2.The Effect of Bupivacaine on the Methemoglobin Formation .

The changes of methemoglobin concentration in the blood after the use of lidocatine or Marcaine were investigated in the human and the rabbits. In the human the drugs were give by the epidural route and intramuscularly in rabbits. Lidocaine did not increase the methemoglobin concentration in humans but marcaine increased the methemoglobin concentration in the blood.

3.Subcutaneous Granuloma Annulare of the Scalp: A case report.

Subcutaneous granuloma annulare (SGA) is a rare, benign noninfectious histiocytic disease of unknown cause, characterized by necrobiosis of the connective tissue surrounded by infiltrates of histiocytes and lymphocytes. We report a case of SGA in a 49/12-year-old boy. The lesions were five nontender subcutaneous nodules in the parieto-occipital scalp, measuring 1.0 cm to 2.0 cm in diameter. Microscopical examination revealed variable stages of multiple necrobiotic areas, which showed central necrobiosis with palisading histiocytes, involving both the lower dermis and subcutaneous fatty tissue. Electron microscopic findings revealed many histiocytes in the necrobiotic areas with degenerating and necrotic collagen fibers as well as regenerating fibroblasts.
4.The Effect of Topical Verapamil in Q-Switched Nd:YAG Laser Induced Glaucoma Rabbits.

To create an experimental glaucoma model in rabbit and to study the effect of elevated intraocular pressure (IOP) on rabbits ocular tissues, we treated trabecular meshwork of 7 adult pigmented rabbits with Q-switched Nd:YAG laser. And then we studied the IOP lowering effect of topical verapamil in experimental glaucoma model. The IOP and outflow facility were measured with an applanation pneumotonograph (Alcon, Texas). During sustained rap elevation, 50ul of 0.25% verapamil was instilled in one eye and normal saline in the fellow eye. The IOP was measured 1, 2, 4, 8 hours after instillation. The measurement was repeated twice at 2 weeks interval. After 5 weeks, the IOP increased from an average baseline IOP of 16.6mmHg to 25.51mmHg, an increase of 55.6%. The outflow facility was decreased from 0.21 microliter/min/mmHg to 0.09 microliter/min/mmHg. There were statistically significant reductions in IOP in both verapamil and placebo-treated eyes(p<0.05). The reduction was greater in verapamil-treated eyes than in placebo-treated eyes(p<0.05). Maximal reduction of IOP was observed 2 hours after single-drop of verapamil, with significant reduction of IOP up to 4 hours duration(p<0.05). During the observation period of this study, cupping of the optic nerve head developed. Histopathologic examination revealed extensive scarring of the anterior chamber angle structure. There are marked hyaline degeneration of retinal ganglion cells in temporal retina.

6.The Measurement of Therapeutic Effect of Bendaline(R) in Early Cataract using with Scheimpflug Camera.

The Scheimpflug photography was used to evauate objectively changes in lens opacity at a month interval in 143 eyes of 78 patients affected by early cataract. from Dec. 1993 to Dec. 1994. A total of 48 patients were treated daily with 1.5g bendazac-lysine, and 30 patients constituted the control group. We checked visual acuity and analyzed change of lens opacity by using axial densitometry and retroillumination of Scheimpflug photography after Bendaline(R) medication up to 9 months After treatment, improvement or no change of subjective visual acuity was observed in 65 eyes(78.3%) of treated group, whereas decrement in 35 eyes (58.3%) of control group. In the axial densitometry, the nuclear opacity began to improve after 3 months of medication and opacities of lens between anterior supranucleus and posterior capsule were statistically significant improved after 5 months of medication (p<0.05). There's no correlation in the change of opacity area under retroillumination (p>0.05). The result show that bendazac lysine may be effective in improvement of visual acuity and delay the formation of lens opacity in early cataract.

7.Comparing the Use of Single and Double Interlocking Distal Screws on a Polarus Intramedullary Nail for Humeral Shaft Fractures.

