should be accomplished by gastroenterologists without much difficulty. It will also be necessary to recommend nutrition care as one of the clinical routines in gastrointestinal clinical practices. Therefore, education on nutrition care is strongly required as a part of gastroenterologist's training.

9.A Clinical Result of Pedicle Screw Fixation in Osteoporotic Spine: Complications and Prevention.

STUDY DESIGN: Retrospective study. OBJECTIVES: To analyze the clinical results of patients who were treated by pedicle screw fixation in osteoporotic spine and suggest the methods for preventing a loss of fixation strength. SUMMARY OF THE LITERATURE REVIEW: There are some methods to decrease failure rate and increase fixation strength in the osteoporotic spine: use bicortical screw, cement augmentation and supporting anterior column by interbody fusion using cages. MATERIALS AND METHODS: Forty-four patients treated by spinal instrumentation using pedicle screw from 2004 to 2011 were followed for at least 12 months. Five men and 39 women were diagnosed as osteoporotic spine (T score <-3.0). Two hundred forty eight pedicle screws were included and statistically analyzed the correlation between the use of bicortical screw, cement augmentation, anterior column support and fixation loss of the pedicle screw. Radiologic results were evaluated to find out the mechanical complications, like loosening of the screw, fixation failure, and nonunion. RESULTS: There were 9 complications associated with mechanical strength, loosening of pedicle screws in 7, sinking down of cage in 4, and nonunion in 4 cases. Using bicortical pedicle screw, cement augmentation and anterior column support were significantly correlated with the increasing fixation strength (P=0.001, P=0.047, P=0.014). In addition, these three factors contribute to stabilize the instrumentation (Linear by linear association, P=0.012). CONCLUSIONS: These 3 methods, using bicortical pedicle screw, cement augmentation and supporting anterior column, are effective to enhance the fixation strength and prevent loss of holding power in the osteoporotic spine.

10.Metal Failure of Pedicle Screw System.

STUDY DESIGN: The metal failure of pedicle screw system followed by posterolateral or posterior fusion used in variable cases(spine fracture, degenerative disorder of spine, deformity of spine) was analyzed retrospectively. PURPOSE: The goal of this study was to analyze frequency and clinical consequence of pedicle screw fixation system failure in the treatment of different etiology(spine fracture, degenerative disorder, deformity of spine) and to evaluate affected factors in metal failure. MATERIALS AND METHODS: We performed survivorship analysis on 442 patients treated with pedicle screw system from September 1990 to December 1999. The average follow-up period was 54 months(from 18 months to 129 months). As affected factors, some variables such as etiology, kinds of system and extent of fusion were subjected to analyzed their influence on metal failure. We also performed analysis about relationship between metal failure and clinical results. We defined the metal failure as 1) breakage of screw or rod 2) screw bending above 5 degrees 3) dissociation of rod-screw coupling system and 4) screw pull out from vertebral body or pedicle. RESULTS: We found out 33 cases of metal failure: among 2786 screws, 41 screws had a problem. The metal failure rate was different between each etiology ; 12 cases in fracture(10%), 21 cases in degenerative disorder(6.9%). There was also difference between a kinds of implants; 13 cases in side assembling type(5.4%), 20 cases in back open type(10.5%). However, there were no difference according to extent of fusion; 6 cases in one segment(6.9%), 21 cases in two segments(8.1%), 6 cases in more than three segments(6.3%). Among the overall patients with metal failure (33 cases), only eight patients were complaint significant symptoms. And three of this eight patients were improved after reoperation The mean interval to metal failure was 14.4 months from operation. CONCLUSION: The metal failure was more common in spine fracture(p<0.05) and back open type pedicle system(p<0.05). However, there was no relationship with extent of fusion(p>0.05). And metal failure did not significantly affect the clinical results(p>0.05).
