1.A Case of Subungual Exostosis.

Subungual exostosis is not a true exostosis, but an outgrowth of normal bone tissue, which is traumatic disputedly in causative factor, involving mainly the great toe. We experienced a case which a hard nodule, l. 0 crn in diameter, of the great toe of the left foot, developed in a 24-year-old woman. Her lesion had begun about 6 months earlier, accompanying the dull pain or tenderness when wearing the shoes or walking. Confirmatory diagnosis was biopsy with biopsy and X-ray findings after nail avulsion. We performed the lesion with curettage on therapeutic trial without effect.

2.Normal Sizes and Weights of Internal Organs of the Korean Young Male Adults.

In autopsy, the evaluation of the size and weight of th internal organs is very important for the understanding of the pathologic conditions. The normal data of the internal organs must have regard to size, weight, sex, age and body weight. There are many reports on the normal value of human organs in Japan and other Western countries. But there are some variations in normal value according to race, geographics and living standard. In Korea, Lee & Roh had reported on weights of various organs of Korean and their proportional weights to body weights in 1957, based upon autopsy records from 1929 to 1941. But, there has been many improvement in socioeconomic status during past half-century in this country. And the Korean body conditions have much improved. Upon the base of above consideration, normal data fit for present Korean is mandatory. We have analysed records of 45 cases of Korean young adults's autopsy cases which were performed from Jan. 1984 to Dec. 1985, and obtained following data. 3.Syphilis Prevalence in Teagu Hope Village.

Serologic tests for syphilis, including VDRL and TPHA tests, and Physical examination were carried out on 1, 605 subjects who lived in Taegu Hope Village, a social welfare center, from April, 198R to May, 1985. We prepared this study comparing with previous reports for healthy individuals and prostitutes. The results were as follows, The reactive rate of VDRL. qualitative test was 11. 6% in total, 10. 3%, in men and l3. 5%. in women. The reactive rates of VDRL. qualitative tests were highest in 7 th decade as 28, 7% and followed by 6 th decade as 16. 7% the decacile as 16. 0% 8 th decade as 14,3% 4th decade as 8. 8% and 3rd decade as 5. 9%, 3 In ]Hg sero-positive subjects, VDRL quantitative tests were done, and 35. 5% were reacted with 1: 8 or high and 29. 0% had one or more syphilitic lesions. 4 In 54 subjects who had one or more syphilitic lesions, 19 had chancres, 17 had alopecia syphilitica, 7 had rnaculopapular syphilid, 7 had condylomata lata. 3 had papular syphilid, and 1 had maculopapular syphilid and alopecia syphilitica. The biological false positive rate was 10. 1,4 in 138 VDRL reactive subjects, using 1;he TPHA test standard.

4.Two Cases of Keratosis Palmaris et Plantaris.

We described here-in a 36-year-old male patient (case 1) with a severe form of keratosis palmaris et plantaris(Thost-Unna type) which was diffuse hyperkeratosis with hyperhidrosis on the entire surface of the palms and soles. His son, 8-year- old boy, showed symmetrical distribution of hyperkeratotic patches on the inner sides of both great toes(case 2). Therefore, it was suggested this genodermatosis occured in two generations of a family. The biopsy taken from the right palm of case 1 showed features of epidermal hyperplasia with hyperkeratosis, hypergranulosis and acanthosis. We tried the oral Vit. A therapy(50,000 unit, daily) in case 2. The hyperkeratotic lesions became slightly improved in 4 months after therapy.

5.A Case of Cutaneous Hemorrhagic Bullous Eruptions in Lymphoma of the Small Intestine.

Authors experienced a 59-year-old male patient with the lymphoma of the small iritestine which involved the adjoining mesenteric lymph nodes, the parietaI pleura, and the skin. The skin lesions, simulating necrotizing angiitis, were deep purple colored, asymptomatic, tense, hemorrhagic bullae with soft consistency on the both thighs and left infraorbital area. He had extranodal type, and diffuse, poorly differentiated lymphocytic lymphoma by the Rappaport classification, and stage IV by the Ann Arbor staging classification. After about 50 days of clinical manifestations, he was died mainly due to massive intestinal bleeding.

7.Obesity Indices and Obesity-Related Quality of Life in Adults 65 Years and Older.

