2.A Case of Toxic Shock Syndrome.

We report a case of toxic shock syndrome in 17-year-old female patient with skin abscess. She had a skin abscess on left inner thigh about 6th days ago, but no treatment was done. And then she presented with the features of high fever, hypotension, generalized rash, myalgia and diarrhea. After the treatment with anti-shock measures, skin wound dressing and systemic antibiotics adminstration, she was completely recovered with generalized desquamation on 10th hospital day.

3.Two Cases of Allergic Contact Dermatitis due to Povidone-iodine.

Povidone-iodine (PVP-I) is a widely used antiseptic for the skin and mucous membrane. It is known that povidone-iodine rarely induce contact sensitization. Recently, we have experienced 2 cases of povidone-iodine contact dermatitis. Standard patch test with PUP-I, PVP and, iodine were performed.

4.Clinical spectrum of epidermolysis bullosa acquisita.

The clinical spectruni of epidermolysis bullosa acquisita(EBA) is much broader than originally thought. Although the full extent of the clinical presenation is still being defined, it is now known that EBA include the followings: a non-inflammatory mechanobullous condition equating wit,h classical EBA; an inflammatory vesiculosullous eruption mimicking bullous pernphigoid; and a mucosal-centered disease with sarring similar to cicatricial pemphigoid. Among the nine cases of EBA, aged between 34 to 70 year-old, seen in recent years, three patients had mechanobullous lesions with skin fragilities and scarrings; three patients had inflammatory bullous eruptions, and three other patient had combined features of mechanobullous/inflammatory bullous lesions. Mucous membrane lesions were recognized in sex cases, and the rnos! frequent site of involvement was the oral mucosae. According to observations of these patients episodes of inflammatory bullous eruptions appeared to be present in seven cases and have been considered as early sympoms of the disease. It has been noted, however, that in two cases lesions develop d as an non-inflammatory mechanobullous from thonset. Based on the ability of EBA to mimick bullous pemphigoid or cicatricial pernphogoid and the fact that such cases have perhaps been missed, we feel EBA is more common than past literature has suggested.

5.A Clinical Observation on l0 Cases of Anatrophic Nephrolithotomy.

The surgical intervention of renal staghorn calculi was performed in 9 patients(lOkidneys) who had undergone anatrophic nephrolithotomy with in situ hypothermia. admitted to the Department of Urology, HaniI Hospital, during four years periods from June, 1982 to May, 1986. This operation technique is simple, easy but uneventful recoveries followed only in a small group. The results were summarized as followings : l. Postoperative hospitalizations were 13.1 days(ranged from 8 to 21). 2. Operating time required 194.5 minutes with mean time(ranged from 150 to 230). 3. Renal artery clamping time required 36.4 minutes with mean time(ranged from 23 to 65). 4. Blood loss during operation was 875ml on the average loss(ranged from 350 to l400). 5. Postoperative complications were nephrectomy l case, pneumothorax l case, deep vein thrombosis l cases remained stone 3 cases.

6.Immunohistochemical Application of Leukocyte Common Antigen and Epithelial Membrane Antigen in the Diagnosis of Large Cell Undifferentiated Tumors.

A total of 42 cases undifferentiated large cell tumors were stained by immunoperoxidase techniques using antibodies against leukocyte common antigen (LCA) and epithelial membrane antigen (EMA). In 18 of the 21 cases studied, initially diagnosed as malignant lymphoma or Hodgkin's disease, reactivity with monoclonal anti-LCA (and noreactivity with monoclonal anti-EMA)indicated that the tumor was a lymphomas. The remaining 3 cases gave the reverse reaction pattern and therefore were classified as carcinoma. One out of 16 cases diagnosed as undifferentiated carcinoma proved to be a case of mialignant lymphoma in 5 patients in whom the original diagnosis was uncertain, a definite diagnosis was possible in all cases and 3 of these proved to be large cell lymphoma; the remainders, undifferentiated carcinoma. It is suggested that the staining of undifferentiated human neoplasms using combinations of antibodies reactive with epithelial and lymphoid cells may result in much greater diagnostic accuracy.

8.Application of Immunohistochemical Stain for Granulocytic Sarcoma.

