3.The in Vitro Proliferative Properties of Fibroblasts Originating from Upper and Lower Dermis of Psoriatic Skin Lesions.

To investigate the heterogeneity of fibroblast proliferation rate depending on the depth of dermis in the psoriatic skin lesions, fibroblasts were obtained from the upper and lower dermis of the forearm skin lesions in 9 psoriatir, patients and of the comparable sites in 7 healthy persons respectively, and were cultured. The in vitro proliferation rates of the fibroblasts were calculated by measuring the efficiency af cell attachment, cell numbers on varying days of culture and the population doubling time during the 3rd passage of subculture. The results were as follows: 1, The efficiencies of cell attachment at 24 hours after seeding were not statistically different between the upper derrnal fibroblasts and the lower ones in both psoriatic skin lesions and normal skins. 2. By rneasuring the cell numbers and the population doubling times, the prolifeatior rate of the upper der mal fibroblasts was greater than that of the lower ones in both psoriatic skin lesions and normal skins and that the fibroblastic proliferation rate was significantly inereased in the psoriatic skin lesion compared to the normal skin.

6.Clinical Effects of Benoxal (Benzoyl peroxide) on Acne Vulgaris.

Benzoyl peroxide has been known to have bacteriostatic activity against Corynebacterium acnes and to have comedolytic action in patients with acne vulgaris. The present study was undertaken to evaluate the therapeutic effect of Renoxal, a 5% benzoyl peroxide lotion, in patients with acne vulgaris. A total of 35 patients entered this study at the Department of Dermatology, Seoul National University Hospital during the five months period from March to July, 1978. All patients were instructed to apply Benoxal on their affected sites once or twice daily; and the number of lesions on three fixed areas were counted before and every week for four weeks after the treatrnent. Two female patients stopped the application due to developrnent of contact dermatitis to Benoxal The results observed in the 33 patients were as follows: 1. The percentage decrease in total number of lesions in four weeks after treatment was 51.1% 2. Closed and open comedones were gradually eliminated; and their numbers were decreased in four weeks after treatment by 45. 8% and 55. 3% respectively. The papules were decreased. by 59. 1 @2 after the first week of treatment, but showed. a transient lag around tbe second week and then marked 61. 5% in four weeks after treatment. 3. Observed side effects included burning sensation,(3 cases), tightness (3 cases), itching sensation (2 cases) and scaling (1 case). All were tolerable without any specific measure or discontinuance of the application. The authors concluded througb this experiment that Benoxal (5% benzoyl peroxide lotion) is a very effective local therapeutic agent in the treatment of acne vulgaris patients.

8.Risk Factors of Blood Requirements during Liver Transplantation.

BACKGROUND: The large volume of blood products are required during orthotopic liver transplantation. Any preoperative and intraoperative factors may influence the intraoperative blood products usage. METHODS: We retrospectively reviewed the demographic information, coagulation screens, thrombelastographic variables, and intraoperative blood requirements in 952 adult patients, who underwent orthotopic liver transplantation at the University of Pittsburgh Medical Center between January 1992 and December 1995. A preoperative coagulation abnormality score (CAS) was calculated by assigning one point of each abnormal result of the coagulation tests (PT, aPTT, platelet count) and thrombelastographic variables (reaction time, alpha angle, maximal amplitude, clot lysis index). Intraoperatively, blood products and pharmacologic coagulation therapy were administered based on thrombelastographic and hemodynamic data. RESULTS: Underlying liver disease, retransplantation one month after the first transplantation, poor preoperative coagulation profiles were predictive of intraoperative blood usage. Age, preoperative PT >15 sec, and CAS were not predictive of intraoperative blood usage. The severe fibrinolysis during operation occurred. More blood products were required in patients with severe fibrinolysis. CONCLUSIONS: The understanding of preoperative and intraoperative factors affecting blood product requirements can help the management of blood component therapy during liver transplantation.
