1.A Case of Ki-1 Positive Large-Cell Lymphoma Transformed from Mycosis Fungoides.

Mycosis fungoides(MF) is a cutaneous T-cell lymphoma of low-grade malignancy characterized by the proliferation of small to medium-sized cerebriform lymphoid cells confined to the skin. It usually shows favorable prognosis, but morphologic transformation occurs in some cases and is often associated with a more aggressive clinical course. Herein we report a case of Ki-1 positive large cell lyrnphoma (Ki-1 LCL) which developed in the plaque stage of mycosis fungoides. Although the lesions responded well to low-dose methotrexate therapy and there was no evidence of local or systemic recurrence until now, continuous follow-up is needed because the prognosis of transformed MF is known to be poor compared with primary cutaneous Ki-1 LCL. We also discussed the differential diagnosis of primary cutaneous Ki-1 positive lymphoproliferative disorders.

2.Echocardiographic Evaluation of Cardiac Functions in Normal Korean Adults.

To evaluate the cardiac functions we examed the M-mode echocardiography with measurements of blood pressure, heart rate and body surface area in 55 normal Korean adults(male 30 persons, female 25 persons) of mean age, 41.7+/-12.3 years. (1) Interventricular septal thickness is 9.5+/-1.7mm and left ventricular posterior wall thickness are 8.6+/-1.5mm at end-diatole, 14.0+/-2.1mm at end-systole. (2) Diastolic and systolic left ventricular internal dimensions are 49.1+/-4.8mm and 31.3+/-5.0mm, respectively. (3) Left ventricular mass by Penn Convention method is 174.4+/-52.1g and left ventricular mass index is 103.2+/-28.8g/m2. (4) Relative wall thickness is 0.35+/-0.06. (5) Left ventricular volumes by Teichholz's method are 114.9+/-27.6ml at diastole and 40.2+/-17.2ml at systole. Therefore, stroke volume is 74.7+/-16.9ml and stroke volume index is 44.5+/-10.7 ml/m2. (6) Cardiac output is 4944+/-1058 ml/min and cardiac index is 2951+/-666 ml/min/m2. (7) Total peripheral resistance is 1454+/-356 dynes-sec-cm(-5) and total peripheral resistance index is 2472+/-623 dynes-sec-cm(-5).m2. (8) Fractional shortening is 36.5+/-6.0% and pressure-volume ratio is 3.27+/-1.19 mmHg/ml. (9) End-systolic wall stress is 61.3+/-19.7x10(3) dynes=cm2. (10) Atrial emptying index is 0.66+/-0.18.

4.Expression of p21 and p53 Proteins in Gastric Adenocarcinoma.

Fifty-four adenocarcinomas of stomach were investigated to assess the expression of p21 and p53 using an immunohistochemical method. The relationship between p21 and p53 expression and the clinicopathologic parameters were analysed. The staining pattern of p21/p53 were: p21+/p53+, p21-/p53+, p21+/p53-, and p21-/p53- in 30, 12, 8, and 4 cases, respectively. Loss of p21 expression was observed in 16 of 54 tumor tissues (29%). p21 expression, however, had an inverse correlation with vascular invasion and depth of tumor invasion. The p21 and p53 protein expression showed intratumoral heterogeneity. In 63% of the adenocarcinoma, a proportional relationship was found between p21 and p53 immunostaining. The present results suggest that p53 independent induction of p21 expression may be involved in the molecular mechanism of these tumors, and expression of p21 protein may be related to a favorable prognosis in gastric adenocarcinomas.

5.Chemical Analysis of Aqueous Humor in Cataract and Glaucoma Patients.

Alteration of the chemical composition of aqueous humor may be closely related to the occurrence of glaucoma. Comparison of chemical composition of aqueous humor in glaucoma patients with that of the normal eyes is thought to be helpful in searching the pathogenesis and treatment modality of glaucoma. Concentration of 8 chemical components in the aqueous humor of cataract eyes as well as glaucoma eyes was analyzed and the differences between the two groups were searched by the authors. Subjects were 47 eyes of 47 cataract patients and 35 eyes of 32 glaucoma patients. During the operation 0.1-0.2cc of aqueous humor was collected by anterior chamber paracentesis and 10cc of blood was also drawn. In cataract patients, the mean values were Na+;145.2, K+;4.0, Cl-;123.7, HCO3-; 22.9 mEq/l, Ca++;6.6, Mg++;1.7, glucose; 48.2 mg/100 ml and ascorbic acid; 245.5 microgram/ml. In glaucoma patients, the mean values were Na+;142.1, K+;4.0, Cl-;126.0, HCO3-;17.6 mEq/I, Ca++;6.3, Mg++;1.9, glucose; 59.4 mg/100 ml and ascorbic acid; 227.5 microgram/ml. Aqueous glucose concentration was 55% of blood glucose level, and aqueous ascorbic acid concentration was 62 times higher than that in blood. Regarding the eight components in aqueous humor, no significant difference was found between cataract and glaucoma patients. Lack of significant difference in ascorbic acid levels between the two groups was thought to be due to the use of carbonic anhydrase inhibitors prior to glaucoma surgery.

