The authors have experienced two cases of meibomian gland carcinoma. The first case was a 65-year-old female who was diagnosed as having chalazion and undergone curettage for twice. The second case was a 70-year old female who believed to be a ruptured chalazion. We had a study of the histopathological finding for two cases of meibomian gland carcinoma of the upper lid with a brief review of relating literature.

PURPOSE: To evaluate the effectiveness of intralesional triamcinolone acetonide injection, and incision and curettage for primary chalazia. METHODS: In order to compare the effectiveness of treatment modality, 103 patients who were diagnosed with primary chalazia were divided into 2 groups. The patients in group A underwent intralesional triamcinolone acetonide injection and the patients in group B underwent incision and curettage. The patients were followed up 1 week and 3 weeks after the procedures to identify the regression of the lesion and the procedure complications. A decision regarding success or failure was made at 3 and 6 weeks after the treatment. Successful treatment was defined as the infallibility in the functional and esthetic aspects as well as the size of regressed lesion. RESULTS: This study included 82 out of 103 patients, who satisfied the inclusion criteria. Forty-four patients underwent intralesional triamcinolone acetonide injection and the remaining 38 patients underwent incision and curettage. The success rate of the initial treatment, which was identified 3 weeks after the procedure, was 81.8% in group A and 86.8% in group B. The cumulative success rate of treatment after 6 weeks was 86.8% in group A and 92.1% in group B. No complications were observed with both treatment modalities. CONCLUSIONS: Intralesional triamcinolone acetonide injection is as effective and safe as incision and curettage for the treatment of primary chalazia.

Metastatic carcinoma of the eyelid is extremely rare; Oltmans reported the first case in 1930. We experienced a case of metastatic adenocarcinoma of the left lower eyelid in a 51-year-old male patient. The lesion was clinically similar to chalazion. Histopathologic findings of biopsy revealed gastric adenocarcinoma.

The meibomian gland adenocarcinoma is very rare and lethal tumor of the eyelid. The upper eyelid is more often affected than the lower eyelid. Clinical feature of this tumor is that it may masquerade as a chalazion or chronic blepharoconjunctivitis for many months before the true diagnosis is established. Treatment of the consists of surgical removal and radiation therapy. The authors have experienced a case of meibomian gland adenocarcinoma which had recurred after two chalazion surgeries at a local clinic in the left lower eyelid of a 61-year-old korean female. The tumor was excised surgically with reconstruction of the lid defect by modified Hughes procedure and was found to be a adenocarcinoma of the meibomian gland histopathologically.

Nowadays, nonincisional double eyelid operation is preferred by lots of patients and surgeons. There are a variety of suture methods but all the suture procedures-including continuous buried suture methods-in the literatures penetrate the conjunctivas. So due to the pain, surgeons had to infiltrate local analgesics into the conjunctiva. This horrifies the patients, gives pain, makes the eyelids and conjunctivas swollen, and sometimes interrupts the movements of the eyelids by affecting the levator palpabrae muscles. This condition could make the eyelid folds asymmetric. Authors changed the procedure by using a chalazion forcep with a small slit which allows the suture materials to get out and we could successfully perform the suture without penetrating the conjunctiva. So we could make double folds maintaining the patient's dynamic and static conditions very well since no direct local analgesics were needed for the conjunctivas, and the results were pretty satisfactory.

PURPOSE: We report a case of the granulocytic sarcoma located at the conjunctival fornix in a 10-year-old girl with acute myeloid leukemia, who was in complete remission after bone marrow transplantation. METHODS: A 10-year-old girl had a 1-week history of firm, dark brown conjunctival mass at lower fornix and mild lid swelling of the right eye. Several months previously she had had some chalazions at lower lid and a history of acute myeloid leukemia that was currently in complete remission after bone marrow transplantation. Other ocular manifestations were not specific. Excision of the conjunctival mass and a biopsy for diagnostic and therapeutic purposes were performed. RESULTS: The 12 x 6 x 5 mm-sized mass was totally excised. The pathologic examination was consistent with granulocytic sarcoma. Three weeks after the biopsy, the patient had a relapse of acute myeloid leukemia. She died 2 months later while undergoing intensive therapy. CONCLUSIONS: This condition should be considered in the diagnosis of granulocytic sarcoma in patients with a history of acute myeloid leukemia.