BACKGROUND: Obesity is known to influence on physical, mental, functional health problems but there have been no study of relationship between body mass index (BMI) and quality of life (QOL) in Korean elderly. METHODS: A total of 216 elderly who had been attending two geriatric welfare facilities in Seoul answered the Korean version of obesity-related quality of life (KOQOL) questionnaire. Height, weight, waist circumference were measured. Body fat (%) was measured with HTM20 by impedance technique. Sociodemographic and medical factors were interviewed. QOL was assessed using KOQOL (ver.1) after excluding two sexual life related questions and one work related question. The total score is 48 points. The higher is the score, the poorer is the QOL. RESULTS: In multivariable analysis, BMI, age, history of diabetes mellitus, history of osteoarthritis were associated with KOQOL score. Especially, BMI was significantly associated with KOQOL score after adjusting for waist circumference, body fat (%) (P = 0.002). CONCLUSION: The more BMI increases, the worse the obesity related QOL is in Korean ambulatory elderly.

8.Surgical Outcomes in Patients with Intermittent Exotropia with Siblings Who Underwent Surgery by the Same Surgeon

Purpose:
We evaluated surgical outcomes in pediatric patients with intermittent exotropia with and without a family history of exotropia surgery performed by the same surgeon.
Methods:
We conducted a retrospective review of the medical records of pediatric patients aged ≤ 18 years who underwent surgery for intermittent exotropia between September 2015 and December 2021. We compared surgical outcomes between patients with and without a family history of exotropia surgery performed using 1:1 propensity score matching. A successful surgical outcome was defined as achieving alignment between an exodeviation of 10 prism diopters (PD) and an esodeviation of 4 PD.
Results:
This study included 60 patients with a family history of exotropia surgery and 60 matched controls with no such history. Notably, the log-rank test revealed no significant differences in surgical outcomes between the two groups (p = 0.622). Furthermore, clinical characteristics, including the onset of exotropia, degree of ocular deviation, and age at surgery, were similar between first- and second-born siblings. Furthermore, no significant differences were observed in surgical outcomes between the two groups (p= 0.540).
Conclusions
Patients with a family history of exotropia surgery do not exhibit significantly different surgical outcomes compared to those with no such history. In addition, no significant differences were observed in surgical outcomes between the first- and second-born siblings.
9.Optic Chiasmal Enhancement Revealed by Magnetic Resonance Imaging in a Patient with Ethambutol-induced Toxic Optic Neuropathy

Purpose:
We report a bitemporal visual field defect accompanied by optic chiasmal enhancement in a patient with ethambutol (EMB)-induced toxic optic neuropathy.Case summary: A 53-year-old woman presented with bilateral visual disturbance that had gradually worsened over the prior 2 months. She had been prescribed 800 mg EMB daily to treat pulmonary tuberculosis for the past 10 months but had stopped EMB 1 month prior because of visual disturbance. She had been prescribed 600 mg rifampicin and 1,000 mg clarithromycin daily for 11 months. Her visual acuity was 0.2 in the right eye and 0.1 in the left. Color vision was reduced. Automated perimetry demonstrated a bitemporal visual field defect, which was more severe in the left eye. Magnetic resonance imaging (MRI) of the optic chiasm revealed contrast enhancement in the body of the chiasm. The optic nerve of the left eye evidenced a higher signal intensity than did that of the right, consistent with the visual field test results. One month after her initial visit, the visual symptoms deteriorated further. We delivered high-dose, intravenous steroid pulse therapy (methylprednisolone 1,000 mg daily for 3 days). At the 1-year follow-up, the visual acuity was 0.4 in the right eye and 0.1 in the left.
Conclusions
Bitemporal hemianopia may develop in patients with EMB-induced toxic optic neuropathy. Enhancement of the optic chiasm, consistent with the visual field defect, could be observed in MRI.
10.A Case of Fibroadenoma Arising in Accessory Axillary Breast Tissue.

A 25-year-old woman had a 2-month history of a 1.7 x 1.4cm sized lirm nodule, which was deeply located in 5.0 x 4.0cm sized soft fatty tissue on the right axilla. There was no history of axillary swelling with the symptoms or signs related to her menstrual periods. She underwent an excisional biopsy of the firm nodule and a simple biopsy of the soft tissue. The histopathologic findinga associated with both lesions revealed the usual feat Ures of fibroadenoma with accessory breast tissue. There was no occurrence or relapse of any other lasions during the careful one-year follow-up after excision of the firm nodule. This is an unusual case in a review of Korea literatures.
