2.Dermatophytoses in Pusan Area.

This study aims to investigate the recent clinical and mycological trends of dermatophytoses in Pusan area. The number of patients with dermatophytoses diagnosed by KOH examination and culture at Maryknoll Hospital was 1, 213 during past 2 years from November 1982 to October 1984. The incidence rate of dermatopbytoses was 9. 8% out of 12,377 cases of out patients in total. Tinea pedis (33 .7%,) was the most frequent diagnosis, followed by tinea cruris, tinea unguium, tinea corporis, tinea faciale, tinea manus and tinea capitis, in the order of decreasing frequency. The majority of cases were found in the age group from 20 to 39 4, The over all ratio of male to female patients was ] 2: 1 5 The positive rate in KOH examination was 96. 2A, and it was especially low in the areas of scalp, face and toe webs. The positive culture rate was 55, 9g, and it was higher in the covered area than in the exposed area. 6 678 strains out of the 1, 213 patients were isolated and identified. The species isolated were, in the order of decreasing frequency, Trichophyton rubrum (558 strains, 82.3% of total organisms isolated), Tn'chophyton mentagrophytes (55,8, 1 Microsporum canis (48, 7. 1g) Epidermophyton floccosum (12, 1 8g) and Microsporum gypseum (5, 0. 7gp) Trichophyton rubrum was the major causative organism in all the types of dermatophytoses except tinea capitis and tinea faciale. In tinea capitis, Microsporum canis was the main causative organism, and Trichophyton ferrugineum was not isolated, at all, which was a specific point, compared with the past tendency. In tinea faciale, zoophilic organisms were mainly isolated.

3.Clinical Evaluation and Patch Test with Vegetables in Housewife' s Hang Eczema.

The importance of vegetables as a allergen for the development of hand eczema has been a matter of debate. The aim of this study was to evaluate the clinical manifestations of hand eczema and to investigate the role of vegetables in the hand eczema of Korean housewives. In this study, clinical evaluatons and patch tests with Hollister-Stier standard allergens and 22 vegetable allergens wer performed in 32 patients with housewife's hand eczema. The results are summarized as follows : 1. The prevalence was highest in fourth(59.4%) and third(25.0%) decades of life and the peak age of onset was third(53.1%) and fourth(31.3%) decades. 2. Only 3 of 32 patients(9.4%) had past histories of atopic dermatitis ; however, childrens of 12 patients(37.5%) had atopic dermatitis. 3. The common skin signs were fissuring(90.6%), scaling(78.1%), erythema(75.0%), hyperkeratosis(71.9%), lichenification(62.5%), and vesicle(43.8%) in descending order. 4. The common aggravating factors were soap or detergent(93.8%), water(90.6%) and rubber glove(25.0%). 5. The most common involved areas of hand eczema were fingers including fingertips and fingertips only. 6. The results of patch test using vegetable allergens with 10 normal controls were all negative. Of the 32 patients with hand eczema, 13(37.5%) showed positive patch test with standard allergens and 5(15.6%) showed positive with vegetable allergens. 7. Among 30 standard allergens, cinnamic alcohol(9.4%). potassium dichromate(9.4%), nickel sulfate(6.3%), and wool wax alcohol(6.3%) were the common allergens which showed positive reactions. Of the 22 vegetable allergens, garlic, Chinese cabbage juice, Chinese cabbage extract, cucumber juice, spinach extract, potato juice showed weak positive reactions in only one patient(3.1%) respectively. From the above results contact sensitivity to vegetables is not significant in Korean housewives with hand eczema.

4.A Case of Early Lepromatous Leprosy Showing Unusual Skin Manifestations.

The skin lesions of leprosy have a great similarity to various other lesions. Fasal has called it the great imitator. Sometimes these skin lesions can b misdiagnosed because the physician does not consider leprosy. We reported a case of early lepromatous leprosy in 42 year old male. His skin lesions were similar to secondary syphilid. There were no sensory changes on the skin lesions and no enlargement of the peripheral nerves. Numerous AFB(+) bacilli from the skin smear confirmed our diagnosis of leprosy.

6.A Case of Multiple Lentigines Syndrome.

