1.The Clinical Study of Pyogenic Arthritis: Pathogeny and Complication

After developement of antimicrobial chemotherapy, morbidity and mortality from pyogenic arthritis has been reduced dramatically, but still this disease has remained as a serious and life threatening infectious disease of childhood or late sequelae in surviving patients. For the period of 6 years from January 1978 to December 1983, seventy nine patients, eigthy one cases who were treated as pyogenic arthritis at Severance Hospital and Yong Dong Hospital were studied retrospectively and the results are summerized as follows. 1. We thought that the main pathogeny of septic arthritis of knee joint are trauma and direct invasion of microorganism, because the frequency of pyogenic arthritis in knee joint are proportional to the age and history of accupuncture therapy and trauma are common. 2. In thirty three cases (40.7%) underlying causes were found, composed of infectious focus in eleven cases(13.6%) correspondent to hematologic transmission, osteomyelitis in seven cases(8.7%), trauma and accupuncture in six cases(7.4%) and eight cases(9.9%) corresponding to direct invasion. 3. In forty five cases(55.6%) microorganism are identified, among which coagulase positive staphylococci are in forty cases(88.9%) . 4. Tc-99m-MDP(Medronate) whole body bone scan were taken at twelve patients and positive findings were eleven cases. 5. In thirty eight cases(46.9%) the complications were appeared: Osteomyelitis of tibia in ankle joint and periarticular or subarticular bone defects in knee joints were common. 6. There were complications in every cases that the duration was more than eleven days, no exceptional.

3.A Case of Generalized Cranuloma Annulare Treated with Dapsone.

Generalized granuloma annulare is manifested by numerous, symrnetrical, shiny, normal-skin colored, discrete or confluent, papular eruption on the sunexposed areas, mostly in females of past middle age. Up to date, a variety of therapeutic approaches have been tried with inconsistent results. Recently several agents have been reported to be inconsistent results. Recently several agents have reported to be effective in the treatment of genealizecl granuloma annulare. Of these, dapsone has been used with favorable respones. The fact dapsone can inhibit the lysosomal enzyme release and the mononuclear cell-rnediated rnyeloperoxidase cytotaxicity would account for its effect in granuloma annulare even in part. We observed a case of generalized granulare in a 49-year old man, who had had numerous normal-skin colored or faintly violaceous, flattopped papular eruption on the sun-exposed areas for 3 years. He was treated successfully with dapsone 100mg a day for about 22 months.

4.Factors Influencing Quality of Caregiving by Caregivers for Elders with Dementia.

PURPOSE: The aim of the study is to determine factors influencing quality of caregiving by caregivers for the elderly with dementia. METHODS: Data were collected from 87 caregivers for elders with dementia who had visited in Busan Metropolitan Center for Dementia and D-University hospital outpatient center from July 10 to September 30, 2015. A self-reported questionnaire was used to assess the severity of the elders' dementia and knowledge of dementia, burdens and quality of caregiving by the caregivers. The SPSS 21.0 version program was used for data analysis. Data were analyzed using descriptive statistics, Pearson's correlation, t-test, ANOVA and multiple regression. RESULTS: Significant predictors of quality of caregiving by caregivers included caregivers' burdens (explanation power 25%), knowledge of dementia (explanation power 4%) and levels of education (explanation power 3%). These factors explained 32.3% of the variances in quality of caregiving. CONCLUSION: Burdens on caregivers were a major factor that decreased quality of caregiving, and knowledge of dementia was a factor that increased it. These findings show that educational programs and intervention for reducing burdens and improving knowledge of dementia are necessary to improve quality of caregiving by caregivers.

5.A Case of Metastatic Calcinosis Cutis Resembling Pseudoxanthoma Elasticum.

This report presents a case of metasric calcinosis cutis resembling pseudoxanthoma elasticum in clinical appearance. The patient, 19-year-old man, has dull yellowish firm folded lesion on both axillae, and immobility of lower extremities because of bony osteoporosis, knee fracture, impaired renal function, Skin biopsy, bone marrow biopsy, bone X-ray, and laboratory findings showed metastatic calcinosis cutis due to histiocytosis X and chronic renal failure.

6.Surgical Anatomy of the Breast for Mammaplasty Using Breast Implants.

The surgical anatomy of the breast for mammaplasty using breast implants has not yet been fully described. The mammary gland is one of the pilo-sebaceus tissues, which is located between anterior and lateral cutaneous branches of the intercostal nerve and between the superficial and deep layer of the subcutaneous fascias, and fed by the perforating cutaneous branch of the internal thoracic artery and mammary branch of lateral thoracic artery. In augmentation mammaplasty, surgeons must understand that implantation is the procedure to form a thin and wide capsule around the implant. Therefore, the implant should be inserted into the pocket surrounded by fascias between the deep layer of the subcutaneous fascia and the superficial layer of the superficial thoracic fascia or between the deep layer of the superficial thoracic fascia and the superficial layer of the deep thoracic fascia. In immediate reconstruction mammaplasty, the lower-lateral part of the superficial thoracic fascia should be preserved in order to separate the layers of the mastectomy and implantation, and the dissection of the interpectoral lymph nodes must be abbreviated to keep the fascias beneath the pectoralis major.
7.Huge Left Ventricular Thrombus and Apical Ballooning associated with Recurrent Massive Strokes in a Septic Shock Patient.

The most feared complication of left ventricular thrombus (LVT) is the occurrence of systemic thromboembolic events, especially in the brain. Herein, we report a patient with severe sepsis who suffered recurrent devastating embolic stroke. Transthoracic echocardiography revealed apical ballooning of the left ventricle with a huge LVT, which had not been observed in chest computed tomography before the stroke. This case emphasizes the importance of serial cardiac evaluation in patients with stroke and severe medical illness.

8.A Case of Tinea Incognito on the Face due to Trichophyton mentagrophytes.

Recently, we have encountered an increasing number of patients who have obtained topical steroids to self-treat various dermatosis. Tinea incognito has been used to describe a dermatophyte infection modified by corticosteroid treatment. Lesions are often atypical appearing and the diagnosis can be delayed or missed. We report a case of tinea incognito with ill-defined erythematous patch and nodules on the right upper eyelid and taken intermittently application with corticosteroid ointment over a period of l year. Mycologic studies including KOH mount and fungus culture were positive for hyphae and colonies of Trichophyton mentagrophytes. The lesion was treated with of itraconazole and cured 3 weeks later.
