2.Dermatophytoses in Pusan Area.

This study aims to investigate the recent clinical and mycological trends of dermatophytoses in Pusan area. The number of patients with dermatophytoses diagnosed by KOH examination and culture at Maryknoll Hospital was 1, 213 during past 2 years from November 1982 to October 1984. The incidence rate of dermatopbytoses was 9. 8% out of 12,377 cases of out patients in total. Tinea pedis (33 .7%,) was the most frequent diagnosis, followed by tinea cruris, tinea unguium, tinea corporis, tinea faciale, tinea manus and tinea capitis, in the order of decreasing frequency. The majority of cases were found in the age group from 20 to 39 4, The over all ratio of male to female patients was ] 2: 1 5 The positive rate in KOH examination was 96. 2A, and it was especially low in the areas of scalp, face and toe webs. The positive culture rate was 55, 9g, and it was higher in the covered area than in the exposed area. 6 678 strains out of the 1, 213 patients were isolated and identified. The species isolated were, in the order of decreasing frequency, Trichophyton rubrum (558 strains, 82.3% of total organisms isolated), Tn'chophyton mentagrophytes (55,8, 1 Microsporum canis (48, 7. 1g) Epidermophyton floccosum (12, 1 8g) and Microsporum gypseum (5, 0. 7gp) Trichophyton rubrum was the major causative organism in all the types of dermatophytoses except tinea capitis and tinea faciale. In tinea capitis, Microsporum canis was the main causative organism, and Trichophyton ferrugineum was not isolated, at all, which was a specific point, compared with the past tendency. In tinea faciale, zoophilic organisms were mainly isolated.

3.A Case of Early Lepromatous Leprosy Showing Unusual Skin Manifestations.

The skin lesions of leprosy have a great similarity to various other lesions. Fasal has called it the great imitator. Sometimes these skin lesions can b misdiagnosed because the physician does not consider leprosy. We reported a case of early lepromatous leprosy in 42 year old male. His skin lesions were similar to secondary syphilid. There were no sensory changes on the skin lesions and no enlargement of the peripheral nerves. Numerous AFB(+) bacilli from the skin smear confirmed our diagnosis of leprosy.

4.A Case of Multiple Lentigines Syndrome.

We report a case of multiple leatiginea syndrome in an 8 year old boy. He had numeroua lentigines acattered over his face, trunk, buttock and thlghe, and eome larger black macular leeians on the trunk and thighs. Gn phyaiaal examinatlon, he wae well developed but he had ocular hyperteloriem. Chest roentgenogram showed hypertrophy of both ventricles. Electrocardiogram and audiogram revealed conduction defects and severe sensorineural deafness, reepectively. Blopsy af dark brown lesion from the back showed the histopathologlc pattern of lentigo.

Although it is commonly appreciated that there is an inordinately large number of anormalies associated with the excretory ducts of the liver, comparatively little attention has been paid to the position of the orfice of the common bile duct into the duodenum. But, obiviously the site of entrance of the common bile duct into the duodenum becomes of great importance to the endoscopist, radiologist, and surgeon in diseases of the extra-hepatic biliary tract diag-nostically and therapeutically. We report 'a case of anomalous termination of the common bile duct into the duodenal bulb with the gall bladder empyema.

Low grade lymphomas are malignancies of predominantly small lymphocytes that typically have long median survival periods due to low proliferative rates. It is considered an indolent disease, but patients with low grade lymphoma can almost never be cured with conventional treatment. New low-grade lymphoma entities have been classified by the International Lymphoma Study Group (ILSG) and are also categorized into the Revised European American Lymphoma (REAL) classification. The REAL classification utilizes a multiparameter definition of clinico-pathologic and biologic entities. According to this classification, we investigated the incidence, various clinical characteristics, treatment outcome and prognostic factors of low grade lymphoma. Many clinical characteristics of low grade lymphoma in Korea differed from those of Western countries, especially in the incidence, therapeutic outcome and prognostic factors. In Korea, although the general incidence of low grade lymphoma is relatively low, the relative number of mucosa-associated lymphoid tissue lymphoma (MALToma) is very high, and the overall survival rate is better than that reported of Western countries. Thus, further investigation on treatment outcome and prognosis of low grade lymphoma entities, other than mucosa-associated lymphoid tissue lymphoma, are warranted.

8.Antimicrobial resistance and frequency of BlaTEM in Escherichia coli isolated from non-diarrheic and diarrheic piglets.

Antimicrobial resistance is one of the most concerns in pig industry. Escherichia (E.) coli have been used for the indicator to monitor the antimicrobial resistance. In this study, 321 E. coli from diarrheic and non-diarrheic piglets were tested for antimicrobial resistance and frequency of Bla TEM. In non-diarrheic piglets, they were resistant to oxytetracycline (93%), streptomycin (92%) and sulfadiazine (90%) but susceptible to ceftiofur (99%), colistin (97%), and enrofloxacin (82%). The isolates from diarrheic piglets were resistant to enrofloxacin (72.9%), ceftiofur (17.6%), and colistin (11.3%), whereas the resistance was 1%, 18% and 3% in case of non-diarrheic piglets, respectively. The resistance for amoxicillin/clavulanic acid (54.1%) and ceftiofur (22%) was high in isolates from post-weaning piglets. The resistance for colistin was 15.2% in nursery piglets. Seventy-three percent of isolates from diarrheic piglets showed high multidrug resistance profile (more than 13 antimicrobials) compared to those from non-diarrheic pigs in which 71% of isolates showed moderate multidrug resistance profile (7 to 12 antimicrobials). The frequency of BlaTEM in E. coli from non-diarrheic and diarrheic piglets was 57% and 69%, respectively. The results might provide the basic knowledge to establish the strategies for treatment and reduce antibiotic resistance of E. coli in piglets.

9.Evaluation of a competitive ELISA for antibody detection against avian influenza virus.

Active serologic surveillance is necessary to control the spread of the avian influenza virus (AIV). In this study, we evaluated a commercially-available cELISA in terms of its ability to detect AIV antibodies in the sera of 3,358 animals from twelve species. cELISA detected antibodies against reference H1- through H15-subtype AIV strains without cross reactivity. Furthermore, the cELISA was able to detect antibodies produced following a challenge of the AIV H9N2 subtype in chickens, or following vaccination of the AIV H9 or H5 subtypes in chickens, ducks and geese. Next, we tested the sensitivity and specificity of the cELISA with sera from twelve different animal species, and compared these results with those obtained by the hemagglutination-inhibition (HI) test, the "gold standard" in AIV sera surveillance, a second commercially-available cELISA (IZS ELISA), or the agar gel precipitation (AGP) test. Compared with the HI test, the sensitivities and specificities of cELISA were 95% and 96% in chicken, 86% and 88% in duck, 97% and 100% in turkey, 100% and 87% in goose, and 91% and 97% in swine, respectively. The sensitivities and specificities of the cELISA in this study were higher than those of IZS ELISA for the duck, turkey, goose, and grey partridge sera samples. The results of AGP test against duck and turkey sera also showed significant correlation with the results of cELISA (R-value >0.9). In terms of flock sensitivity, the cELISA correlated better with the HI test than with commercially-available indirect ELISAs, with 100% flock sensitivity.
