6.Chronic Sclerosing Hyaline Change and Fatty Metamorphosis Resembling Alcoholic Liver Diseas in Prader-Willi Syndrome.

A complex syndrome, later called as Prader-Willi syndrome, was first described in 1956 by Prader et al, and Zellweger and Schneider characterized this syndrome as hypogonadism, hypotonia, hypomentia and boesty. It is not rare in western countries and more than 400 cases have been reported until 1983. But our interest arose because of our recent experience of diffuse noncirrhotic fibrosis of the liver in a 6 year-old boy who had the clinical features of Prader-Willi syndrome. The core of liver showed destruction of most of the hepatic lobules, particularly of the acinar zone 3, and replacement bt diffuse fibrosis. The remaining liver cells underwent fatty change, and the overall changes resembled chronic sclerosing hyaline disease of the alcoholic type. Inflammation was negligible. This particular case suggests that the severe fatty change of liver could result in irreversible damage to the hepatocytes and progressive fibrosis.
7.Correction in Rotational Deformity with Thoracolumbosacral Orthosis in Idiopathic Scoliosis.

STUDY DESIGN: Retrospective study OBJECTIVES: This study examined the effect of a TLSO brace treatment for the correction of axial rotational deformities in idiopathic scoliosis. SUMMARY OF THE LITERATURE REVIEW: A TLSO brace treatment is effective in correcting 2-dimensional deformities of idiopathic scoliosis but is questionable in axial rotational deformities. MATERIALS AND METHODS: Fifty three cases treated by bracing in March, 1999~February, 2005 at our department were reviewed. The posteroanterior and lateral radiographs were checked in the standing position throughout the study. The change in axial rotational deformity were analyzed using Nash & Moe method and Perdriolle's method. RESULTS: The rotational deformity had improved in 2 curves, was aggravated in 8 curves, and showed no change in 43 curves with the TLSO brace. CONCLUSIONS: The TLSO brace treatment in axial rotational deformity of idiopathic scoliosis is not effective in correcting the rotational deformity but is effective in preventing the progress of a rotational deformity.

8.The Effect of Thyrotropin Releasing Hormone on the Somatosensory Evoked Potentials and the Ultrastructure of Myelinated Nerve Fibers in Injuried Spinal Cord.

The purpose of this study was to determine the effect of treatment with thyrotropin releasing hormone(TRH) on the somatosensory evoked potentials(SSEPs) and ultrastructures of myelinated nerve fibers in injured spinal cords. Spinal cords of cats were injured expermentally at the second lumbar vertebra level with 20gm-20cm(400gm-cm) impact force using modified Allen's weight drop method. The animals were treated with TRH(2mg/Kg, bolus, then 2mg/Kg/hour, IV, for 4 hours) 1 hour after injury. SSEPs were checked serially in both treated and untreated groups for 24 hours. And the fine strucures of myelinated nerve fibers in the white matters were observed in electronmicroscope before and after the injury in both groups. In treated groups, the inital positive waves were re-elicited 3 hours after injury which was eliminated immediately after injury. Fine structure of the myelinated nerve fibers were changed progressively. With time after injury, the myelinated nerve fibers showed enlarged periaxonal spaces, irregulary contoured axons, and disarray of myelin sheaths. By 4 hours after injury, there appeared marked separation and disrray of myelin sheaths, and mitochondria and neurofibrils in axoplasm showed serve degeneration. The result of this study suggested that TRH has a beneficial effect on the re-elicitation of SSEP's and the preservation of myelinated nerve fibers in the acute stage of spinal cord injury.
