2.Anesthetic Management for Thoraco-Xiphopagus Conjoined Twins: A case report.

The incidence of conjoined twins is so rare that few anesthesiologists have an opportunity of managing them. Especially in Korea, there are only a few reports describing the anesthetic management for surgical separation of newborn conjoined twins. We experienced the successful anesthetic management for surgical separation of thoraco-xiphopagus conjoined twins without any particular problems. After applying the noninvasive monitors (ECG, pulse oximeter), one of the twins (twinA) with congenital heart disease was administered with intravenous ketamine for induction of anesthesia and intubated without neuromuscular blocker. Anesthesia was maintained with N2O-O2 and hand ventilation using Mapleson D breathing circuit. After maintaining airway of the twinA, the twinB was intubated and maintained with the same manner. Eighteen days after the separation procedure, the twinA with congenital heart disease died and the other one, twinB has been alive with normal growth and development.

3.A Case of Corneal Opacity in X-linked Ichthyosis Patient.

X-linked ichthyosis is a hereditary dermatosis characterized by large dark and thick scaly skin of trunk, extremities, scalp and neck. Rare manifested ocular signs include scales on lid and lashes, corneal opacity, lens opacity, and peripheral retinal granular hyperpigmentation. The authors experienced a case of diffuse punctate corneal opacities observed in the deep stroma or pre- Descemet's layer which developed in a 12-year old male patient who visited our clinic complaining symtoms of itching and dark thick scales on trunk, extremities, and scalp since at birth a.nd diagnosed as X-linked ichthyosis by clinical and histologic features.

5.The Effects of Non - depolarizing Neuromuscular Blockers on the Succinylcholine Induced Muscle Fasciculation , Postoperative Musle Pain and Relaxation.

To study the effects of small amounts of non-depolarizing neuromuscular blockers on the muscle fasciculation, postoperative muscle pains and relaxation time by succinylcholine, three groups of patients were pretreated with vecuronium (0.01 mg/kg), pancuronium (0.015 mg/kg) and gallamine (0, 2 mg/kg) 3 minutes before succinylcholine injection, respectively and compared their results with control group-saline pretreated group. The results are as follows; 1) Intensity of muscle fasciculation deereased significantly in all groups with non-depolarizing neuromuscular blockers, and frequency had the fewest in pancuronium pretreated group than the others. 2) Intensity of postoperative muscle pains decreased significantly in groups pretreated with pancuronium and gallamine, and frequency had the fewest in gallamine pretreated group than the others. 3) Pretreatment with non-depolarzing neuromuscular blocker had no effects on the onset time of muscle relaxation of succinylcholine but recovery of muscle relaxation by succinylchoine was more rapidly occured than control group. Above results, it will be suggest that gallamine and pancuronium can be used more effectively to prevent muscle fasciculation and postoperative muscle pain without influence on succinylcholine-induced muscle relaxation.

6.A Retrospective Analysis on Historical Anesthetic Trends in Chonnam University Hospital: From 1962 to 1996 .

BACKGROUND: The purpose of this study was to evaluate the historical anesthetic trends for last 35 years at Chonnam National University Hospital (CNUH). METHODS: The anesthetic records of total 101,637 anesthetic cases performed at CNUH from January 1961 to December 1996 were reviewed and analyzed according to age, sex, department, operation site, physical status, preoperative findings, anesthetic methods and agents, duration of operation, etc. RESULTS: Annual anesthetic cases was increased remarkably year by year; annual surgical patients increased 19 times for 35 years. The ratio of male to female was 1.18:1. The percent of geriatric anesthesia was increased year by year. In preoperative physical status from ASA definitions, 83.6 % of total patients was assigned class I and II. The average departmental distributions in descending order were surgery, orthopedic surgery, obstetrics and gynecology, neurosurgery, urology, ENT, thoracic surgery, ophthalmology, plastic surgery, oral and maxillofacial surgery, and others. The distributions of operation site in descending order were lower abdomen, neck, upper abdomen, lower extremities, head, chest, spine, upper extremities, and breast. Inhalational anesthesia was the most frequent used methods. Ether was abandoned from 1983, and major volatile anesthetics was halothane from 1981 to 1991. The use of enflurane has steadily increased and isoflurane has also been used with increasing frequency since 1992. CONCLUSIONS: Anesthetic cases are markedly increased year by year, and major volatile anesthetics and neuromuscular blocker were changed from halothane to enflurane and from pancuronium to vecuronium, respectively.

