2.CT findings of pulmonary actinomycosis and causes of misdiagnosis

Objective To scrutinize the CT features of pulmonary actinomycosis and analysize the causes of misdiagnosis.Methods CT images of eleven patients with pulmonary actinomycosis proved by pathology were retrospectively reviewed.Among them,6 were scanned with additional enhanced CT scan.Results Among the 11 patients,the lesions located in the inferior lobe in 7 patients,and in the superior lobe in 4 patients.The lesions affected two or more pulmonary segments in 9 patients,6 of them involving two lobes.Irregular mass or nodules with necrotic low-attenuation areas were the main signs in all patients,and segmental consolidations were found in 6 patients.Peripherally progressive enhancement was found in all patients with contrast enhanced CT scan.Vicinity pleural thickening was found in 11 patients,while extrapleural fat deposition was found in 6 patients.One case showed erosion of the chest wall and rib.Conclusion Pulmonary actinomycosis is a chronic suppurative granulomatous inflammation,which is often misdiagnosed with neoplasm or other lung disease because of its mass-like performance.The peripherally progressive enhancement on CT has a certain diagnostic value for P\pulmonary actinomycosis.
3.Acoustic Analysis of Voice Following Different Phonation Conditions in Children

Objective To assess voice parametric characteristics of normal children in different age?sex?and phonation conditions. Methods Voice data of 240 children were collected and analyzed with Dr. Speech software. Acoustic signal data were measured in three phonation conditions: the lowest modal voice, natural voice, the highest modal voice. Results Perturbations and normalized noise energy from the highest value to the lowest one by one were that of the lowest modal voice, natural voice, the highest modal voice. The sequences of standard deviation of fundamental frequency were that of the highest modal voice, the lowest modal voice, natural voice. Conclusion The voice quality of normal children had distinctive characteristics in different phonation conditions.
4.Effect of Tangshen Particle on Glucose and Lipidmetabolism and Renal Function of Diabeticmice

[Objective]To observe the curative effect of the Tangshen Particle on diabetes.[Methods] ICR mice were induced for diabetes by Alloxan intravenous injection,take Tangshen Particle and Xiaoke pill as the comparison group,observe each group’s glucose,lipid,renal index,urea,creatinine.[Results] The Tangshen Particle was able to decline glucose,lipid,renal index,urea,creatinine on diabetic mice,but the effect on high dose group was most obvious.[Conclusion] Tangshen Particle has good glucose-lowering and lipid-lowering effect on diabetic mice,and it has preventive and therapeutical effects on diabetic nephropathy .
5.Intrascrotal Dermoid Cyst.

Dermoid cyst is a benign cystic teratoma consisting of 3 germ cell origin tissues with predominant ectodermal origin and most commonly found in the ovary but rare in the scrotum. We reported two cases of intrascrotal dermoid cysts developed in 7 months and 3 years old boys which were confirmed hiatologically. In one case, the cyst developed from the testis and in another from the paratesticular tissue.

6.The impact of neoadjuvant chemotherapy combined with tumor cell death and postoperative intraperitoneal chemotherapy on efficacy and quality of life of patients with ovarian cancer

Objective To observe the effect of neoadjuvant chemotherapy combined with tumor cells reduction and postoperative intraperitoneal perfusion chemotherapy on the efficacy and quality of life of ovarian cancer.Methods From October 2010 to October 2013 at the Department of gynecology,Taizhou Municipal Hospital,65 cases of ovarian cancer patients for the treatment of stage Ⅲ and Ⅳ were randomly divided into intervention group (n =32) and control group (n =33).Patients in the intervention group received neoadjuvant chemotherapy after tumor cell reduction cytoreductive surgery and received postoperative intraperitoneal hyperthermic perfusion chemotherapy and conventional surgery after chemotherapy.Patients in the control group received conventional tumor cell cytoreductive surgery plus postoperative chemotherapy.Surgical status,the effectiveness of treatment,and adverse reactions were compared between two groups.Moreover,survival quality score,relapse rate,and mortality differences were compared between two groups.Results For the intervention group,operation status and effective treatment were better than the control group.The proportion of adverse reactions had no significant difference between two groups.Survival quality had no statistically significant difference between before treatment and after receiving treatment.Quality of life in the intervention group was better than that of the control group.Relapse rate,and mortality rate in the intervention group was lower than the control group.Conclusions Accepted neoadjuvant chemotherapy combined with tumor cytoreductive surgery and surgery after intraperitoneal hyperthermic perfusion chemotherapy can improve the effectiveness of treatment of ovarian cancers,and improve the quality of life,with better short-term effect.
7.CHANGES IN THE EXPRESSION OF RD-5 mRNA IN THE INTESTINE OF SCALDED RATS AFTER DELAYED FLUID RESUSCITATION

