1.Kupffer Cells in Hepatocellular Carcinoma.

Kupffer cells are tissue macrophages (histiocytes) fixed in hepatie sinusoids. Since malignant hepatocytes are the only tumor parencymal cells of the hepatocellular carcinoma, theoretically there are no Kupffer cells within the hepatocellular carcinoma. To clarify whether it is true or not, 12 cases of hepatocellular carcinoma of the trabecular type with some extents of the non-neoplastic surrounding liver were subjected to immunoperoxidase staining for lysozyme and S-100 protein and the results are as follows. 1) Kupffer cells were stained positively by the immunoperoxidase staining for lysozyme but not for S-100 protein, indicating that they are monocyte derived macrophages. 2) Kupffer cells were also present within the hepatocellular carcinoma, but were 2-7 times fewer within the hepatocellular carcinoma than in the non-neoplastic areas (p<0.05). 3) The non-neoplastic hepatic tissue of patients with serum HBsAg shows a tendency to have more kupffer cells than those without HBsAg.

2.Simple Cyst Occurred in an Accessory Ovary.

The accessory ovary can be defined as an extraovarian tissue that is located near the normal ovaries and is connected to the broad ligament, infundibulopelvic ligament or utero-ovarian ligament. It has very rarely been reported. The majority was found during abdominopelvic surgeries for any other purposes, because they were usually small and less than 1 cm in diameter adn gave no particular symptoms related simply to their presence. We reported a case of accessory ovary in which developed a simple cyst of a largest diameter of 12 cm, and discussed the significance of the accessory ovary in clinical and pathological aspects.

3.Histological Grade of Prostatic Adenocarcinoma.

The authors attempted to choose what has the best reproducibility and predictability for prognosis of the prostatic adenocarcinoma among four most widely used gradings methods; the Gleason's Mostofi's, Bocking and MD Anderson hospital systems. According to these gradings systems, each of two pathologists made histologic gradings of 40 consecutive prostatic adenocarcinomas which had been diagnosed with the surgically resected specimens. Correlation between the histological grades and the clinical stages was studied and a comparison was made among each system. For the comparison, the Gleason's and MDAH systems were revised as 3 grades and adjusted to the other gradings systems. In this study, MDAH grading system yielded the highest reproducibility as represented by 90% agreement, as compared with the other systems which showed 82.5~87.5% agreement. By the Gleason's, Mostofi's and Bocking's systems, 46.2%, 23.1% and 46.2% of grade 3 tumors respectively fell under the clinical stage A. On the contrary, there were no cases of grade 3 in stage A and no cases of grade 1 in stage D, by MDAH gradings system. These results suggest that MDAH gradings system is superior to the other systems in reproducibility and for predicting the biological behavior.

4.Clinical observation on prematurity subjected for EEG.

This study included fifty two premature babies in whom EEG was performed at National Medical Center, from Jan. 1990 to Jun. 1991. clinical and EEG records of all subjects were analyzed and the following results were obtained. 1) The day of first EEG performed were;11 cases within 10 days, 20 cases from 11 days to days, 13 cases from 21 days to 30 days and 8 cases beyond 31 days. 2) The results of first EEG were; within normal limit in 34 cases, mild abnormality in 3 cases, moderate abnormality in 11 cases and marked abnormality in 4 cases. 3) The abnormal EEG ratio according to the sex was higher in female than in male (43.5% vs 27.6%). 4) The abnormal EEG ratio according to the gestational age was higher in shorter gestational age group than in longer gestational age group (40.9% under 32 weeks of G.A. vs 30.0% beyond 33 weeks of G.A.). 5) The abnormal EEG ratio according to the birth weight was not significant. 6) The abnormal EEG ratio according to the ventilator use was higher in used group than in not used group (38.5% vs 30.8%). 7) Combined diseases in 18 cases of abnormal EEG group were jaundice (14 cases), hyaline membrane disease (7 cases), asphyxia (4 cases), anemia (4 cases), hydrocephalus, sepsis, PDA, hypocalcemia, UTI and IVH. 8) The mean apgar score in abnormal EEG group was lower than normal EEG group both in one minute and five minute. 9) Two cases among 18 cases of abnormal EEG group showed abnormal brain sonographic findings, but one case among 34 cases of normal EEG group showed abnormal brain sonographic finding. 10) Ten cases were followed up with serial EEG, six cases of which were treated with anticonvulsant. We observed normalization of the EEG in 5 cases of anticonvulsant treated group.

5.The Clinical Study of Pyogenic Arthritis: Pathogeny and Complication

After developement of antimicrobial chemotherapy, morbidity and mortality from pyogenic arthritis has been reduced dramatically, but still this disease has remained as a serious and life threatening infectious disease of childhood or late sequelae in surviving patients. For the period of 6 years from January 1978 to December 1983, seventy nine patients, eigthy one cases who were treated as pyogenic arthritis at Severance Hospital and Yong Dong Hospital were studied retrospectively and the results are summerized as follows. 1. We thought that the main pathogeny of septic arthritis of knee joint are trauma and direct invasion of microorganism, because the frequency of pyogenic arthritis in knee joint are proportional to the age and history of accupuncture therapy and trauma are common. 2. In thirty three cases (40.7%) underlying causes were found, composed of infectious focus in eleven cases(13.6%) correspondent to hematologic transmission, osteomyelitis in seven cases(8.7%), trauma and accupuncture in six cases(7.4%) and eight cases(9.9%) corresponding to direct invasion. 3. In forty five cases(55.6%) microorganism are identified, among which coagulase positive staphylococci are in forty cases(88.9%) . 4. Tc-99m-MDP(Medronate) whole body bone scan were taken at twelve patients and positive findings were eleven cases. 5. In thirty eight cases(46.9%) the complications were appeared: Osteomyelitis of tibia in ankle joint and periarticular or subarticular bone defects in knee joints were common. 6. There were complications in every cases that the duration was more than eleven days, no exceptional.

