1.Two Cases Report Meibomian Gland Carcinoma.

The authors have experienced two cases of meibomian gland carcinoma. The first case was a 65-year-old female who was diagnosed as having chalazion and undergone curettage for twice. The second case was a 70-year old female who believed to be a ruptured chalazion. We had a study of the histopathological finding for two cases of meibomian gland carcinoma of the upper lid with a brief review of relating literature.

2.Comparison of Effectiveness Between Intralesional Triamcinolone Injections and Incision and Curettage for the Primary Chalazia.

PURPOSE: To evaluate the effectiveness of intralesional triamcinolone acetonide injection, and incision and curettage for primary chalazia. METHODS: In order to compare the effectiveness of treatment modality, 103 patients who were diagnosed with primary chalazia were divided into 2 groups. The patients in group A underwent intralesional triamcinolone acetonide injection and the patients in group B underwent incision and curettage. The patients were followed up 1 week and 3 weeks after the procedures to identify the regression of the lesion and the procedure complications. A decision regarding success or failure was made at 3 and 6 weeks after the treatment. Successful treatment was defined as the infallibility in the functional and esthetic aspects as well as the size of regressed lesion. RESULTS: This study included 82 out of 103 patients, who satisfied the inclusion criteria. Forty-four patients underwent intralesional triamcinolone acetonide injection and the remaining 38 patients underwent incision and curettage. The success rate of the initial treatment, which was identified 3 weeks after the procedure, was 81.8% in group A and 86.8% in group B. The cumulative success rate of treatment after 6 weeks was 86.8% in group A and 92.1% in group B. No complications were observed with both treatment modalities. CONCLUSIONS: Intralesional triamcinolone acetonide injection is as effective and safe as incision and curettage for the treatment of primary chalazia.

3.A Case of Meibomian Gland Adenocarcinoma.

The meibomian gland adenocarcinoma is very rare and lethal tumor of the eyelid. The upper eyelid is more often affected than the lower eyelid. Clinical feature of this tumor is that it may masquerade as a chalazion or chronic blepharoconjunctivitis for many months before the true diagnosis is established. Treatment of the consists of surgical removal and radiation therapy. The authors have experienced a case of meibomian gland adenocarcinoma which had recurred after two chalazion surgeries at a local clinic in the left lower eyelid of a 61-year-old korean female. The tumor was excised surgically with reconstruction of the lid defect by modified Hughes procedure and was found to be a adenocarcinoma of the meibomian gland histopathologically.

4.Metastatic Gastric Adenocarcinoma of The Lower Eyelid.

Metastatic carcinoma of the eyelid is extremely rare; Oltmans reported the first case in 1930. We experienced a case of metastatic adenocarcinoma of the left lower eyelid in a 51-year-old male patient. The lesion was clinically similar to chalazion. Histopathologic findings of biopsy revealed gastric adenocarcinoma.

5.Double Eyelid Operation Using Continuous Buried Suture Method Avoiding Conjunctival Injury by a Newly Made Chalazion Forcep.

Nowadays, nonincisional double eyelid operation is preferred by lots of patients and surgeons. There are a variety of suture methods but all the suture procedures-including continuous buried suture methods-in the literatures penetrate the conjunctivas. So due to the pain, surgeons had to infiltrate local analgesics into the conjunctiva. This horrifies the patients, gives pain, makes the eyelids and conjunctivas swollen, and sometimes interrupts the movements of the eyelids by affecting the levator palpabrae muscles. This condition could make the eyelid folds asymmetric. Authors changed the procedure by using a chalazion forcep with a small slit which allows the suture materials to get out and we could successfully perform the suture without penetrating the conjunctiva. So we could make double folds maintaining the patient's dynamic and static conditions very well since no direct local analgesics were needed for the conjunctivas, and the results were pretty satisfactory.

6.Effectiveness for Intralesional Triamcinolne Acetonide Injections for Chalazia in Pediatric Patients.

PURPOSE: To evaluate the effectiveness of intralesional triamcinolone acetonide injection for chalazia in pediatric patients. METHODS: We performed a retrospective chart review of 58 patients (70 lesions) under fifteen years of age and 40 patients (50 lesions) aged fifteen and over, who underwent intralesional triamcinolone acetonide injections and performed follow-ups for at least three months between January 2007 and October 2008. Data regarding age, sex, lesion size, location, number of injections, treatment success, surgery, and complications were evaluated. RESULTS: Cumulative treatment success of TA was 64.3% with the first injection, 82.9% with a second injection, and 88.6% with a third injection in patients under fifteen years of age. The average number of TA injections was 1.67+/-1.03 in primary chalazia and 1.48+/-0.87 in recurrent chalazia, while the cumulative treatment success until third TA injection was 89.8% in primary chalazia and 85.7% in recurrent chalazia inpatients under fifteen years old. The average number of TA injections was 1.95+/-1.18 in primary chalazia and 1.80+/-1.14 in recurrent chalazia, while the cumulative treatment success until the third TA injection was 82.5% in primary chalazia and 70.0% in recurrent chalazia in patients aged fifteen and over. No complications were noted with TA injections. CONCLUSIONS: Intralesional triamcinolone acetonide injection in chalazia is an effective and safe treatment in pediatric patients.

