3.Clinical assessment of neonatal transient tricuspid insufficiency: Doppler echocardiographic study.

Eight neonates with transient tricuspid insufficiency are presented which was confirmed clinical and two dimensional echocardiographic assessment. We found that two dimensional Doppler echocardiography was very useful in the detection of transient tricuspid insufficiency during neonatal age as noninvasive method. Transient tricuspid insufficiency is a clinical disorder in the newborn period caused by myocardial dysfunction, secondary to asphyxia with or without hypoglycemia and associated with right ventricular overloading caused by pulmonary hypertention. The clinical diagnosis was based on a history of perinatal distress, distinctive murmur, ECG changes, biochemical abnormalities and myocardial imaging. 1) The sex ratio of TTI was 1:1. 2) The average gestational age was 34 weeks and mean body weight was 2.06 Kg, respectably. 3) Major symptoms were dyspnea, cyanosis, and tachypnea. 4) Tricuspid regurgitation was detected from the lst day to the 4th day of the life and was improved from the 7th day to the 30th day of the life. 5) The peak velocity through tricuspid valve ranged from the 2.7 m/sec to 4.0 m/sec and the estimated right ventricular pressure ranged from 39 mmHg to 74 mmHg. 6) Associated diseases were neonatal hyperbilirubinemia (100%), prematurity (87.5%), atrial right to left shunt (87.5%), patent ductus arteriosus (75%), hyaline membrane disease (25%), and transient tachypnea of newborn (12.5%).

4.Evaluating Scapular Notching after Reverse Total Shoulder Arthroplasty.

BACKGROUND: Scapular notching can happen at diverse location depending on implant design or operative technique, therefore, it is easily misdiagnosed. Thus, this study purposed to suggest a method helpful to assess scapular notching. METHODS: The subjects were 73 cases of reverse shoulder arthroplasty (RSA) for cuff tear arthropathy during the period from May 2009 to April 2014 and followed-up for over a year. There was medialized RSA in 22 cases, bone increased offset RSA (BIO-RSA) in 36 cases, and metal increased offset RSA (metal-RSA) in 15 cases. Scapular notching was not determined by bone defect at the inferior of glenosphere as Sirveaux's classification, but scapular notching at the site where the rotational route of the polyethylene of humeral implant met the scapular neck were examined. The results were compared with conventional method. RESULTS: By conventional method, scapular notching was observed in 10 cases (45.5%) in medialized RSA, 12 cases (33.3%) in BIO-RSA, and none in metal-RSA. By new method, it was observed in 9 cases (40.9%) in medialized RSA, 10 cases (27.8%) in BIO-RSA, and none of metal-RSA. The site of scapular notching was apart from glenoshpere in 18 cases, and at inferior of glenosphere in 1 case. Absorption of bone graft was observed in 4 (11.1%) out of 36 cases of BIO-RSA. CONCLUSIONS: It is hard to distinguish scapular notching from absorption of bone graft in BIO-RSA, and bone absorption at the lateral lower end of glenoid in medialized RSA. Thus, it is considered useful to assess scapular notching at the site where the rotational route of the polyethylene insert meets scapular neck.

5.INFLUENCE OF TOOTH SURFACE ROUGHNESS AND TYPE OF CEMENT ON RETENTION OF COMPLETE CAST CROWNS.

Bond strength of luting cements to dentin is a critical consideration for success of complete cast crowns. This study was performed to evaluate the relationship between surface characteristics of teeth prepared for complete cast crowns and retention of cemented restorations. Eighty artificial crowns were cast for standardized complete crown tooth preparations accomplished with the use of a special device on recently extracted human teeth. Coarse diamond(#102R, Shofu) and superfine finishing diamond(#SF102R, Shofu) burs of similar shape were used. Crowns in each group were randomly subdivided into four subgroups of 10 for luting cements selected for this study: zinc phosphate cement (FLECK'S), polycarboxylate cement (Poly-F), reinforced glass ionomer cement (Fuji PLUS), and adhesive resin cement (Panavia 21). Retention was evaluated by measuring the tensile load required to dislodge the artificial crown from tooth preparations with an Instron testing machine, and analysed by one-way ANOVA and Student's t-test. The obtained results were as follows ; 1. When tooth preparation was done with coarse diamond bur, retentive force was diminished in order of Panavia 21, Fuji PLUS, FLECK'S, and Poly-F. Retentive forces showed the significant difference between Fuji PLUS group and FLECK'S group(p<0.001). 2. When tooth preparation was done with superfine diamond bur, retentive force was diminished in order of Fuji PLUS, Panavia 21, FLECK'S and Poly-F. Retentive forces showed the significant difference between Panavia 21 group and FLECK'S group(p<0.001). 3. Retentive force in coarse tooth surfaces was significantly higher than that in superfine tooth surface with all luting cements(p<0.001), and cement residues were almost retained with in the cast crown in all groups.

