2.Splenic Lymphangioma: A case report.

Lymphangioma of the spleen is a rare neoplasm and approximately 100 cases have been reported in the world literature. This lesion may involve the spleen alone or may be a part of a syndrome of multivisceral involvement. The pathogenesis is unclear whether this is a true neoplasm of a developmental defect. We report a case of a 87-year-old woman with solitary splenic lymphangioma. The cut surface mass is not encapsulated and located at the subcapsular area in the mid portion of spleen. It is composed of variable sized cystic spaces containing myxoid tissue. Microscopic finding reveals dilated lymphatic spaces lined by endothelial cells and the content is proteinaceous eosinophilic fluid.

3.Intracoronary Radiation Therapy Using Re-188 after percutaneous Coronary Angioplasty.

Percutaneous coronary angioplasty is well established therapeutic modality in the management of coronary artery disease. However, the high restenosis rate of 30 to 50% limits its usefulness. The principal mechanism of restenosis, ntimalhyperplasia, is the proliferative response of vessel wall to injury, which consists largely of smooth muscle cells. A large body of animal investigations and a limited number of clinical studies have established the ability of ionizing radiation to reduce neointimal proliferation and restenosis rate significantly. Human studies have been reported that intravascular radiation after first restenosis inhibits a second restenosis. Encouraged by these reports, we are also conducting a double blind, placebo-controlled, randomized trial to evaluate this new therapeutic modality in patients with coronary artery stenosis. The objective of our trial is to determine the safety and efficacy of catheter-based solutional beta emitting radioisotope system in preventing restenosis after angioplasty. This review describes the vascular brachytherapy systems and isotopes that have been utilized in the initial clinical trials performed in this area of post PTCA coronary restenosis. The results of many worldwide ongoing clinical trials will determine whether this new technology will change the future practice of vascular intervention.

4.A Case of Cutaneous Hemorrhagic Bullous Eruptions in Lymphoma of the Small Intestine.

Authors experienced a 59-year-old male patient with the lymphoma of the small iritestine which involved the adjoining mesenteric lymph nodes, the parietaI pleura, and the skin. The skin lesions, simulating necrotizing angiitis, were deep purple colored, asymptomatic, tense, hemorrhagic bullae with soft consistency on the both thighs and left infraorbital area. He had extranodal type, and diffuse, poorly differentiated lymphocytic lymphoma by the Rappaport classification, and stage IV by the Ann Arbor staging classification. After about 50 days of clinical manifestations, he was died mainly due to massive intestinal bleeding.

5.Prevalence of HBV DNA in Packed Red Blood Cells.

Assays for HBsAg, HBV DNA, anti-HBc and anti-HBs of 285 units of packed red blood cells supplied by Taegu Red Cross Blood Center were performed to evaluate the correlation between the prevalence of HBV DNA and the serologic markers for hepatitis B virus. None of 285 plasma samples was positive for HBsAg, however, HBV DNA were detected by polymerase chain reaction in 2 samples which both presented only with anti-HBc positivity.. Of 204 samples tested for anti-HBs, 96 samples(47.1%) were positive and among 216 samples tested for anti-HBc, 80 samples(37.0%) were positive. Of 193 samples tested for both anti-HBs and anti-HBc, 80(41.1%) were all negative and 48(24.9%) were positive on both tests. Those samples which showed positivity only to anti-HBc were 25(13.0%). Considering the above results, transfusion-transmitted hepatitis B virus infection could be prevented by discarding anti-HBc positive blood, however, that may bring insufficient supply of donor bloods in the country like Korea where the prevalence of anti-HBc is high. Anti-HBc positive blood unequivocally positive for anti-HBs should be considered noninfectious for HBV and should be allowed to be transfused. It would reduce the amount of discarding donor blood as the routine blood donor screening tests presently used at Korea Red Cross Blood Center supplemented by anti-HBs and anti-HBc testing.

8.Glycerol Interference on the Measurement of Triglyceride Concentration.

BACKGROUND: As more than 80% of Korean hospital laboratories don't use glycerol blank for the triglyceride measurement, we investigated free glycerol interference on the measurement of triglyceride concentration. METHODS: We collected 237 specimens which had more than 300mg/dL of triglyceride measured by Abbott (without glycerol blank, USA) reagent from the 217 patients visiting Catholic University Hospital of Taegu Hyosung from September, 1995, to May, 1996. We retested them with Youngdong (without glycerol blank, Korea) reagent, and IRC reagent (with glycerol blank, Japan), and also measured free glycerol concentration with Youngdong reagent. Then we examined the clinical records of the patient showing increased glycerol concentration. RESULTS: Average triglyceride concentration measured by Abbott reagent was 448.0+/- 165.9 mg/dL and average glycerol interference to triglyceride concentration was 3.4+/- 6.7%, There were 8 patients (3.4%) who revealed more than 10 % of glycerol interference. Except these patients the average glycerol interference was 2.4+/- 1.4%. Among these 8 patients, 3 patients were treated with Frucenil and other 3 were with Intralipose(R), and the other 2 were diagnosed as diabetes and preeclampsia respectively. In vitro test, Frucenil mixed with serum in 1% (v/v) increased triglyceride concentration as high as 640.6% with Abbott reagent, but it did only 48.4% with IRC reagent. CONCLUSIONS: Triglyceride measurement by non glycerol blank method exceeded the recommended limit of National Cholesterol Education Program's precision, and this method extremely overestimated the triglyceride concentration for the patient with the glycerol containing fluid therapy. So all laboratories would be encouraged to use the glycerol blank method for triglyceride measurements.
