1.Physical Gowth According to Sexual Maturation of Korean Adolescents.

Adolescence is the period in which physical, mental and social maturation occurs and it is an important transit stage, changing from childhood to adulthood. This is a period in which important changes occur physically such as rapid growth in height and weight gain but significant differences in growth may occur for the same age. Therfore, the degree of sexual maturation may be an important indicator in evaluating growth in adolescence. Six thousand five hundred and eighty six male and female students from grade 4 to senior high from Seoul, district town and countryside were evaluated for sexual maturity using Tanner staging, weight, height and body mass index and compared for growth. There was a rapid growth in height of 6.7cm in boys between the ages of 11 and 14 years and 6.3 cm in height for girls between the ages of 10 and 12 years, showing earlier growth in height for girls. Growth in height according to sexual maturation showed the greatest growth during stages 2~3 for boys and girls. Boys grew an average of 18.94 cm and girls an average of 18.03 cm in height during the whole sexual maturation period. There was an increase of 5.5 kg in weight for boys during the same age as for growth in height, that is between 11 and 14 years of age. In girls, an average weight gain of 5.1 kg occurred between 11 and 13 years of age, slightly later than that of growth in height. Rapid gain in weight occurred during Tanner stages 2~3 for girls. Total gain of 20.70 kg for boys occurred during the whole sexual maturation period while girls gained total 19.73 kg. Body mass index increased gradually during the whole adolescent period. It was greater in boys during the ages of 9 and 12 years and greater in girls the age of 13 years and again in boys after 17 years of age. During the whole maturation period, boys gained 3.24 and girls, 4.05 of body mass index showing more gain in girls. Height, weight and body mass index showed normal variations during the sexual maturation index showed normal variations during the sexual maturation period and the results may bring an increased understanding that adolescent physical growth was closely related to sexual maturation.

2.Molecular Diagnostic Test for Prader-Willi Syndrome with SNRPN Expression.

PURPOSE: Prader-Willi Syndrome(PWS) is caused by absence of paternal contributions of the chromosome region 15q11-q13. To detact this region, high resolutional cytogenetic analysis, FISH with probe at PWS critical region or microsatellite polymorphism can be used. The gene for the small nuclear ribonucleoprotein polypeptide N(SNRPN) is not expressed in patients with PWS. We conducted molecular analysis with RT-PCR with SNRPN primers to find out more useful diagnostic tool in PWS. METHODS: Four patients with obesity and other characteristics of PWS were studied. The exprssion of SNRPN and control gene were studed by RT-PCR from peripheral lymphocytes. RESULTS :The SNRPN expression in reverse transcribed RNA from blood were easily detected in normal control but not in patients with suspected Parder-Willi Syndrome. CONCLUSION: We conclude that SNRPN expression study is a useful diagnostic method for detection of Prader-Willi Syndrome.

3.Chronic Hydroxyurea-induced Dermatomyositis-like Eruption Showing Epidermal Dysmaturation.

Hydroxyurea is an effective treatment for a variety of myeloproliferative disodrers. A distinct cutaneous reaction to long-term administration of hydroxyurea has been characterized and designated hydroxyurea dermopathy. Epidermal dysmaturation refers to histologic changes that may be observed in the epidermis after any significant cytoreductive therapy. We report a patient with hydroxyurea-induced dermatomyositis-like eruption showing epidermal dysmaturation who developed an erythematous scaly patches on the dorsal aspects of the hands while on long-term administration of hydroxyurea for chronic myelogenous leukemia.

4.A Case of Glassy Cell Carcinoma of the Uterine Cervix.

Glassy cell carcinoma (GCC) of the uterine cervix is a rare and highly malignant tumor, accounting for only 1%~2% of all cervical carcinomas. It is typically composed of malignant cells having a moderate amount of cytoplasm with "ground glass" appearance, distinct cell membranes that stain with eosin or periodic acid-Schiff, and large nuclei with prominent nucleoli. Since its original description in 1956 by Glucletmann and Cherry, 200 - 250 cases of GCC of the uterine cervix have been listed in the literature. We report here the clinicopathological study of one case of glassy cell carcinoma with brief review of the literature.

5.Nitroblue Tetrazilium dye Reduction Test for Differential Diagnosis of Varilus Menigitis.

The usefullness of the nitroblue tetrazolium (NBT) test as an aid in the differential diagnosis of various meningitis was evaluated in infancy and childhood. 36 cases of patients with various meningitis who were admitted to the department of pediatrics in Severance Hospital from June, 1975 to March, 1976 and 8 cases of healthy children as control were the subject of this study. The following results were observed; the mean proportion of NBT-positive neutrophils was 4.0?2.0% in healthy control group, 17.0?.7% in patients with bacterial meningitis, 6.0?.3% in tuberculous menigitis, and 5.o?,1% in aspetic meningitis. And we couclued that NBT test is useful in differenciating various form of meningitis.