BACKGROUND: Our aim was to make a comparative analysis of radiological and clinical outcomes of using either one or two interlocking distal screws on a Polarus intramedullary nail for the internal fixation of humeral shaft fractures. METHODS: From January 2008 to March 2014, we enrolled 26 patients with humeral shaft fractures who were operated on using intramedullary nails. The patients were divided into 2 groups according to how many interlocking distal screws were used to lock the Polarus nail: in group 1, a single interlocking distal screw was used in 12 patients; and in group 2, double interlocking distal screws, in 14 patients. We compared the degree of recovery of the displaced fracture fragments between the two groups. To compare the nonunion and shoulder function, we assessed each patient's modified American Shoulder and Elbow Surgerns (ASES) score. RESULTS: We found that 10 of 12 fractures achieved union in group 1, and 13 of 14 fractures, in group 2. We did not find a meaningful difference in the time to bone union between the two groups. The percentage of recovery of displaced fracture fragments until union was 66.9% for group 1 and 59.41% for group 2. At the final follow-up, we found that the scores for shoulder joint modified ASES was 78.7 for group 1 and 80.7 for group 2. CONCLUSIONS: Our results show that if locked appropriately, even a single screw on a Polarus nail can provide satisfactory radiological union and improved clinical outcome after intramedullary nailing of humeral shaft fractures.

8.Prognostic Relevances of Bone Marrow Features in Patients with Multiple Myeloma.

BACKGROUND: Survival time of patients with multiple myeloma has been reported to be closely related to the cytology of bone marrow smears and the histologic features of bone marrow biopsies. However, there have been many differences in morphological criteria applied by various authors. In this study, we evaluated the prognostic relevance of bone marrow features in patients with multple myeloma by investigation of the cytologic feature and the histologic patterns. MATERIALS AND METHODS: One hundred and seven previously untreated patients with multiple myeloma, admitted to Asan Medical Center, between 1989 and 1997, were studied. Bone marrow aspirations and biopsies were analyzed according to the criteria such as cytologic differentiation, volume of infiltration, pattern of infiltration, degree of hematopoiesis, and presence of fibrosis. RESULTS: 64 cases (59.8%) of 107 patients with multiple myeloma were plasmacytic type and 43 cases (40.2%) were plasmablastic type. Each median survival time was 35.0 months and 18.0 months (P<0.05). The patients with more than 25% of plasmablasts showed shorter median survival time than those with 1ess than 25% (18 months vs 38.9 months, P<0.05). The patients with nodular or packed marrow pattern revealed poorer prognosis than those with interstitial or interstitial/nodular pattern (P<0.05). The patients of plasmablastic type disclosed larger volume of myeloma cell infiltration and more packed marow pattern than those of plasmacytic type. CONCLUSIONS: The cytologic differentiation, the volume of infiltration and the patterns of infiltration were reliable predictors of survival in myeloma patients. Thus, for the prognostic evaluation and therapeutic plans, the descriptions for cytologic differentiation (especially percentage of plasmablasts), volume of infiltration and pattern of infiltration should be included in the bone marrow interpretation of multiple myeloma.

9.Effect of two different calcium hydroxide paste removal techniques on apical leakage: an electrochemical study.

This study evaluated the effect of two different calcium hydroxide (Ca(OH)2) paste removal techniques on the apical leakage of canals obturated with gutta percha cones and sealer after removing a Ca(OH)2 dressing using an electrochemical method. Seventy extracted single-rooted teeth were instrumented on with Profile rotary files under NaOCl irrigation. Fifty-eight canals were filled with calcium hydroxide paste, which was then removed using one of the following two techniques. In group A, calcium hydroxide was removed using only NaOCl irrigation, and in group B, the canals were re-prepared with a Profile rotary files-one size larger than the previous instrument and were irrigated with NaOCl. In both groups, the root surfaces were coated twice with nail varnish from CEJ to an area 4 mm away from the apex after canal obturation. Apical leakage was measured using an electrochemical method for 24 days. All the specimens showed leakage that increased markedly in the first three days. There was no significant difference between the two groups (p > 0.05). The effect of two calcium hydroxide paste removal techniques on the apical leakage was not different during a short period.

10.A Case of Synchronous Gastric Non - Hodgkin's Lymphoma and Renal Cell Carcinoma.

We report a 58-year-old man who developed synchronous gastric non-Hodgkin`s lymphoma (NHL) and renal cell carcinoma. He presented with epigastric discomfort for 2 months. Endoscopic finding of the stomach disclosed a large inegular ulceration with nodular margin on the upper body. Constrast enhanced CT scan of the abdomen showed an ulceration and focal wall thickening in the greater curvature side of stomach, and an enhanced bulging mass in the left kidney incidentally. The tissue obtained by radical proximal gastrectomy and nephrectomy showed diffuse large B-cell lymphoma on stomach and chromophobic type of renal cell carcinoma on kidney. To our knowledge, this is the first report of synchronous gastric NHL and renal cell carcinoma in Korea.
