2.Bilateral Naviculo-Medial Cuneiform Coalition: One Case Report

We report one case of bilateral naviculo-medial cuneiform coalition in ten year old girl. She complained of pain in the medial aspect of the mid-foot related to long periods of standing and activity. Routine roentgenographs, specifically lateral views showed a bony bridge between the navicular bone and medial cuneiform. A biopsy revealed as fibrocartilaginous tissue. The bony bridge consists about one third in lateral view and 30 degree cephalad tilting in the antero-posterior view. Bilateral resection arthroplasty, with interposing flesh muscle fibers of the abductor hallucis, was performed for restoration of motion in the naviculo-medial cuneiform joint and relief of localized pain. Four months after surgery, pain around the mid-foot had almost complete subsided. One year postoperative, neither recurrence nor disability was observed. We will discuss the diagnosis and treatment of this rare case of bilateral naviculo-medial cuneiform coalition.

9.Factors Influencing Fatigue in Elderly People with Chronic Pain.

PURPOSE: This study was done to examine fatigue in elderly people with chronic pain and identify factors influencing fatigue. METHODS: Participants in the study were 296 elders suffering from pain for more than 6 months. The data were collected by personal interviews using questionnaires and were analyzed using Pearson correlation coefficients, t-test, ANOVA, and stepwise multiple regression with SPSS/WIN 15.0 version. RESULTS: The mean score for fatigue was 30.5, and fatigue showed a significant positive correlation with depression (r=.57, p<.001), perceived stress (r=.12, p=.035) and pain disability (r=.52, p<.001), but a negative correlation with social support (r=-.21, p<.001) and ADL (r=-.22, p<.001). Depression was found to be a significant predictor of fatigue. CONCLUSION: The results of this study show that psychological factors like depression have more influence on fatigue in elders than physical activities or daily capability. Therefore, it is important to consider psychological factors which influence fatigue as well as the fatigue itself in order to efficiently control fatigue.

10.Factors Influencing the Serum Bilirubin Level in Air Force Pilots.

BACKGROUND: The Republic of Korea Air Force (ROKAF) makes its pilots undergo regular aero-medical checkups. About 15% of the pilots undergoing this check-up show increased serum bilirubin levels without any abnormality in other liver function test items, and even reexamination is required for those with a marked increase. In this study, we investigated the factors associated with increased serum bilirubin. METHODS: Questionnaires and blood tests were used for the ROKAF pilots who visited the Aerospace Medical Center for the aero-medical check-up. Various information from the questionnaire and blood test was statistically analyzed to identify the factors associated with increased bilirubin. RESULTS: In univariate analysis, age, fasting > or =12 hours, exercise in the last 3 days, and last alcohol intake > or = 120 g were statistically associated with increased serum bilirubin. In multivariate analysis, fasting > or =12 hours, exercise in the last 3 days, and last alcohol intake > or = 120 g were independent risk factors significantly associated with increased bilirubin. CONCLUSION: Prolonged fasting, recent exercise and drinking were associated with increased serum bilirubin as previously known. Pilots expecting regular aero-medical check-up should avoid intense exercise and drinking in order to get accurate blood test results.
