Chronic wound has very complicated pathogenesis and is difficult to treat; the situation has been improving owing to the progress in wound healing research and the clinical application of new techniques. This article focuses on the pathogenesis of chronic wound and introduces progress in various kinds of treatment, such as skin substitute, growth factors, negative pressure treatment, etc.

Andrology has a history of less than 3 decades in China, while TCM gynecology has developed for over a thousand years. 3.Treatment of femur periprosthetic fracture after total hip arthroplasty in elderly patient

ObjectiveTo discuss treatment experiencing in femur periprothetic fracture after hip arthroplasty in pre-, intra- and post-surgery.Methods From March 2005 to May 2011, 27 patients (11 males, 16 females) aged >75 years with femur periprothetic fracture after hip replacement were studied. The mean time from primary hip replacement to femur perprosthetic fracture was 63 months(ranging from 7 to 83 months). According to vanconver classification, type A applying steel wire of character for 8 bind technique was 5 cases, type B applying prolonging stem technique locking compress plate (LCP)technique surrounding instrument technique was 19 cases and type C applying bone graft and plate fixation technique was 3 cases. All patients were followed up from 9 to 25 months (average, 16 months). The results of treatment were evaluated by Harris score and X rays. Results Bony heal was obtained in 22 patients and was not obtained in five cases among whom bony heal occurred in 3 cases and 2 patients could only keep prothetic and limb stable after receiving the bone graft once more. Walking was free in 14 cases,9 cases needed assistant walking instrument,and only 4 cases stayed in the bed freely. Harris score for last follow-up was 52 to 93 (mean 76).Conclusions Based on the patient general status, the fracture types and practical and stable principle, the effective methods can get good results in the treatment of femur periprosthetic fracture in aged patient.
4.Health information social service in medical college and university libraries and measures for its improve-ment

5.Role of functional abnormality of sphincter of Oddi on formation of intrahepatic calculi in rabbits

Objective To investigate the relationship between motor abnormality of sphincter of Oddi(SO) and hepatolithiasis.Methods The hepatolithiasis rabbit models were established in adult rabbits(n=64) which were randomly divded into 2 groups:control group(n=16)and experimental group(n=48),including 2 wk,4 wk and 8 wk group(n=16 per group).During the formation process of hepatolithiasis,bile character and ratio of stone formation were both observed,then the myoelectric waves of SO were recorded.Finally,The SO segment was sliced and observed by transmissional electron microscope.Results The rate of true stone formation in group 8 wk(68.75%,11/16)was higher than that in group 4wk(12.50%,2/16)and in group 2wk(0.00%,0/16).Compared with the control group,there was significant decrease of both SO myoelectric peak amplitude and SO myoelectric frequency in the experimental group.Electron microscopy showed that SO cells in control group and 2wk group had no change,but the 4wk and 8wk groups underwent changes including fibrolysis of myofilament,increase of collagen fibers,fusion and thickening of microfilament,decrease and array disorder of macula densa,swelling of chondrosome and endoplasmic reticulum,widening of perinuclear space and deviation and pyknosis of nucleus,and formation of inclusion body in cytoplasm;and the degeneration degree in 8wk group was obvious than that in group 4wk.Conclusions The functional abnormality of SO may contribute important effects on the formation of hepatolithiasis.Fibrous degeneration of SO maybe the pathogenesis of functional abnormality of SO.

10.The changes of serum bilirubin level in elderly patients with acute cerebral infarction and its significance

Objective To investigate the changes of serum bilirubin level in elderly patients with acute cerebral infarction and its significance.Methods 164 hospitalized elderly patients,who suffered from acute cerebral infarction within 1 week after onset,were divided into 2 groups according to age:group A aged over 60 years(n=85) and group B aged 40-60 years(n=79),and 66 healthy subjects aged over 40 years were collected as controls(group C).Serum bilirubin levels in all subjects were determined.The ratio of pulse pressure over mean arterial pressure(PP/MAP) in group A and B was calculated.Nerve function scores in the three groups were detected before and after 2 weeks of treatment.Meanwhile,the data of risk factors including blood glucose,blood pressure,blood lipids,smoking and drinking in group A and B were collected.Results Compared with group C,serum total,direct,indirect bilirubin levels were increased in group A and B(both P<0.01),and the change was smaller in group A than in group B(P<0.05).The nerve function scores was lower in group A than in group B before and after treatment [(35.2±12.6) vs.(44.3±7.9),(40.7±9.1) vs.(51.3± 4.1),t=5.58,9.73,both P<0.01],but PP/MAP and the numbers of risk factors were higher in group A than in group B [(0.46±0.06) vs.(0.38±0.06),93.01 vs.71.20,both P<0.01].There were no significant correlations of serum total,direct and indirect bilirubin levels with nerve function scores in group A or B(all P>0.05).Conclusions Serum bilirubin level is increased in patients with acute cerebral infarction,but the endogenous antioxidant capacity is decreased because of aging,multiple risk factors and more serious atherosclerosis in elderly patients,and the increment of bilirubin level is relatively smaller in acute cerebral ischemia,leading to the reduced protective effect against stress.Serum bilirubin level may influence the prognosis in patients with acute cerebral infarction.
