4.Digital subtraction angiography (DSA) in renal-related conditions

DSA(Digital Subtractin Angiography) is a valuable diagnostic imaging method in many clinical fields, includingranal-related conditons. Sixty four renal DSA examinations were performed in 59 patients with renal-relateddiseases from Jan. 1984 to Dec. 1985. Summary of these were as follows: 1. Intraarterial (IA)-DSA is performed in6 cases, intravenous(IV)-DSA in 58 cases. In 58 Examinations of IV-DSA, diagnostic image quality is obtained in 51cases(88%). 2. In investigations of a possible renovascular etiology of hypertension, IV-DSA,is a safe, sensitiveand accurate method. On screening for evaluation of renovascular hypertension, RSP should be replaced with IV-DSA,because IV-DSA is moe sensitive and accurate and can detect not only anatomic change of renal arttery but alsofunctional hemodynamic change. 3. IV-DSA is valuable in diseases with morphologic changes of vessels. Incharacterization of a known renal mass, and evaluation of hematuria, suspected aneurym and renal trauma, IV-DSA isvery useful diagnostic imaging modality. 4. In evaluation of potential renal donors, IV-DSA is an accurate andsafe method with 82.4% of accuracy. IV-DSA also is useful in follow-up of allograft recipients. 5. Ininvestigation of diabetic nephropathy, glomerulonephritis, pyelonephritis, IV-DSA is little helpful. 6. Theadvantages of DSA are well known, particularly post-procedure process using computer programs is helpful forobtaining informations of hemodynamic change or time-sequence-curve of density etc. More technical improvementwith this modality is required for improvement of the image quality and resolution. And more accumulation ofclinical experience is required in order to increase the diagnostic accuracy.

5.Atypical manifestation of solid and papillary epithelial neoplasm of the pancrease: case report.

We report a rescently experienced case with atypical radiologic manifestation of solid and papillary epithelial neoplasm of the pancreas in a 37 years old female patient. The tumor had heavy calcification on its capsule wall and septa. Instead of the usual encapsulated lesion with partly solid and partly cystic-hemorrhagic components, the lesion consisted of numerous fine cavitations containing air without fluid component except focal abscess fluid.

6.Expression pattern of Hepatitis B Viral Core Antigen (HBcAg) and Surface Antigen (HBsAg) in Liver of the Inactive HBsAg Carriers.

To understand better the complex natural course of HBV infection, the expression patterns of HBcAg and HBsAg in the liver of 51 inactive serum HBsAg carriers (24 CPH and 27 NPD) were studied by immunohistochemical methods. The inactive serum HBsAg carriers were devided into 3 groups by the following expression patterns of serum HBeAg/anti-HBe status and tissue HBcAg and HBsAg. Pattern A (18 cases) : HBeAg+, cHBcAg+ (94.4%), mHBsAg+ (61.1%), pATTERN B (14 cases) : anti-HBe+, nHBcAg+, cHBsAg+, Pattern C (19 cases) : anti-HBe+, HBcAg-, cHBsAg+ (89.5%). There were no significant differences between CPH and NPD, lthough the core free pattern was more common in the latter. The cHBcAg was expressed in 17 of 18 (94.4%) HBeAg seropositive cases but only one of 33 cases with serum anti-HBe, suggesting that the cHBcAg is intimately related to HBeAg. Since the inactive HBsAg carriers also expressed cHBcAg and/or mHBsAg, the necro-inflammatory activity of HBV infected liver is assumed to depend on the host immune response rather than their presence alone

8.Case of Decreased Serum Valproic Acid Concentration During Concomitant Use of Meropenem in Patients with Traumatic Brain Injury

Blood valproic acid levels were decreased in patients with traumatic brain injury who had been using valproic acid to control irritability and aggression, and the symptoms worsened coincident with the administration of meropenem, a carbapenem-based antibiotic. Two weeks after the discontinuation of meropenem, valproic acid levels were restored to an effective therapeutic concentration. Therefore, when valproic acid and carbapenem-based antibiotics are used together, blood valproic acid levels may decrease due to drug interaction, and irritability symptoms may worsen. Thus, close monitoring of serum valproic acid levels is required.
9.Medullary Carcinoma of Thyroid Gland with Co-existing Papillary Carcinoma.

We report a medullary carcinoma of the thyroid gland with a coexisting occult sclerosing papillary carcinoma. This rare case, in that the two types of carcinoma are completely separated and not intermixed together, is different from the previously reported mixed follicular and parafollicular cell carcinomas of the thyroid gland, so-called "differentiated thyroid carcinoma, intermediate type". The two separate tumors of the present case are undoubtedly of coincidental double primaries, and it can be a assumed that at least some of the mixed follicular and parafollicular cell carcinomas are results of collision of carcinomas derived from two embryogenetically different cell types.
