Partial trisomy 10q syndrome is a rare chromosome anomaly characterized by severe mental and growth retardation, craniofacial dysmorphia with prominent forehead, fine arched eyebrows, deep set small eyes and micrognathia, In addition, other physical manifestations have been reported as skeletal anomaly, congenital heart disease, inguinal hernia, and so on. We report a case of partial trisomy 10q syndrorne with certain stigmata which confirmed by chromosome analysis.

2.Visual Field Defects and Disc Findings in Glaucoma.

To Know the quantitative relation between the visual field defects and the disc cupping in glaucoma, 14 patients(26 eyes) with primary open angle glaucoma were studied. Visual field defects was measured with semi-automated Autofield-D perimeter. Optic disc was photograped with Canon CF-60Z wide angle camera and neuroretinal rim area was analyzed by using microcomputer (VAX-11/750) digitizer system. The following results were obtained: 1. Neuroretinal rim area on the nasal side was larger than that on the temporal side(p<0.05). There were no significance between superior and inferior side. 2. In the case of visual field defects, nasal side was larger than temporal side.(p<0.05). There was no significance between superior and inferior side. 3. The correlation between neuroretinal rim area and visual field area was significant in the mild visual field defects detected by using 2.9 filter and the moderate and marked field defects detected with either 2.4 or clear filter. 4. The correlation between neuroretinal rim area and visual field area(r=0.696) was higher than that between cup-disc ratio and visual field defects(r=0.568).

3.A clinical and statistical analysis of the facial bone fractures: From 1994 to 1998.

This retrospective study comprised patiens with facial bone fractures in various accidents and treated in the department of Plastic and Reconstructive Surgery, Pildong hospital, Chung Ang university during the past 5 years from January 1994 to December 1998. The medical records of these patients (fractures) were reviewed and analysed in order to gain insight into a clinical pattern and understnad the therapeutic modalities. The following results were obtained: 1. The monthly incidence was highest in August and most common accident time was between 6:00 p.m and midnight. 2. The mean age of patients was 28.3 years, and age range was 3 to 69 years. The most prevalent age group was the third decade of life (42.9%). Males predominated more than female in the ratio of 4.1:1. 3. The most common etiology was assault (29.6%) followed by traffic accidents (28.5%), fall (23.1%), sports (7.9%). 4. 369 facial fractures in 277 patients were classifed. The most common sites were nasal bone, followed by zygoma, mandible, maxilla, orbit. 5. Associated soft tissue injuries were combined in 168 patients (60.6%), and fractures other than facial bone were developed in 61 patients (22%). 6. Treatment of facial bone fracture consisted of open reduction (42.6%), closed reduction (40.1%), and conservative treatment (17.3%). 7. The complication rate was 11.6% and the most common complication was the eye problem.

4.Expression and Clinical Significance of Transforming Growth Factorbeta1 (TGFbeta1) and TGFbeta ReceptorII in Pancreatic Cancer and in Chronic Pancreatitis.

PURPOSE: Transforming growth factorbeta (TGFbeta) is an extracellular ligand that binds to a heterodimeric receptor, initiating signals that regulate growth, differentiation, and apoptosis. Many cancers, including pancreatic cancer, harbor defects in TGFbeta signaling and are resistant to TGFbeta-mediated growth inhibition. Recently, it has been reported that enhanced expression of TGFbeta1 is associated with the progression of a pancreatic ductal cell carcinoma (PCA) and chronic pancreatitis (CP). We investigated the difference in the expressions of TGFbeta1 and the TGFbeta receptor II (TbetaRII) in PCA and CP and the clinical significance of TGFbeta1 and TbetaRII in PCA. METHODS: Surgically resected pancreatic specimen were obtained from 26 patients with a PCA and 12 with CP; 5 normal pancreatic tissue specimens were also obtained. The specimens were immunostained for TGFbeta1 and TbetaRII. RESULTS: Immunohistochemical analyses of TGFbeta1 and TbetaRII revealed positive immunostaining in 73.1% and 66.7% of the tumors, respectively. In the ductal epithelial cells of PCA and CP, there were no significant differences in the expressions of TGFbeta1 and TbetaRII. In contrast, their stromal expressions tended to be stronger in PCA than in CP; especially, the expression of TbetaRII was significantly higher in PCA (p=0.008). Also the presence of TGFbeta1 and TbetaRII in the cancer was significantly associated with node metastasis, advanced tumor stage (p<0.01), and short survival time (p<0.05). CONCLUSION: There might be some pathological difference in the stromal reactions of TGFbeta1 or TbetaRII between PCA and CP. The presence of TGFbeta1 or TbetaRII indicates a role in disease progression.

7.Huge Aneurysmal Bone Cyst of the Sacrum: A Case Report.

We present a rare case of 22-year-old man who had a huge aneurysmal bone cyst of the sacrum. Preoperative adjuvant selective arterial embolization of the branches of internal iliac arteries was performed. The next day, the patient received a radical surgical excision and posterolateral screw fixation. The patient was discharged without neurological problems or symptoms. Currently, the patient has been treated without local recurrence and is symptom free. Overall, in similar cases, an aneurysmal bone cyst, chordoma, osteoblastoma and giant cell tumor should be included in a differential diag- nosis based on the imaging studies.

8.Clinical Characteristics of Sleep Disorders in Patients with Parkinson's Disease

In order to investigate the sleep quality and influencing factors in patients with Parkinson's disease (PD),201 PD patients were enrolled and underwent extensive clinical evaluations.Subjective sleep evaluation was assessed using the Pittsburgh Sleep Quality Index (PSQI),and the Epworth Sleepiness Scale (ESS).It was found that poor sleep quality (77.11%) and excessive daytime sleepiness (32.34%) were commonly seen in PD patients and positively correlated with disease severity.Then 70 out of the 201 PD patients and 70 age-and sex-matched controls underwent a polysomnographic recording.The parameters were compared between PD group and control group and the influencing factors of sleep in PD patients were analyzed.The results showed that sleep efficiency (SE) was significantly decreased (P<0.01),and sleep latency (SL) and the arousal index (AI) were increased (P<0.05) in the PD group as compared with those in the control group.SE and total sleep time (TST) were positively correlated with the Hoehn and Yahr (H&Y) stage.There was significant difference in the extent of hypopnea and hypoxemia between the PD group and the control group (P<0.05).Our results indicate that PD patients have an overall poor sleep quality and a high prevalence of sleep disorder,which may be correlated with the disease severity.Respiratory function and oxygen supply are also affected to a certain degree in PD patients.
