3.Clinical Evaluation of Adrenal Tumors.

We retrospectively analyzed 40 adrenalectomies due to 13 cases of primary aldosteronism, 12 cases of Cushing's syndrome and 15 cases of pheochromocytoma, which were performed at Chungnam National University Hospital during the last 10 years. Of the 25 patients who presented with adrenal cortical tumors(primary aldosteronism and Cushing's syndrome), 23 had an adenoma and two had cortical hyperplasia. The overall ratio of males to females was 2:1. As to the age distribution, primary aldosteronism was predominant in the 4th and the 5th decades and Cushing's syndrome; in the 3rd and the 4th decades; however, pheochromocytoma was almost evenly distributed in 2nd through the 6th decades. Headaches, fatigue, and general weakness were the most frequent symptoms in primary aldosteronism; weight gain or a change in fat distribution in Cushing's syndrome; and headaches or blurred vision related to hypertension and palpitation in pheochromocytoma. Postoperative complications mostly occurred in patients with Cushing's syndrome: There were four wound infections, including one abdominal abscess, one pleurisy, one cerebral and cerebellar infaction, and two intestinal obstructions, but there were no postoperative deaths.

5.The Study of Reliability and Validity of Attention Deficit Disorder Evaluation Scale-School Version(ADDES-SV).

The purpose of this study was to examine the reliability and validity of attention deficit disorder evaluation scale-school version(ADDES-SV). Between October 1996 and November 1996, ADDES-SV was administered to 263 elementary school students in the second grade and between Jun 1996 and October 1996, 28 attention deficit hyperactivity disorder patients. In the reliability test, the test-retest reliability coefficient was significantly high and that of inattention was 0.88, that of impulsivity was 0.80, that of hyperactivity was 0.83 and total score was 0.83. In the reliability test by internal consistancy, the Cronbach alpha coefficient was significantly high and that of inattention was 0.98, that of impulsivity was 0.87, that of hyperactivity was 0.87(p<0.05). The half-sprit reliability coefficient by first-second half method showed high correlation and that of inattention was part 1: 0.96, part 2: 0.96 impulsivity was part 1: 0.95, part 2: 0.93, hyperactivity was part 1: 0.92, part 2: 0.94(p<0.05). Inter-rater reliability by pearson correlation coefficient was significantly high and that of inattention was 0.92, that of impulsivity was 0.87, that of hyperactivity was 0.89 and total score was 0.89. The concurrent validity between ADDES-SV and CAP scale was 0.85 in attention deficit hyperactivity disorder patient group and 0.79 in normal control group(p<0.05). In discriminant validity test between attention deficit hyperactivity disorder patient group and normal control group, the patient group showed higher score(p<0.05). The total discriminant capacity of the patient group in ADDES-SV was 94.6%. In this point of view, ADDES-SV scale showed high reliability and validity in applying to Korean subjects and was proved to be the useful screening test tool for attention deficit hyperactivity disorder research.

7.The Study of in Vivo Visual Pathway Tracing using Magnetic Magnanese Tracer.

Purpose: To evaluate the tracing of optic nerve tract using manganese enhanced magnetic resonance imaging. Materials and Methods: After injecting 30 microliter of MnCl2 (1 mol) into the retina of female New Zealand white rabbit, the contrast enhancements at major anatomical structures of optic nerve tract were evaluated by high resolution T1-weighted images 12 hours, 24 hours, and 48 hours after MnCl2 injection using 3D FSPGR (Fast Spoiled Gradient Recalled echo) pulse sequence at 1.5 T clinical MR scanner with high performance gradient system. Also, for quantitative evaluation, the signal-to-noise ratios of circular ROI on anatomical locations were measured. Results: The major structures on the optic nerve tract were enhanced after injecting MnCl2. The structures, which showed enhancement, were right optic nerve, optic chiasm, left optic tract, left lateral geniculate nucleus, left superior colliculus. The structures on the contralateral optic pathway to the right retina were enhanced whereas the structures on the ipsilateral pathway did not show enhancement. Conclusion: The Mn transport through axonal pathway of optic nerve system was non-invasively observed after injecting MnCl2 at the retina, which is the end terminal of optic nerve system. This Mn transport seems to occur by voltage gated calcium (Ca2+) channel and in case of direct injection into the retina, the fast transport pathway of voltage gated calcium channel seems to be responsible for Mn transport.

8.Familial Adenomatous Polyposis with Gallbladder Polyps.

Familial adenomatous polyposis(FAP) is an inherited autosomal dominant disorder characterized by the appearance in early life of numerous adenomas of colon and rectum. According to the recent studies, familial polypolis is a disease which produces polypoid lesions not only in the large intestine but also in the stomach duodenum, ileum and jejunum. The natural course of extracolonic lesions is variable, but the colonic polyps are associated with high incidence of cancer. The risk of malignant change is virtually 100% if untreated. The authors report a case of FAP in a 36-year-old-female who presented colorectal polyposis with multiple adenomas of the gallbladder. A review of literature with emphasis on the association of adenoma of the gallbladder was made.

9.Hook Plate Fixation for Isolated Greater Tuberosity Fractures of the Humerus.

BACKGROUND: The purpose of this study was to investigate the outcomes after fixation using a 3.5-mm locking compression plate (LCP) hook plate for isolated greater tuberosity (GT) fractures of the proximal humerus. METHODS: We evaluated the postoperative radiological and clinical outcomes in nine patients who were followed up at least 1 year with isolated GT fractures. Using the deltopectoral approach, we fixed the displaced GT fragments with a 3.5-mm LCP hook plate (Synthes, West Chester, PA, USA). Depending on the fracture patterns, the hook plate was fixed with or without augmentation using either tension suture or suture anchor fixation. RESULTS: All the patient showed successful bone union. The mean time-to-union was 11 weeks. The radiological and clinical outcomes at the final follow-up were generally satisfactory. The mean visual analogue scale for pain, the University of California at Los Angeles score, the American Shoulder and Elbow Surgeons score, and the subjective shoulder value were 1.4, 30.3, 84.3, and 82.2%, respectively. The mean active forward flexion, abduction, external rotation, and internal rotation of the shoulder were 156.7°, 152.2°, 61.1°, and the 10th thoracic vertebral level, respectively. Only one patient presented with a postoperative complication of shoulder stiffness. The patient was treated through arthroscopic capsular release on the 5th postoperative month. CONCLUSIONS: We conclude that fixation using 3.5-mm LCP hook plates for isolated GT fractures of the proximal humerus is a useful treatment method that provides satisfactory clinical and radiological outcomes.
