1.Analysis on the occurrence of malignant tumors after kidney transplantation

Objective To study the incidence of malignant tumors in the patients undergone renal transplantation, and to explore the mechanism of higher incidence. Methods A retrospective study was performed on 829 patients undergone renal transplantation followed by immunosuppression therapy from 1998-2006. 15 cases developed malignant tumors among the 829 patients with incidence of 1.9%. The incidence of urinary system tumor was 66.7%, of hepatoma was 13.3%, and 6.7% for lymphoma, cutaneous cancer and pulmonary carcinoma respectively. Results 15 patients suffered from renal tumors 11-73 months after kidney transplantation, in whom renal function was normal in 14 patients, and in 1 patient the grafted kidney showed dysfunction. No tumor metastasis was found in those cases, except in 3 patients ureteral lumen infiltration was found. Most patients accepted operations, with the dose of the immuosurpression drugs adjusted or reduced, and supplemented with chemotherapy or rediotherapy and immunotherapy. Conclusions The pathogenesis might be related with immunosuppression drugs postoperatively, and also the effects of transplantation itself and the primary disease before transplantation, for instance, uroepithelium malignancy was prolonged medication was frequently the reason for renal transplantation.

Objective To observe the effect and study intervenient action of Kuijieling Granules (KG) in treating ulcerative colitis (UC) during the active period by the clinical experiment. Methods The patients of active UC (damp-heat syndrome) were randomly divided into 2 groups (KG+SASP group and SASP group) to observe pathological changes of the mucosa,syndrome effect and main symptoms scores,and the protein expression of TLR4,CD14 and NF-?B p65 were detected by immunohistochemistry. 4.AFLP Analysis on genetic diversity of germplasm resource in Dryopteris fragrans

Objective To determine the genetic diversity of germplasm resource in Dryopteris fragrans by amplified fragment length polymorphism(AFLP) technique.Methods Forty-six samples from six populations of D.fragrans were analyzed by AFLP DNA markers,and the genetic diversity was evaluated by NTSYS-PC 2.10 and PopGen32.Results The percentage of polymorphic bands(PPB) reached to 76.67%,observed number of alleles(Na) was 1.766 7, effective number of alleles(Ne) was 1.504 6,Nei′s gene diversity index(H) was 0.292 1,Shannon information index(I) was 0.431 6,and genetic differentiation index(Gst) was 0.412 5.Conclusion Genetic diversity is high,gene diversity in populations(Hs) is 0.170 5,and gene diversity among populations(Dst) is 0.119 8.Because of environmental specificity,its resource on the spot should be protected.
5.Exploration and application of nursing ethical decisions path based on clinical case

Objective To explore the effective procedures and practices to solve the ethical dilemma of nursing, and to building nursing ethics decision path. Methods Using inductive-deductive method, summarize predecessors′ research on nursing ethics mode and decision-making process, and establish nursing ethical decisions paths, then display the application based on clinical case. Results Six-steps-Nursing ethics decision path was established, and tested. Conclusions Pay attention to strengthen the cultivation of nurses nursing ethics accomplishment and ability of the decision.
6.Value of exercise stress test guiding aerobic exercise rehabilitation in patients with myocardial infarction

Objective: To explore value of exercise stress test guiding aerobic exercise rehabilitation in patients with myocardial infarction.Methods: A total of 102 patients with myocardial infarction in recovery were selected.According to random number table, they were randomly and equally divided into routine rehabilitation group (received routine rehabilitation guidance) and aerobic exercise rehabilitation group (received aerobic rehabilitation exercise in guidance of exercise stress test).Amount of exercise, hemodynamic parameters and psychological state were compared between two groups.Results: After treatment, amount of exercise: compared with routine rehabilitation group, there were significant rise in amount of exercise[(6.79±0.58)METs vs.(7.42±0.69)METs], maximum heart rate[(126.27±5.26) beats/min vs.(138.18±5.81) beats/min], heart rate blood pressure product[(221.87±21.46) vs.(244.85±23.09)]and exercise duration[(10.36±1.36) min vs.(11.18±1.45) min]in aerobic rehabilitation group, P<0.05 or <0.01;hemodynamic parameters: compared with routine rehabilitation group, there were significant rise in cardiac output[(5.36±0.57) L/min vs.(5.72±0.68) L/min], cardiac index[(2.96±0.53) L min-1 m-2 vs.(3.25±0.52) L min-1 m-2], stroke volume[(73.16±8.47) ml vs.(78.12±9.14) ml]in aerobic exercise rehabilitation group, P<0.05 all;psychological state: compared with routine rehabilitation group, there were significant reductions in scores of Hamilton rating scale for anxiety[HAMA, (9.74±3.26) scores vs.(6.35±1.18) scores]and Hamilton rating scale for depression[HAMD, (14.83±4.58) scores vs.(10.56±3.27) scores]in aerobic exercise rehabilitation group, P<0.05 both.Conclusion: Exercise stress test guiding aerobic exercise rehabilitation can help to improve exercise function and hemodynamic indexes, alleviate the negative mood, then promote patients recover.
7.Advances on the methods of enhancing the immunological functions of exosomes

Exosomes are membrane vesicles released into the extracellular environment upon exocytic fusion of multivesicular bodies with the cell surface. As a result of initial studies showing that exosomes display signal and immune factors,a particular interest has emerged in their use as a cellular vehicles for stimulation of immune responses in vivo.In recent years,on the base of the knowledge of the biological properties and functions, exosomes have been rebuilt and bound to different kinds of immunological stimulators as a result to enhanced antitumor funotions.In this paper,the related methods,immunological stimulators and the properties of man-made exosomes are mainly reviewed.