6.Computed tomographic findings of intracranial cysticercosis

Cysticercosis, like other parasitic disease, is still endemic in many parts of the world, and is not infrequently found in Korea. The authors reviewed CT findings of 54 cases of the cysticercosis involving the centralnervous system which were proven by pathology and clinical findings. The resuls are as follows; 1. Male to femaleratio is 3:1 and the most prevalent age group is the fifth decade. The most common symptom is seizure(56%). 2.Involved areas in the brain are parenchymal (66%), intraventricular (15%), leptomeningeal (40%) and the mixed(15%). 3. 7.Selective Protein Index and Response to Treatment in the Childhood Nephrotic Syndrome.

In a group of 59 children with nephrotic syndrome who had been admitted to Severance Ho-spital, during the period of 2 years from July of 1978 till June of 1980, the histological picture seen on renal biopsy, the selective protein index, and the response to the steroid therapy were studied. The degree of selective protein index is not related to the total daily amount of urinary protein excretion and selective protein index remains relatively constant over months in the minimal lesion nephrotic syndrom. Those patients with primary renal disease and the nephrotic syndrome who displayed high selectivity types of proteinuria usually responds initially to steroid therapy within 8 weeks. Studies of selective protein index allow the pediatrician to avoid renal biopsy with safety in nephrotic children aged 1 to 5 years, but distinguish any given renal disease with certainty. The mesangial proliferative glomerulonephritis with immune deposits has relatively poor selectivity of proteinuria and without immune deposits, it has high or moder-ate selectivity of proteinuria. Response to treatment of steroid in the Henoch-Sch?lein Purpura was poor and not related with selective protein index.

8.Chronic Cough and Sinusitis in Children: The Role of Antimicrobials.

We evaluated the role of antimicrobials in the treatment of chronic maxillary sinusitis in children with chronic cough. At the first visit 216 of 276 cases with chronic cough had radiologically abnormal maxillary sinuses (78.3%). By the Waters x-rays around 50 cases each were assigned randomly to four groups; among them, 48 cases were given an amoxicillin, 51 cases were given trimethoprim-sulfamethoxazole (TMS), 53 cases were given amoxcillin plus TMS and 50 cases were given expectorant plus decongestant (control) group. Thus, 202 children were included for data analysis. Treatment was given for two weeks in each group. The group treated with antimicrobials had a significantly higher cure rate than that of expectorant plus decongestant group (P < 0.001). Most of the patients who had roentgenographic improvement have successfully recovered from the chronic cough and the other cardinal signs of chronic sinusitis.

9.Thyroid Disease in Childhood.

Diseases of the thyroid gland are manifested by either increase in gland size, alterations in hormonal secretion, or both. Changes in gland size and weight are associated with toxic or nontoxic goiter, adenomas, thyroiditis, or malignancies. Many reports can be found to study thyroid disease, but there is no report of thyroid disease in childhood. So the present studies have surveyed the record of patient with thyroid disorders seen at Severance h spital from 1969 to 1979. The following results were obtained. 1. Among 76 cases, 56 cases were fmeale and theoratio of male and female thyroid disease was 1:3.5. 2. In classification of thyroid diseases according to function, hyperthyroidism was 20 cases(26.3%), euthyroidism 38 cases(50%), and hypothyroidism 18 cases(23.7%). 3. Disease distribution according to age, congenital hypothyroidim comprises about 75% of thyroid disease below 10 year old, and adenoma 42%, hyperthyoidism 27% of thyroid disease from 10 to 18 years old. 4. The most common sign and symptom was goiter(75%) in hyperthyroidim, palpable mass(100%) in euthyroidism, distended abdomen(83.3%) in hypothyroidism. 5. Treatment of thyoid disease was medical treatment(44 cases), surgical treatment(23 cases), and observation(9 cases).

10.A Case of Erythroplasia of Queyrat Treated with Topical 5% 5-Fluorouracil Cream.

Erythroplasia of Queyrat is an uncommon intraepithelial carcinoma in situ affecting the mucosal surface of the penis with a significant risk of invasion. Treatment is often difficult and is associated with significant recurrence rates. Topical 5-fluorouracil treatment is very effective and regarded as the treatment of choice for histologically confirmed erythroplasia of Queyrat because of its superior results compared with those of surgery or radiation therapy. We report a typical case of erythroplasia of Queyrat involving the mucosa of the glans penis in a 64-year-old male patient who was treated by topical 5% 5-Fluorouracil cream and has achieved a long-term complete response and no recurrence after 12months.
