2.Microbial Contaminations of Ultrasonic Scalers, High-Speed Handpieces and Water Syringe Lines.

The purpose of this study was to evaluate various methods of decontamination of ultrasonic scalers, high-speed handpieces and air-water syringes in dental equipments. Eimination of possible sources of microbial infection in dental operatories should be of primary importance. Microbial contamination levels of high speed handpieces, air-water syringes and ultrasonic scalers of 11 dental units in Seoul National University Hospital were evaluated after flushing the lines, alcohol sponge rubbing, or soaking in 0.1% chlorhexidine for 1,3 and 5 minutes. The result suggests that flushing the lines or soaking the tips in 0.1% chlorhexidine before use of the water systems may reduce the microbial levels. Soaking in 0.1% chlorhexidine for 5 minutes was most effective in reducing bacterial contamination.

3.Lemierre's Syndrome Presenting with Atypical Features.

Lemierre's syndrome is characterized by anaerobic bacterial infection in the head and neck, causing thrombophlebitis of the jugular vein. This disease is usually associated with a history of pharyngitis. The most common pathogens are Fusobacterium species, particularly Fusobacterium necrophorum. Lemierre's syndrome is seen most commonly in teenagers and young adults. We present a case report of a 67-year-old man with an atypical clinical manifestation of an uncommon pathogen in Lemierre's syndrome with epilepsia partialis continua.

4.Clinical Evaluation After Periodontal Flap Surgery with/without Non-Surgical Periodontal Therapy.

In the treatment of chronic adult periodontitis, scaling and root planing have been generally performed prior to periodontal flap surgery. The purpose of this study was to evaluate the clinical significance of non-surgical periodontal therapy prior to periodontal flap surgery in patients with chronic adult periodontitis. Fifty six molars showing bilateral bony defects and 4-6mm periodontal pocket in twelve patients with chronic adult periodontitis were selected. By randomized split-mouth design, in one side, flap operation was performed 4 weeks after scaling and root planing, in the other group, flap operation was only performed without scaling and root planing. Probing pocket depth, gingival recession, clinical attachment level, gingival index, plaque index, and tooth mobility were measured at baseline, before flap operation and post-operation 12 weeks. The difference of clinical parameters between groups and between each measuring time was statistically analyzed. The results were as follows; 1. At 12 weeks after flap operation, there was no significant difference in probing pocket depth, gingival recession, clinical attachment level, gingival index, plaque index and tooth mobility between groups(p<0.05). 2. In both groups, probing pocket depth, gingival recession, clinical attachment level, gingival index, plaque index and tooth mobility were significantly improved at 12 weeks after flap operation compared to baseline(p<0.05). Within the limitation of short period, the results failed to demonstrate that the non-surgical periodontal therapy prior to periodontal flap surgery could have clinical significance in improvement of clinical parameters compared to periodontal surgery without non-surgical periodontal therapy.

5.Optimal Culture Condition for Antifungal Susceptibility Tests of Malassezia globosa.

BACKGROUND: Although numerous culture conditions for Malassezia species were suggested, there were not so many objective evaluation articles in the literature. OBJECTIVES: We examined the various culture conditions for Malassezia globosa. METHODS: Malassezia globosa culture conditions were assessed by Dixon's agar, modified Leeming-Notman medium in diverse oil content and temperature conditions. RESULTS: Maximum growth rate of Malassezia globosa was achieved at 3% olive oil. The optimal temperatures for the maximal growth of M. globosa were observed at 32~34degrees C. CONCLUSION: In this study, we established the optimal culture condition for M. globosa, and confirmed its excellent utility for the antifungal susceptibility tests for M. globosa and M. restricta. Our results can help the investigators plan to do the prospective researches involving Malassezia species, such as the susceptibility test for newly developed antifungal agents.

6.A case of restrictive dermopathy.

Restrictive dermopathy is a rare autosomal recessive disorder in which rigidity or tautness of the skin from the second trimester causes a fetal akinesia deformation sequence (FADS) and early death. Characteristic features include taut skin with prominent subcutaneous vessels, widely open fontanelles and cranial sutures, distinctive facies, flexion contractures, pulmonary hypoplasia, sparse eyelashes and and eyebrows, thin dysplastic clavicles. The histologic abnormalities of the skin are located in a thin dermis, consisting of compactly arranged collagen fibers, scanty elastic fiber. The dermoepidermal junction is flat, and epidermal appendages are absent, miniaturized or immature. The presence of adipose tissue may be increased. We report on the first Korean case of restrictive dermopathy with typical clinical features and histological findings.

A hyperintensity in the subarachnoid space on fluid-attenuated inversion-recovery (FLAIR) images is often caused by diseases such as subarachnoid hemorrhage or meningitis. Oxygen has a known paramagnetic effect and also causes signal changes in the subarachnoid space on FLAIR images. These changes usually develop when the inspired air contains a high oxygen fraction. Here we present a patient with a hyperintensity in the subarachnoid space on FLAIR images whose inspired air contained only a low oxygen fraction.