8.A STUDY ON THE SHOCK-ABSORBING BEHAVIOR OF RESTORATIVE MATERIALS AND INTERMOBILE CONNECTOR USEDIN IMZ IMPLANTS.

The purpose of this study was to evaluate the shock absorbing effect of 4 restorative materials and intramobile connector. The damping effect of four restorative materials used to veneer test crown rigidly connected to IMZ implant and subjected to an impact force was measured. These materials included a gold alloy(stabilor G) : a noble metal ceramic alloy(Degudent H) : porcelain(Duceram) : composite resin(Dentacolor). In addition, this study compared damping effect of same restoretive materials after using polyoxymethylene intramobile connector(POM IMC). The result of this study suggest that : *In case of using metal IMC 1. Veneered composite resin(group IV) reduced the impact force by 75%, when compared to an equivalent thickness of porcelain(group III). Group IV reduced the impact force by 87% and 89%, respectively, when compared to Stabilor G(group I) and Degudent H(group II). 2. The impact force recorded was higher for the alloy with the higher elastic modulus.(Stabilor G, group I, Young's modulus 107 Gpa, versus Degudent H, Group II, Young's modulus 95 Gpa) 3. It took the longest time for composite resin veneered group(IV) to reach to peak force when compared group I, II, III. *In case of using POM IMC 4. The mean impact force recorded were reduced by 79%(group I), 78%(group II), 69%(group III), 84%(group IV), respectively, when compared to using metal IMC. 5. The time required to reach the peak force were increased by 78%(group I, II) 87% (group III), 34% (group IV), respectively, when compared to using metal IMC

9.Blood P50 Calculated from a Single Measurement of pH, PO2 and SO2 under General Anesthesia.

BACKGROUND: The P50 which is the oxygen tension corresponding to 50% saturation at pH 7.40, PCO2 40 mmHg and temperature 37oC is an accepted and useful reference to characterize the oxygen affinity for hemoglobin. This study evaluated the accuracy of P50 calculated from a single measurement of pH, PO2 and SO2 compared with linear regression and sigmoid Emax model in patients undergoing general anesthesia. This study also compared P50 of just after anesthetic induction to P50 of 90 min later. METHODS: This study was conducted in 10 patients undergoing total abdominal hysterectomy. Just after induction of anesthesia, 20G catheter was indwelled at cephalic vein of forearm and venous return was occluded with a tourniquet. After 4 min, 6 blood samples were obtained every one minute. After 90 min, 6 blood samples were obtained with same method. All blood samples were analyzed with blood gas analyzer within 30 min after withdrawal. P50 was calculated from three methods of calculation, linear regression and sigmoid Emax model. The difference between the actural P50 and calculated P50 was evaluated. RESULTS: P50's calculated from three methods were 25.8+/-1.1, 26.1+/-1.1, 26.0+/-1.2 mmHg respectively. P50's 90min after anesthetic induction (26.1+/-1.3, 26.3+/-1.2, 26.2+/-1.2 mmHg) were significantly greater than P50's just after anesthetic induction (25.5+/-0.8, 25.8+/-1.0, 25.9+/-1.1 mmHg)(P<0.05). 10.Clostridium Difficile Colitis in Childhood: Associated Antibiotics.

PURPOSE: The following study was performed to reveal the relationship between Clostridium difficile colitis in childhood and associated antibiotics. METHODS: From January 2000 to June 2002 at Seoul National University Children's Hospital, 85 symptomatic pediatric patients who showed positive stool culture for Clostridium difficile were included. The implicated antibiotics within 2 months before stool culture were analyzed. Of the 85 patients, there were 50 males and 35 females, and their average age was 2.5 years. RESULTS: There was a history of implicated antibiotics within 2 months in 55 cases (67%). Forty-three patients (78%) of them showed Clostridium difficile in stool culture during antibiotics treatment. The time interval between the initiation of antibiotics and stool culture ranged from one day to 7 weeks (mean 10 days) in these patients. In the remaining 12 patients, Clostridium difficile was detected after the discontinuation of antibiotics. The time interval between the discontinuation and stool culture ranged from one day to 7 weeks (mean 12 days). The associated antibiotics were cefotaxime (20 cases), amikacin (15 cases), ampicillin (13 cases), cefazolin (8 cases), vancomycin (8 cases), etc. In 31 cases, more than one antibiotics were prescribed. CONCLUSION: When diarrhea occurred in young children during antibiotic usage or with a past history of recent antibiotic usage, Clostridium difficile should be investigated as a cause of diarrhea for proper management.