Objective To study the effects of naked DN A encoding vascular endothelial growth factor 165 (VEGF 165 )on cerebra l infarction in rats. Methods Following establishme nt of a permanent middle cerebral artery occlusion (MACO) model by nylon suture embolization in Wistar rats, the puCCAGGS/hVEGF 165 was directly injected i nto the ischemic tissues through skull hole. Seven days later, the rats were sac rificed. 9.Expression and analysis of recombinant human prothrombin-2 in Pichia pastoris

Objective To prepare recombinant human prothrombin-2 expressed in Pichia pastoris, and assay the enzymatic and clotting activities of prothrombin-2 activated by prothrombin activator ecarin.Methods Human prothrombin-2 gene and Echis carinatus ecarin gene were synthesized separately on the basis of the cDNA sequences published in GenBank.The gene of prothrombin-2 was cloned into the expression vector pPICZαA.The expression vector pPICZαA/prothrombin-2 was transformed into glycoengineered P.pastoris, and then prothrombin-2 engineered P.pastoris was screened.The expression products were induced by methanol, purified by two-step chromatography and identified by diges-tion by PNGase F and analysis of pepetide fingerprint.The ecarin gene was cloned into the expression vector pcDNA3.1. The expression vector pcDNA3.1/Ecarin was transformed into HEK 293T cells and the culture supernatant of HEK 293T/Ecarin was collected.The reaction product of HEK 293T/Ecarin cell culture supernatant and purified prothrombin-2 was analyzed by S-2238,which was the chromogenic substrate for thrombin.Fibrinogen was used to measure blood clotting time. Results The purified protein of P.pastoris expressed prothrombin-2 culture supernatant was 37 ×103 .The relative molecular mass(Mr) of the purified protein was reduced to 35 ×103, which was consistent with the theoretical Mr of prothrombin-2 molecular weight.The purified protein was proved to be prothrombin-2 by peptide fingerprint identification. The purified prothrombin-2 processed by HEK 293T/Ecarin culture supernatant could hydrolyze S-2238 to produce yellow pNA, and D405 of pNA increased with the volume of the processed prothrombin-2 that could promote the plasma coagulation.The blood clotting time was close to that of the thrombin kit.Conclusion Prothrombin-2 is prepared by P.pastoris and activated toα-thrombin by ecarin.This technique may replace the method of extraction of prothrombin from plasma and can be used for the treatment of war wounds or for future clinical research.
10.Long-term effectiveness of treatment to chronic lymphocytic leukemia at different stage

Objective To investigate the clinical efficacy of different treatment in 35 chronic lymphocytic leukemia (CLL) patients. Methods Patients were treated with different regimen according to Binet stage. Patients at stage A were subcutaneously injected with interferon (3-6) MU/day, consecutive for 5 days every week. The dosage could be reduced to 2-3 times a week in long term maintenance phase after the 6 months loading treatment if there was no disease progression. Those at stage B or C were initially treated with chemotherapy regimen of FC/FC -R or CHOP/COP, and interferon were administered during chemotherapy interval, after complete remission (CR) or partially remission (PR) as maintenance therapy.Results Twenty patients were at stage A and treated with interferon, with 5 patients(25%) achieving partial remission (PR), 14 patients at stable status while no patients acquiring CR. Three of the 5 patients who achieved PR collapsed after 36.3 months at average. Eight of the 14 patients at stable status deteriorated to stage B and received chemotherapy after mean 74 months interferon maintenance treatment. In total, 27 patients in the current observation were finally included at stage B or C. Patients at stage B or C in FC/FC-R chemotherapy regimen achieved CR at 38.9% and total effective rate 77.8%, which were superior to that of CHOP/COP prescription (CR 11.1 %). The mean survival time for patients at stage A, B and C were 155.2,97.5 and 82.9 months, respectively and were statistically significant via Kaplan-Meier analysis method (P =0.032). Ten patients died in this observation, 2 at stage A, 4 at stage B and C, respectively, among whom 9died of infection and 1 for gastric cancer bleeding. The side effects of interferon were generally mild during the long term treatment. Conclusion Patients with CLL need to be individually treated with different regimen by considering disease stage and other prognosis criteria. Interferon could be applied at early phase of CLL and may reduce occurrence of infection after long term treatment.