A 23-year-old woman visited our clinic for the evaluation of a small bean-sized mass in the right lower eyelid. She noticed a mass in the right lower eyelid 6 years ago. The mass was treated with incision and curettage under the impression of chalazion twice at the local clinic, but recurred soon after the operation. On examination, the visual acuity was 1.0 in both eyes. A firm, non-tender, smooth, movable and small bean-sized mass was palpable in the right lower eyelid. A white scar from the previous operation was seen on the palpebral conjunctiva of the right lower eyelid. The mass was excised through the conjunctiva. The histopathological diagnosis of the excised mass was benign mixed tumor. The patient was recommended of further radical excision. The pentagon-shaped excision of the right lower eyelid under frozen section control was done 2 weeks after the first operation. The patient is doing well as of now. The benign mixed tumor of the lower eyelid can arise from either ectopic lacrimal gland or accessory lacrimal gland.

Adenocarcinoma of the Meibomian gland is very rare but lethal tumors. Clinically, most of the time sebaceous gland carcinoma is mistaken for a chalazion. Any recurrent chalazion should be submitted for histologic study, and any chronic, recalcitrant, atypical blepharitis should be biopsied. The treatment of the choice is excisional biopsy and followed by radiation therapy. A 30 year old Korean male had a hard, nontender mass on the left upper lid which had recurred after chalazion surgery. The tumor was removed surgically as repair with modified Wheeler direct full halving procedure, and found to be a adenocarcinoma of the Meibomian gland histopathologically. There was no evidence of recurrence and metastasis to remote site a year after surgery with radiation therapy.

PURPOSE: To compare the efficacy of an intralesional steroid injection in the treatment of chalazion according to triamcinolone acetonide (TA) concentrations. METHODS: A total of 108 patients with 120 chalazia received an intralesional injection of TA. Patients were divided into 3 groups according to the concentrations of TA: 5 mg/ml, 10 mg/ml, and 40 mg/ml. A regular follow-up was performed and the size of lesion and recurrence were evaluated. RESULTS: Success was defined as a minimum of 80% decrease in size with no recurrence. The success rate was 78.1% in the 5 mg/ml group, 76.2% in the 10 mg/ml group, and 78.4% in the 40 mg/ml group. These results were not statistically significant (p = 0.999, Fisher's exact test). Yellow deposits developed in 4 out of 37 lesions in the 40mg/ml group, and in 1 out of 42 lesions in the 10 mg/ml group. Skin depigmentation was observed in 1 case in the 5 mg/ml group. CONCLUSIONS: Intralesional TA injection is an effective treatment for chalazion. There is no significant difference in success rates according to the concentrations of TA. However, caution is advised in cases of high injection concentrations of TA to prevent yellow deposits.

Eyelid tumors, including metastatic lid tumors, present clinically as recurrent chalazia, multicentric nodules, ul cerated lesions, diffuse swelling, or chronic blepharocon juntivitis. Lid tumors consist of benign lesions(69%) and malignant lesions(31%). Basal cell carcinoma and squa mous cell carcinoma account for 92% of the malignant lesions. Metastatic lid disease was found to be of rare occurence, accounting for 0.1% of all lid lesions and 0.3% of malignant lid lesions. We report a case of metastatic lid carcinoma of sto mach cancer. A 60-year-old man developed a painless, slow-growing nodular lesion of his right lower eyelid. Excisional biopsy revealed metastatic undifferentiated ade nocarcinoma. We believe this is the first case in Korean literature.
