1.Analysis on Rational Use of Cephalosporins in 583 Cases of a Large General Hospital

OBJECTIVE To study the rational use of antibiotics in a comprehensive general hospital.METHODS The clinical data of 583 cases used the five kinds of most frequently antibiotics during Nov 20-26,2006 in a comprehensive general hospital were collected and analyzed.RESULTS The total five kinds of most frequently antibiotics used in this hospital were the third generation of cephalosporins,cefoperazone/tazobactam(Kaisi),flomoxef(Fuomaning),cefoperazone/tazobactam(Kaishute),cefpiramide(Xibaiao) and cefoperazone/tazobactam(Xianbita).The daily frequency of these antibiotics was rational to the drug metabolic dynamics characteristics.Some patients received the long time therapy of antibiotic,the longest one was 22 days.The frequency of over-doses in these patients was 19.9%.CONCLUSIONS Antibiotics use is apparently rational in this hospital and the training and education for drug use should be enhanced in the future.

Objective To compare the therapeutic effect and safety of naloxone ,flumazenil and naloxone combined wiht flumazenil in treating acute diazepam poisoning and to find out ideal therapeutic treatment.Methods 45 patients were divided into three groups at random: naloxone group (n=15), flumazenil group (n=15) and naloxone combined with flumazenil group (n=15). Besides given the same routine therapy to each group, naloxone group received 0.4mg naloxone through intravenation and 1.2mg naloxone through intravenous drip; flumazenil group received 0.1mg flumazenil through intravenation and 0.9mg flumazenil through intravenous drip; naloxone combined with flumazenil group received 0.4mg naloxone and 0.1mg flumazenil through intravenation and 1.2mg naloxone and 0.9mg flumazenil through intravenous drip. Each patient's consciousness status was graded after given drug treatment 0, 5,15,60,90,180 minute. Testing the BR, SR, CK, CK-MB of every patient and recording to the Bp, HR, RR, SpO 2 and the rhythm of the heart continuously. Objective To study the effect of testosterone on skeletal muscle atrophy of rats induced by dexamethasone,and to explore the relationship between androgen and ubiquitin ligase expression in skeletal muscle.Methods Forty female SD rats were randomly divided into four group:control group(CON),dexamethasone group(DEX),testosterone group(TES)and testosterone plus dexamethasone group(TES+DEX).Blood samples were collected from the abdominal aorta and then centrifuged.The supernatant was collected and stored at-20℃.Gastrocnemius was removed and stored at-80℃.The cross-sectional area(CSA)of muscle fiber was determined by microscopic images of digitized muscle sections,using a computer-based imaging processing system.The expression of muscle atrophy F-box(MAFbx)and muscle RING finger-1(MuRF1)mRNA in skeletal muscle was determined by real-time PCR.The expression of testosterone protein in serum was determined by immunoradiometry.Results Dexamethasone increased the expression of MAFbx and MuRF1 mRNA in skeletal muscle(P

6.Surface ultrastructure of Parvatrema timondavidi (Digenea:Gymnophallidae) according to its developmental stages.

Surface ultrastructure of Parvatrema timondavidi developmental stages was studied using a scanning electron microscope. The metacercariae were collected from the marine clam, Tapes philippinarum, and juvenile and worms adult were recovered at 1, 2, 3, and 7 days after experimental infection of mice. The metacercariae had a large oral sucker and characteristic lateral projections. Around the lip of the oral sucker many type I and type II sensory papillae were observed, and type III papillae were located symmetrically on the medial side of the lateral projection. Numerous type I papillae were grouped around the genital pore. The tegumental spines were distributed over the worm surface except the lip of the sucker and genital pore. The 1-day old worm had a well-developed ventral sucker, with 6 type II sensory papillae on its outer surface and another 6 type I papillae on the inner side, Two small type I papillae were seen on the anterior side of the ventral sucker. The genital pore was and 15 type I papillae were grouped around it. The 2-, 3-, and 7-day worms revealed that as they grew to be adults, the spine tips became multipointed, the genital pore formed a genital atrium, and the cytoplasmic process became well differentiated. In 2- and 3-day worms 10 type II papillae encircling the lip of the oral sucker, and additional 4 papilled at the dorsal side of 4 dorsal type II papillae were a characteristic feature. The distribution pattern of sensory papillae around the oral sucker and genital pore, and 2 type I papillae on the anterior side of the ventra sucker, was so peculiar in P. timondavidi, that they seem to be useful keys for taxonomic differentiation from other gymnophallids.