7.The study of serum lipids in grossly obese elementary school students in Seoul.

Our study surveded 418 obese first grade elementary school children(279 male and 139 female), in seoul, and measured levels of serum total cholesterol, HDL cholesterol, LDL cholesterol, triglyceride were measured in addition to the measurement of body weight and height. The following results were obtained. 1) Obesity index was catergorized into mild, moderate, and severe, and the percentage of mild was 6.8%, moderate 48%, severe 45.7% respectively in surveyed children. 2) The average level of total cholesterol was 1.66.60 27.63mg/dl, HDL cholesterol 50.22+/-0.68mg/dl, LDL cholesterol 82.23+/-1.26mg/dl. 3) Atherogenic index was calculated, and obtained value was 2.45+/-0.48. It showed higher statstical significance in boys than in girls(p<0.05). 4) There was no significant difference in average level of total cholesterol, LDL cholesterol, and triglyceride in accordance of the severity of obesity. 5) Statistical significance was found in different average ievels of HDL cholesterol and atherogenic index according to obesity index (p<0.05). 6) There was a statistical significance in the average values of Kaup index and Rohrer index in accordance with obesity index (p<0.000). 7) There was no correlation between abnormal levels of total cholesterol, HDL cholesterol, and LDL cholesterol and the severity of obesity. 8) Abnormal levels of triglyceride, atherogenic index, Rohrer index and Kaup index had a statistical significance since their levels. 9) There was a correlation between atherogenic index and body measurement. Total cholesterol level showed correlation with obesity index, and HDL cholesterol with Kaup index and arm circum ference.

9.Observation of the Serum Uric Acid in Essential Hypertension.

We studied serum uric acid in 120 patients of essential hypertension visiting Department of Internal Medicine of Seoul Red Cross Hospital from October 1985 to November 1986 and analyzed its values in terms of various clinical and laboratory parameters. The results wereas follows; 1) The mean concentration of serum uric acid and incidence of hyperuricemia were 6.5+/-1.86mg%, 50.8% in hypertension group and those of normal control were 3.75+/-1.27mg%, 13.3%, respectively with significant high values in hypertension group. 2) The mean concentration of serum uric acid and the incidence of hyperuricemia were significantly correlated with the levels of diastolic blood pressure but there was only correlation between the incidence of hyperuricemia and the level of systolic pressure. 3) The mean concentration of serum uric acid and the incidence of hyperuricemia were significantly correlated with the level of BUN and serum creatinine, especially more in serum creatinine. 4) The mean concentration of serum uric acid and the incidence of hyperuricemia were not correlated with each level of serum cholesterol and triglyceride. 5) There was significant correlation between mean concentration of serum uric acid and abnormal urine finding but not in incidence of hyperuricemea.

10.A Study on Changes of Serum HDL-Cholesterol Level in Some Diseases.

A study on changes of serum HDL-cholesterol level in various diseased states was performed. Diseased groups were nephrotic syndrome, liver cirrhosis, hyperthyroidism, hypothyroidism and chronic alcoholism. Following results were obtained. 1) Compared to normal control group, levels of HDL-cholesterol in nephrotic syndrome were lower but the difference was significant statistically. 2) In liver cirrhosis, the result showed much lower levels than normal control group with high significance. 3) In both hyper-and hypothyroidism group, the levels of HDL-cholesterol were significantly lower than normal control group. 4) In chronic alcoholism, the levels were significantly higher than normal control group. 5) Except for the group of chronic alcoholism, the levels of HDL- cholesterol were higher in female groups than male groups, but was of no significance statistically.