7.Clinical Observations on Cosmetic Ocular Tatooing.

To investigate the present status with its side effect of ocular tatooing for cosmetic purpose in our country, 150 patients with the eyelid and eyebrow tatooing who visited my clinic from July 1986 to October 1988 were enrolled in this study. Six patients who complained the early complications after the cosmetic tatooing were included in the tatooing group, and 150 patients without the tatooing were used as a control group for the statistical study. The following results were obtained: 1. The ages tatooed ranged from 19 to 68 years old age with the mean age of 39.65 +/- 9.46 years. Fourth and fifth decades consited of 76% of the total subjects tatooed. 2. At their first visit, blepharitis, external hordeolum, pingueculitis, subconjunctival hemorrhage and keratitis were more statistically frequent than the control gruop(p<0.05), but chalazion was not so. 3. Cosmetic tatooing was gradually noted after 1984 and was most frequently performed in 1986(41%). Thereafter, it reduced from 1987(35%) until now. 4. One half was tatooed once and 1 to 3 times of tatooing were added for the remainder. 5. Ninetyseven % of tatooing was done by non-medical doctor in beauty parlor or the other places excluding the hospital, and only 3% was performed by medical doctor in the hospital. 6. Eyelid tatiooing was noted in 96% of the total subjects tatooed and eyebrow tatooing was shown in over half of them(53%). 7. Early complication of the tatooing was confirmed by history taking in 29% of the total subjects tatooed, which consited of lid swelling(23%), subcutaneous hemorrhage of lid(4%), itching sensation(2%) and conjunctival redness(1%). Clinical Observations on Cosmetic Ocular Tatooing Six patients were treated for one to 10 days for the early complication of tatooing under the diagnosis of lid edema(100%), keratitis(83%), subcutaneous hemorrhage of lid(50%), blepharititis(33%), conjunctivitis(33%), corneal abrasion(33%) and conjunctival foreign body(17%). 8. Thirtysix % out of the patients confirmed by history taking was regretful after the cosmetic tatooing.

8.A Case of Meibomian Gland Adenocarcinoma.

Adenocarcinoma of the Meibomian gland is very rare but lethal tumors. Clinically, most of the time sebaceous gland carcinoma is mistaken for a chalazion. Any recurrent chalazion should be submitted for histologic study, and any chronic, recalcitrant, atypical blepharitis should be biopsied. The treatment of the choice is excisional biopsy and followed by radiation therapy. A 30 year old Korean male had a hard, nontender mass on the left upper lid which had recurred after chalazion surgery. The tumor was removed surgically as repair with modified Wheeler direct full halving procedure, and found to be a adenocarcinoma of the Meibomian gland histopathologically. There was no evidence of recurrence and metastasis to remote site a year after surgery with radiation therapy.

9.The Effects of Intralesional Steroid Injection for Chalazion According to the Concentrations of Triamcinolone Acetonide.

PURPOSE: To compare the efficacy of an intralesional steroid injection in the treatment of chalazion according to triamcinolone acetonide (TA) concentrations. METHODS: A total of 108 patients with 120 chalazia received an intralesional injection of TA. Patients were divided into 3 groups according to the concentrations of TA: 5 mg/ml, 10 mg/ml, and 40 mg/ml. A regular follow-up was performed and the size of lesion and recurrence were evaluated. RESULTS: Success was defined as a minimum of 80% decrease in size with no recurrence. The success rate was 78.1% in the 5 mg/ml group, 76.2% in the 10 mg/ml group, and 78.4% in the 40 mg/ml group. These results were not statistically significant (p = 0.999, Fisher's exact test). Yellow deposits developed in 4 out of 37 lesions in the 40mg/ml group, and in 1 out of 42 lesions in the 10 mg/ml group. Skin depigmentation was observed in 1 case in the 5 mg/ml group. CONCLUSIONS: Intralesional TA injection is an effective treatment for chalazion. There is no significant difference in success rates according to the concentrations of TA. However, caution is advised in cases of high injection concentrations of TA to prevent yellow deposits.

10.Two Cases of Meibomian Gland Adenocarcinoma.

The Clinical Resemblance of the rare carcinoma of the meibomian gland to the relatively frequent chalazion should be kept in mind by anyone performing eye lid surgery. The author recently experienced two cases of Meibomian gland adenocarcinoma of upper lid. In the first case a 65-year old female visited our hospital in March, 1976 with a large mass in the right upper eyelid which had showed progressive enlargement of two years duration. A Diopsy was carried out and the histopathological examination revealed sebaceous adenocarcinoma. She was treated with local resection with full thickness containing normal tissue around the lesion of upper lid and reconstructive surgery for upper eyelid. A radiation therapy was performed with good result. In second case, a 80-year old female visited our hospital in Dec. 1975 with a large mass in the left upper eye lid which had showed progressive enlargement of one year duration. The tumor was resected and the histopathologic examination revealed sebaceous adenocarcinoma. A radiation therapy was performed with good result.