Granulocytic sarcoma is a rare localized tumor composed of granulocytic precusor cells. Granu-locytic sarcoma occurs in a variety of clinical conditions and it is often misdiagnosed histologically. Differential diagnosis frorh lymphoma or nonhematopoietic malignancies such as undifferentiated carcinoma or sarcoma is difficult in the routing histologic examination. An evaluation of clinical and histopathologic features was done on 4 cases of granulocytic sarcoma which were diagnosed at Pusan Paik Hospital from 1988 to 1992. During the period, 282 cases of myelogenous leukemia were diagnosed. Immunohistochemical reaction for lysozyme, myelopero-xidase, leukocyte common antigen, epthelial membrane antigen and cytokeratin was assessed comparing to lymphoma and undifferentiated carcinoma. The histologic features of the granulocytic sarcoma revealed thin nuclear membrane, fine chromatin pattern and one or two small nucleoli. It also often involved the vascular wall and infiltrated the native structures without destruction. Immunohistochemical stain revealed that all(4 cases) of granulocytic sarcoma showed diffuse and strong positivity for myeloperoxidase, and partial but strong positivity for lysozyme. One case of granulocytic sarcoma was negative and 3 cases revealed focal positive reaction for LCA, and all 4 cases was negative for cytokeratin and EMA. In summary, careful observation under light microscopy with immunohistochemical stain for myeloperoxidase, lysozyme, and LCA is helpful in the differential diagnosis of granulocytic sarcoma from malignant lymphoma and cytokeratin and EMA is useful for differential diagnosis from undifferentiated carcinoma.

9.Occurrence of Respiratory Atopy in Atopic Dermatitis.

BACKGROUND: Respiratory atopy (RA) is frequently associated with atopic dermatitis (AD) and the age of onset is usually later than that of the dermatitis. OBJECTIVE: The aim of this study was to investigate the prevalence, the onset, and the duration of RA associated with AD in Korea. We also tried to correlate the existence of RA with the severity and prognosis of AD. METHODS: One hundred and eighty one patients with AD were studied. Information about the age at onset and the duration of RA were obtained from the medical history of patients or from the medical records. The severity of AD was graded. RESULTS: RA was found in 66 cases of AD (36.5%). The average age at onset of asthma was 6 years and the average duration of asthma was 6 years. The average age at onset of allergic rhinitis (AR) was 12.4 years and the average duration of AR was 6.8 years. The prevalence of asthma with AD was the most common in the age group between 8 and 11 years while the prevalence of AR was the most common in the age group between 20 and 23 years. The higher occurrence of RA in the older age group was considered to represent persistence of AD in-to adult life. We could not find any difference in the severity of AD between pure AD patients and AD patients with RA. CONCLUSION: The prevalence of RA in AD in Korea was considered to be similar to that of other reports. The average onset of asthma in Korea was a little later, whereas the average on-set of AR was a little earlier than that in the other reports. The duration of RA, which has hardly been mentioned in the literature, was considered to be about 6 years in Korea. It is likely that RA is a poor prognostic factor for AD, but does not affect severity of dermatitis.

10.5-Fluorouracil, Leucovorin ( FL ) Combination Chemotherapy in Advanced or Recurrent Colo - rectal Cancer.

PURPOSE: We studied the effectiveness and toxicities of 5-fluorouracil+leucovorin, combination chemotherapy in advanced or recurred colo-rectal cancer patients, who didn't have previous chemotherapy and enrolled from August 1993 to July 1998. MATERIALS AND METHODS: All patients were treated with leucovorin followed by 5-fluorouracil for 5 consecutive days every 4 weeks. Among 43 patients who were enrolled, 40 patients received treatment at least 2 courses, and they were evaluable. Male to female ratio was 21 to 19. In serum CEA level, 27 patients were greater than 5 ng/ml and 13 were less than 5 ng/ml. And primary site was colon in 21 patients and rectum in 19 patients. RESULTS: The complete response rate was 7,5% and the partial response rate was 25%. The median survival duration was 14.7 months, the median response duration was 16.0 months, and median time to progression was 7.3 months. In the analysis of response, survival duration, time to progression according to various characteristics of patients, serum CEA level and liver involvement were revealed significant difference in survival duration, time to progression (p=0.0122, 00350 & 0.0202, 0.0123) on univariate analysis, but no significant difference on multivariates. Hematologic and non-hematologic toxicities were mild and tolerable. CONCLUSION: This study indicates that the combination of 5-fluorouracil (370 mg/m) and leucovorin (20 mg/m) is effective and tolerable regimen in advanced or recurred colo-rectal cancer patients without previous chemotherapy.