We report a case of multiple leatiginea syndrome in an 8 year old boy. He had numeroua lentigines acattered over his face, trunk, buttock and thlghe, and eome larger black macular leeians on the trunk and thighs. Gn phyaiaal examinatlon, he wae well developed but he had ocular hyperteloriem. Chest roentgenogram showed hypertrophy of both ventricles. Electrocardiogram and audiogram revealed conduction defects and severe sensorineural deafness, reepectively. Blopsy af dark brown lesion from the back showed the histopathologlc pattern of lentigo.

7.Clinical Profile and Visual Outcomes after Treatment in Patients with Dysthyroid Optic Neuropathy.

PURPOSE: To report the clinical data and visual outcomes after treatment of patients with dysthyroid optic neuropathy (DON). METHODS: We retrospectively reviewed the medical records and orbital images of 40 patients (65 eyes) with DON and analyzed the visual outcomes after treatment with intravenous steroids pulse therapy, radiotherapy and orbital decompression. RESULTS: The study included 21 men and 19 women, with 10 (25%) being diabetic patients. Visual field test results revealed defects in 88.7% of DON eyes; afferent pupillary defects in 63.2%; reduced color vision in 78.5%; and abnormal visual evoked potentials in 84%. Orbital imaging showed moderate to severe apical crowding in 95% of the orbits and intracranial fat prolapse in 24.2%. Median best corrected visual acuity improved from 0.4 to 1.0 after one year of treatment (p < 0.001). We noted more improvement in vision with the use of decompressive surgery than with non-surgical methods (p < 0.05). Recurrences occurred in 7 patients who had not received orbital radiotherapy. CONCLUSIONS: Visual field defects and apical crowding seen on orbital imaging were the most sensitive indicators for the detection of DON. Treatment with intravenous steroids pulse therapy, radiotherapy and orbital decompression effectively improved visual outcomes in cases of DON.

9.Endothelial Function of Cornea Preserved in Korean Corneal Storage Media.

PURPOSE: To evaluate the endothelial function of cornea preserved in newly developing korean corneal storage media (CS002, CS003) by estimating the permeability of corneal endothelium and the change of corneal thickness. METHODS: The cornea were divided into six experimental groups - fresh group immediately after enucleation, 4degrees Cmoist chamber group preserved for 24 hours and 48 hours, Optisol & CS002 group for 1, 3, 5, and 7 days, and Likorol & CS003 group for 7, 10, and 14 days after enucleation, and then corneal endothelial permeability(Pac) was measured using carboxyfluorescein solution. Corneal thickness was measured using pachymeter(fine focus adjustment) of the specular microscope. RESULTS: Corneal endothelial Pac (x1 0(- 4) cm/min) was 3.64+/-0.33 in fresh group, 4.79+/-0.28 in 4degrees Cmoist chamber group for 24 hours. Each endothelial Pac of CS002 group at 5 and 7 days was 5.81+/-0.55 and 5.65+/-0.58, which were different with 4degrees Cmoist chamber preservation group for 24 hours(p<0.05) but not different with Optisol groups at same days. Each endothelial Pac of CS003 group at 7, 10, and 14 days was 4.34+/-0.34, 4.66+/-0.59, and 4.66+/-0.27, which were not different from those of Likorol. Each corneal thickness of CS002 and Optisol group at 7days was 417.80+/-19.37 mu m and 421.00+/-19.75mu m, which were resemble increment. Corneal thickness was 426.75+/-22.43mu m in CS003 group and 476.00+/- 40.08mu m in Likorol group at 7days. There was statistical difference between the two group(P<0.05), and this difference was sustained for 14days (P<0.05). CONCLUSION: There was no difference in the effect on corneal endothelial permeability between korean corneal storage media such as CS002 and CS003, and that of previous corneal storage media such as Optisol and Likorol. Corneal thickness of cornea preserved in korean corneal storage media was thinner than that of Likorol.

10.A Case of Malignant Chondroid Syringoma with Lung Metastasis.

Malignant chondroid syringoma, previously called "mixed tumor of the skin of the salivary gland type"is a fairly uncommon type of sweat gland tumor. Malignant chondroid syringoma frequently arises from the trunk and extremities, whereas the benign tumor is common to the head region. The present case occurred in a female. The malignant nature of the tumor was evident from repeated recurrences after excision of the mass and histopathological study. Lack of response to radiotherapy and chemotherapy led to widespread metastasis. We report a case of malignant chondroid syringoma with lung metastasis in a 39-year-old female patient and response to chemotherapy. We also reviewed the literatures of malignant chondroid syringoma.