7.NBI system versus conventional endoscopy in diagnosis of colorectal adenomas: a systematic review

Objective To evaluate the validity of narrow-band imaging (NBI) system in detection of colorectal adenoma, as compared to that of endoscopy of the colon and rectum, in a systematic review. Methods Relevant literatures were retrieved from Medline (January 1966 to October 2008), OVID (January 1996 to October 2008), EMBASE (January 1980 to October 2008), Coehrane Library (Issue 3, 2008) and Chinese Biological Medicine Disk (CBM disk, January 1997 to October 2008). Quality of the literatures retrieved was assessed based on the Cochrane Reviewers' Handbook and Jadad's score. RevMan version 4. 2 software was used for meta-analysis. Results Seven randomized clinical trials (2838 patients) were included in the study. Compared with white-light colonoscopy, no significant difference was observed in terms of adenoma detection rate (OR 1.18, 95% CI 1.00-1.39, P=0.06) by NBI system, which could significantly improve total number of detection for fiat lesions of the colon and rectum (pooled WMD 0.14, 95% CI 0.02-0.26, P=0.02), but with a longer withdrawn time (pooled WMD 1.05, 95% CI 0.08-1.22, P<0.01). Conclusions Detection rate for flat lesions of the colon and rectum, not for adenoma, can be improved by NBI system and meanwhile its withdrawn time is prolonged, indicating that routine use of NBI system for detecting colorectal adenomas may be recommended only with its further refined technique.

Objective To investigate the clinical effect of methotrexate combined with dilatation and curettage on the treatment of cesarean section scar pregnancy and the effect on HCG (chorionic gonadotropin). Methods 130 cases with cesarean section scar pregnancy admitted in Taizhou hospital from January 2016 to March 2017 were randomly divided into two groups. The control group wasreceived dilatation and curettage. The study group was given methotrexate combined with dilatation and curettage.The effect and the influence to HCG in the two groups were compared. Results The effective rate in the study group was significantly higher than that in the control group (P<0.05). There was no significant difference in HCG level between the two groups before treatment. The levels of HCG in the study group were significantly lower than that in the control group (P<0.05). Conclusion Methotrexate combined with curettage on the treatment of cesarean scar pregnancy, can improve the therapeutic effect, reduce postoperative bleeding and hospitalization time, reduce the level of HCG.
9.Effect of morphine preconditioning on mitochondrial permeability transition pore after myocardial ischemia-reperfusion injury in rats

Objective To investigate the effect of morphine preconditioning on mitochondrial permeability transition pore (MPTP) and its protective mechanism after myocardial ischemia-reperfusion injury. Methods A rat model of ischemia-reperfusion injury was established. Forty rats were injected with 2-3[H] DOG and then divided into 4 groups randomly: a sham operation (S) group, an ischemia-reperfusion injury (IR) group, a morphine preconditioning (Mp+IR) group, and a cyclosporine A preconditioning (CsA+IR) group. We monitored the concentrations of serum creatine kinase-Mb (CK-Mb) and cardiac troponin I (cTnI), and measured myocardial mitochondrial 2-3[H] DOG, cytochrome c content, Ca2+ concentration ([Ca2+]m), the velocity of Ca2+ intake and reaction half time of mitochondrial permeability transition pore (MPTP t1/2) in the 4 groups. Results The concentrations of serum CK-Mb and cTnI decreased more in the Mp+IR group and the CsA+IR group than those of the IR group. The concentrations of 2-3[H]DOG and [Ca2+]m in the IR group were evidently higher but the level of cytochrome c was lower than those of the sham operation group. The concentrations of 2-3[H] DOG and [Ca2+]m in the Mp+IR group decreased whereas the concentration of cytochrome c increased compared with those in the IR group. Mitochondrial 2-3[H]DOG content was positively correlated with the concentration of calcium (r=0.797, P<0.01). The 2-3[H]DOG and [Ca2+]m content were negatively correlated with cytochrome c in the IR group (r=-0.805 and r=-0.648, respectively, P<0.01). MPTP t1/2 in the IR group was shortened evidently, and that in the Mp+IR and CsA+IR group was significantly lengthened. Conclusion Morphine preconditioning may have myocardial protective effect through unburdening the calcium overload and lengthening the MPTP t1/2.