6.An Evaluation Study on the Cardiovascular Risk Factors of the Staff in a University.

To get basic data for the development of health care toward the staff of a university, an evaluation study of the risk factors for the cardiovascular disease utilizing data from questionnaire and health examination performed in 1994 for 1233 staff of Chosun University and its affiliated Hospital. The results are as follows: 1. The mean values of systolic blood pressure was 122.0+/-14.7mmHg for the male and 111.8+/-12. lmmHg for the female. The mean values of diastolic blood pressure was 82. 5+/-12.0mmHg for the male and 73.7+/-10.0mmHg for the female. The mean values of serum total cholesterol was 185.6+/-33.4mg/dl for the male and 173.5+/-32.6mg/dl for the female. The mean values of body mass index(BMI) was 23. l+/-2.4kg/m2 for the male and 21.5+/-2.3kg/m2 for the female. The smoking rate of the male was 47. 6% and that of the female was 0.4%. 2. The systolic and diastolic blood pressure levels increased with age in both sexes. Under 55 years of age the mean blood pressures of the male were higher than that of the female, but the levels of both sexes at 55-64 year old group did not show any big difference. The mean values of serum total cholesterol and BMI also increased with age in both sexes. Under 45 years of age, the serum cholesterol and BMI levels of the male were higher than that of the female, but on the contrary, levels of the male were lower than that of the female at 55-64 year old groups. The mean cholestrol and BMI levels of both sexes at 45-54 year old group was similar. As the age increases, the rate of exsmoker increased and the rate of current smoker decreased among the male. 3. The additive cardiovascular risk scores were calculated considering the values of blood pressure, serum total cholesterol level and the smoking status. The distribution curve of risk scores for the male showed 39. 1% under 4 points and 60.9% above 4 points with the peak at 4 points. 61.7% of the female showed that the risk scores were under 4 points and the rest was more than 4 points with maximum 9 points. As a whole the risk scores of the female were lower than that of the male. 4. By the analysis of association between the risk score of cardiovascular disease and the subjective symptom under the stratification of age and sex, the prevalence of exertional dyspnea was significantly higher at 35-44 year male group and prevalence of dizziness was significantly higher at 45-64 year male group as the risk score of cardiovascular disease increased. Among 25 34 year and 45-64 year female group persons with lower risk score of cardiovascular disease showed higher prevalence of dizziness than persosns with higher risk score.

7.Short-term Effect of Iron on the Hyperplastic Lesions of Chemical Hepatocarcinogenesis.

This study was undertaken to elucidate the short-term effect of iron on the hyperplastic lesions of experimental hepatocarcinogenesis. The Solt-Farber's resistant hepatocyte model was chosen for the experiment, and Sprague-Dawley rats wee divided into six groups: normal control, iron-rich diet administration with or without hydroxyquinoline. The iron content, microscopic changes, bromodeoxyuridine(BrdU) labelling index and the DNA polidy were studied. In the carcinogen administered group, oval cell proliferation and consecutive hyperplastic lesions of hepatocyte developed regardless of iron administration. The hepatic iron content was increased rimarkably by iron administration, but gradually decreased as the hyperplastic lesions developed in carcinogen administered groups. Although the administration of iron without carcinogen induced hepatic accumulation of stainable iron, the hyperplastic lesions appeared to be lack of it. BrdU labelling indices of the oval cells and the hyperplastic lesions of hepatocyte were very high and were not significantly altered by iron administration. Most liver cells had diploid or tetraploid DNA content, but there was an increase of diploidy as the development of hyperplastic lesions regardless of iron administration. The results indicate that the chemical carcinogen-induced hyperplastic lesions of hepatocyte do not accumulate iron, and that short-term iron administration does not affect the development of hyperplastic lesions and their proliferative activity and DNA ploidy.

8.Ilizarov Method for Knee Arthrodesis in Septic Knee Joint.

Although joint infection with severe destruction remains a serious problem and severe bone loss is relative contraindication of arthrodesis, knee arthrodesis is most commonly a salvage procedure. Its goals are to relieve pain and restore the patient to functional level of activity. The purpose of this study is to evaluate the results and complications of Ilizarov method for knee arthrodesis in the presence of infection. We retrospectively reviewed the records of twelve patients who had managed with knee arthrodesis with Ilizarov method. Indications for the operation included a infected skeletal defect secondary severe open trauma in four patients, an infection at the site of an arthroplasty in three (with failure of previous arthrodesis with monofixator in one), an infected charcot joint in four and one pyogenic arthritis spreading from osteomyelitis of proximal tibia. The average age of the patients at the time of operation was fifty-three years (range twenty-two to eighty years). Follow-up averaged 17 months. The minimum follow up periods was 9 months. Average duration of Ilizarov fixator application was 7.2 months. In 3 cases we performed corticotomy and internal bone transport to treat large bone defect (17cm, 8cm, 6.5cm). Autoiliac bone graft was done in fusion site in two cases, and in three cases we performed bone graft at the docking site. Solid osseous union occured in each patient. There was no reinfection and nonunion. Average shortening was 3.4 cm. Complications were related to pin tract. When last seen, all patients were free of pain and could walk without cruthes or cane. Despite its pin tract problems, bulky cumbersome and expensive apparatus, the Ilizarov method is one of effective method for knee arthrodesis in the presence of infection and large bone loss especially.