Objective To investigate whether intestinal bacterial translocation in scalded rat with delayed fluid resuscitation (DFR) was partly due to the reduction of the expression of rat defensin (RD), which was the most important defensin in rat intestinal defensin family excreted by Paneth cells. Methods Fifty-six SPF rats were randomized to 3 groups: sham injury group (n=8); early resuscitation (ER) group (n=24), receiving fluid resuscitation immediately after scald (30% TBSA, third degree); delayed resuscitation group (n=24), receiving fluid resuscitation 6 hours after scald. The animals (n=6, at each time point) were sacrificed at 8, 24, 72 hours after injury. The expression of RD-5 in the terminal ileum was determined with PCR technic. Morphological changes in ileal Paneth cells were observed. Quantitative bacterial cultures of mesenteric lymph nodes (MLN), liver, spleen and lung were also done. Results In both ER group and DR group, ileal RD-5 mRNA expression level was significantly increased at 8 hours after injury, and it began to decrease at 24 hours until the end of the experiment, when the level was much lower than that of the sham group. When two experimental groups were compared, RD-5 mRNA expression level was significantly higher at 8 hours and significantly lower at 72 hours in DR group than in ER group. There were no obvious morphological changes in ileal Paneth cells in all 3 groups. Although high incidence of bacterial translocation was observed in both experimental groups, it was significantly higher in DR group than in ER group. Conclusions DFR did not cause Panth cell damage, and RD-5 changes did not contribute to bacterial translocation in 24 hours after DFR, as the increase in RD-5 mRNA might be an innate protective reaction. However, the decrease in RD-5 production at 72 hours may play a role in bacterial transloca- tion.
8.Clinical significance in reserving and reconstructing parotid fascia

Objective To evaluate whether it can prevent Frey's syndrome by reserving parotid fascia or reconstructing parotid fascia with sternocleidomastoid muscle and remaining of parotid fascia in parotidectomy. Methods Fifty-two patients undergoing parotidectomy with maintained and reconstructed parotid fascia were followed up. A test of chewing vitamin C slice guidance and iodine-amylum pilot was conducted for observing the incidence of postoperative Frey's syndrome. Results Flush with perspires was found only in 5 out of the 52 patients (9.61%), which was lower than the incidence after traditional parotidectomy. Conclusion The method of reserving and reconstructing parotid fascia with sternocleidomastoid muscle and remaining of parotid fascia in parotidectomy can efficiently prevent Frey's syndrome, and can also repair the hollow abnormity of the face.
9.Transfection of interleukin 12 and interleukin 2 fusion gene into spleen for treatment of chemically induced hepatocellular carcinoma in rat

Objective To study the inhibitory effect of intrasplenic injection of retroviral packaging cells encoding human interleukin-2 (hIL-2) and mouse interleukin-12 (mIL-12) fusion gene on the growth of chemically induced hepatocellular carcinoma in rats. Methods The retroviral vector GCIL12EIL2PN encoding hIL-2 and mIL-12 fusion gene was constructed. The retroviral packaging cell line PA317 transfected with the vector was injected into the spleens of rats with established chemically induced hepatoma on on the 90th day (early-stage treatment) or the 105th day (late-stage treatment). The survival time and toxic effect were observed. The serum mIL-12 and hIL-2 levels were assayed with ELISA, and the cytotoxicity of the natural killer (NK) cells was measured by means of a 51Cr-release assay using YAC-1 tumor cells as the target. Results The average survival time (after chemical induction) in the early-stage treatment rats and the late-stage treatment rats were 188.1?14.2 days and 168.5?13.6 days, respectively, in IL-12+IL-2 combination gene treatment group, and it was longer than that of IL-12 gene treatment group (168.2?13.4 days and 149.1?13.8 days, respectively, P
10.Effect of low molecular weight heparin on hypercoagulability in extensive severe burn casualties

Objective To observe the effect of low molecular weight heparin(LMWH)on hypercoagulability in extensive severe burn patients.Methods 9 patients were assigned as LMWH treatment group(TG),and another 12 burn patients with the similar extent of burn injury admitted in the same period were assigned as routine treatment group(RTG).All the treatments were the same except 5000U LMWH was given via subcutaneous injection per 12 or 6 hours to the TG group for 3-7days.The patients in normal control group(NCG)consisted of patients receiving plastic surgery.The following coagulation parameters were determined before and after the heparin treatment:prothrombin time(PT),thrombin time(TT),activated partial thromboplastin time(APTT),international normalized ratio(INR),fibrinogen(Fib)and platelet(PLT).Results INR,APTT and PLT in both TG and RTG groups were significantly lower compared with that in NCG before treatment,and Fib in two former groups were higher than that in NCG.Compared with those values before treatment,Fib decreased and PLT increased significantly after LMWH treatment in TG.However,there were no significant changes in other parameters in TG after LMWH treatment.Severe side effects such as haemorrhage were not found in patients in TG.Conclusion LMWH could ameliorate hypercoagulability following severe burns.