8.Safety and Efficacy of Ultrasound-Guided Fiducial Marker Implantation for CyberKnife Radiation Therapy.

OBJECTIVE: To evaluate the safety and technical success rate of an ultrasound-guided fiducial marker implantation in preparation for CyberKnife radiation therapy. MATERIALS AND METHODS: We retrospectively reviewed 270 percutaneous ultrasound-guided fiducial marker implantations in 77 patients, which were performed from June 2008 through March 2011. Of 270 implantations, 104 were implanted in metastatic lymph nodes, 96 were in the liver, 39 were in the pancreas, and 31 were in the prostate. During and after the implantation, major and minor procedure-related complications were documented. We defined technical success as the implantation enabling adequate treatment planning and CT simulation. RESULTS: The major and minor complication rates were 1% and 21%, respectively. One patient who had an implantation in the liver suffered severe abdominal pain, biloma, and pleural effusion, which were considered as major complication. Abdominal pain was the most common complication in 11 patients (14%). Among nine patients who had markers inserted in the prostate, one had transient hematuria for less than 24 hours, and the other experienced transient voiding difficulty. Of the 270 implantations, 261 were successful (97%). The reasons for unsuccessful implantations included migration of fiducial markers (five implantations, 2%) and failure to discriminate the fiducial markers (three implantations, 1%). Among the unsuccessful implantation cases, six patients required additional procedures (8%). CONCLUSION: The symptomatic complications following ultrasound-guided percutaneous implantation of fiducial markers are relatively low. However, careful consideration of the relatively higher rate of migration and discrimination failure is needed when performing ultrasound-guided percutaneous implantations of fiducial markers.

9.Comparative Analysis of T-Score Discordance between a Registry-Based Korean Population and Atypical Femoral Fracture Patients of a Single Institution

Background:
The purpose of this study was to analyze the epidemiology of T-score discordance between the spine and femur in the South Korean population and compare the prevalence of T-score discordance between the Korean osteoporosis population and atypical femoral fracture (AFF) patients.
Methods:
A total of 12,422 subjects from the Korea National Health and Nutrition Examination Survey were reviewed retrospectively. T-score discordance was defined as a difference of ≥ 1 standard deviation between the lumbar spine (LS) and femoral neck (FN) bone mineral density (BMD). The prevalence of T-score discordance (low LS [LS BMD < FN BMD], low FN [LS BMD > FN BMD], and total [low LS + low FN]) was investigated in the osteoporosis and non-osteoporosis groups and stratified by sex and age. Tscore discordance of 63 patients with AFFs diagnosed at a single institution was compared with that of the Korean osteoporosis population using propensity score matching.
Results:
T-score discordance was prevalent in the Korean osteoporosis population (44.8%), and low LS discordance (37.5%) was more frequently seen than low FN discordance (7.2%) (p < 0.001). The prevalence of total and low LS discordance was significantly higher in AFF patients than in the Korean osteoporosis population (total discordance: 69.8% and 42.5%, respectively; low LS discordance: 63.5% and 31.7%, respectively; p < 0.001).
Conclusions
T-score discordance was highly prevalent in the Korean osteoporosis population, and low LS discordance was more common than low FN discordance. Nevertheless, the prevalence of low LS discordance was significantly higher in AFF patients than in the Korean osteoporosis population.
10.Clinical Observation of Sarcoidosis.

BACKGROUND: Sarcoidosis is a systemic noncaseating granulomatous disorder of unknown origin, which can involve any organs including the skin. OBJECTIVE: The purpose of this study is to investigate the clinical characteristics of sarcoidosis in patients in Seoul and Gyeonggi-do. METHODS: A total of 62 patients with sarcoidosis, where sarcoidosis was confirmed by histopathologic examinations at seven university hospitals in Seoul and Gyeonggi-do from 2000 to 2006, were enrolled in this study. The clinical characteristics were analyzed by a thorough review of medical records, histopathologic and radiologic examinations. RESULTS: Man to woman ratio was 1 : 1.48 and 42% of the patients were in their fifth decade. The most frequent chief complaints were cutaneous symptoms (37%), followed by respiratory symptoms (31%). The most frequently involved organs were intrathoracic organs (84%), followed by the skin (37%), peripheral lymph nodes (13%), bones (11%), spleen (8%) and eyes (5%). The most common cutaneous lesion was subcutaneous nodule (74%), followed by papule (13%). Extremities (65%) were the most frequent sites of cutaneous lesions. Systemic organ involvements were observed in 70% of patients with cutaneous sarcoidosis. Among them, the most frequently involved organs were intrathoracic organs (81%), followed by bones (25%), eyes (19%), and peripheral lymph nodes (19%). CONCLUSION: The results were generally similar with the previous domestic reports. Compared with the foreign reports, cutaneous involvement was more frequent and ophthalmic involvement was less frequent. Subcutaneous nodule was the most frequent cutaneous lesion which seems to be a unique feature of this study.