6.Comparison of SITA-standard with full threshold strategy of Humphrey field Analyzer in Glaucoma.

PURPOSE: The Swedish Interactive Thresholding Algorithm (SITA) strategies is a new diagnostic tool of Humphrey automatic perimetry and has been reported to reduce testing time without loss of useful diagnostic information. To test the usefulness of this new diagnostic method, we compared the SITA and full threshold diagnostic method, which had been used previously. METHODS: Forty six patients (46 eyes) who were followed up for glucoma or ocular hypertension were in this study. Autoperimetry was performed using both SITA and full threshold strategies. RESULTS: The SITA showed shorter testing time (7.74+/-1.25 min) than the full threshold (14.4+2.43 min). There were significant differences between the full threshold MD (-8.06+8.11 dB) and SITA MD (-7.45 +/-8.32 dB), as well as full threshold PSD (4.47+/-2.50 dB) and SITA PSD (3.67+/-2.56 dB), when compared with data of greater than -12 dB MD respectively(p<0.05). Two cases showed changed diagnosis in GHT. CONCLUSIONS: SITA is a good test method with short test time. However, changing from full threshold to the SITA method should be reconsidered due to variation in the amount of field defect depending on the damage progression and the possibility of changes in diagnosis.

7.Comparison of Transpupillary and Transscleral Diode Laser Cyclophotocoagulation.

Transscleral and transpupillary diode lasers are methods of cyclophotocoagulation used to lower intraocular pressure by reducing aqueous humor production through ciliary ablation. When compared with the neodymium[Nd]-YAG, the diode laser has characteristics of lower transmission rate through the sclera and more absorption by pigment or melanin in the ciliary body, therefore the transpupillary method might be more effective in Asians who have greater ciliary body pigmentation than caucasian. We compared the efficacy of diode laser transpupillary and transscleral cyclophotocoagulation on lowering of the intraocular pressure in Asian patients. We performed transpupillary[group 2, n=9]and transscleral [group 1, n=21]diode laser cyclophotocoagulation in medically uncontrolled glaucoma patients. Preoperative mean intraocular pressures were 35.0 +/-13.1 millimeters of mercury[group 1]and 31.8 +/-9.8 millimeters of mercury[group 2], respectively. Postoperative mean pressure were 23.0 +/-14.8 millimeters of mercury[group 1]and 24.8 +/-9.9 millimeters of mercury [group 2], respectively. The intraocular pressure decreased in both groups[p=0.002 group 1, p=0.025 group 2]with statistical significance. Complications were phthisis, bullous keratopathy[group 1]and corneal edema[group 2]. Both transscleral and transpupillary diode laser cyclophoto-coagulation may be performed safely to decrease intraocular pressure. Retrospective studies and long term follow up are further needed to determine the safety of laser cyclophotocoagulation.

8.The Effect of Topical Corticosteroids on the Intraocular Pressure.

The ability of topical corticosteroids to induce an increase in intraocular pressure(IOP) limits the usefullness in ocular disease. To evaluate of the effects of different types of corticosteroids on the IOP, we applied two kinds of corticosteroics to 103 outpatients without intraocular disease. Subjects were randomly divided into two groups. The first group consisted of 67 patients were treated with 0.12% prednisolone. The second group consisted of 36 patients were treated with 0.1% fluorometholone. In the first group, following six weeks of topical application, patients demonstrated significant increase in the IOP (p<0.05). In the second group, no significant changes in the IOP after six weeks of application (p>0.05). In the first group, 72 of 134 eyes(54%) demonstrated increase in the rop. In the second group, 34 of 72 eyes (47%) demonstrated increase in the IOP. The IOP response failed to correlate significantly with sex, age, baseline IOP, refractive error and cup/disc ratio in two different corticosteroids (p>0.05). These findings do emphasize the need to measure the IOP of all patients who undergoing topical steroids application for an extended period.

9.A Design of A Mobile Remote Monitoring PDA Prototype Base on Embedded Linux with Wireless LAN Environment.

OBJECTIVE: Telemedicine uses common technologies that provide a conduit for information exchange between physicians nurses and patients. In addition to patient records, medical professionals can obtain vital signs and other reference data through telemedicine applications. also, recent advancement in RF technology and wireless communications has enabled the development of noble networks. Mobile telemedicine involves more than just communicating via mobile phone of PDA that sends and receives medical data on Portable Internet. METHODS: This paper describes the design of a mobile telemedicine PDA prototype based on Embedded Linux that can be used to monitor the medical information using a single board computer(SBC). RESULTS: The device hardware architecture consist of signal conditioning circuit, single chip micro-controller, LCD display and wireless network interface card(W-NIC). Embedded Linux was ported as its operating system and application software was developed QT/embedded 2.3.7. CONCLUSION: This system would be very useful for patients with chronic illness, especially in an emergent environment, and for containing medical cost in the aged society.
