2.Corneal Curvatures of the Infants.

Eyeball grows rapidly in the early years of life. The growth of the cornea, lens and axial length of the eyeball influences the refractive state of the eye. Axial length of the eyeball grows rapidly and it causes myopic changes of the eye. But decrease of the power of the cornea and lens is accompanied, so that the refractive state of the eyeball does not change rsignificantly in adult compared that of the infant. By using A-O keratometer, the corneal curvatures of the infants were examined in 18 infants(male 9, female 9, gestational age 29~42 weeks) and the results showed that the corneal curvatures were steepist in the youngest premature newborn and decreased significantly with age.

3.Cryotherapy in a patient with acquired tufted angioma.

A 26-year old woman had tender erythematous grouped papules and plaque(10 cm in diameter) with underlying deep seated rodules on the right shoulder for 2 years. A biopsy specimen showed vascular tufts composed of endothelial cells in the mid and lower dermis, which was diagnostic of acquired tufted angioma. After 4-months of cryotherapy with liquid nitrogen, most of the lesions had disappeared as well as her subjective symptom.

5.Abrasion of abutment screw coated with TiN.

STATEMENT OF PROBLEM: Screw loosening has been a common complication and still reported frequently. PURPOSE: The purpose of this study was to evaluate abrasion of the implant fixture and TiN coated abutment screw after repeated delivery and removal with universal measuring microscope. MATERIAL AND METHODS: Implant systems used for this study were Osstem and 3i. Seven pairs of implant fixtures, abutments and abutment screws for each system were selected and all the fixtures were perpendicularly mounted in liquid unsaturated polyesther with dental surveyor. After 20 times of repeated closing and opening test, the evaluation for the change of inner surface of implant and TiN-coated abutment screw, and weight loss were measured. Mann-Whitney test with SPSS statistical software for Window was applied to analyze the measurement of weight loss. RESULTS: TiN-coated abutment screws of Osstem and 3i showed lesser loss of weight than non-coated those of Osstem and 3i (P < .05, Mann-Whitney test). CONCLUSION: Conclusively, TiN coating of abutment screw showed better resistance to abrasion than titanium abutment screw. It was concluded that TiN coating of abutment screw would reduce the loss of preload with good abrasion resistance and low coefficient of friction, and help to maintain screw joint stability.

6.A Case of Juvenile Spring Eruption of the Ears.

Juvenile spring eruption(JSE) of the ears is an unusual type of photodermatosis, which develops on the light exposed areas of the ears of boys and young male adults in the early spring months. JSE has received little attention in the literature, and to our knowledge no cases have been reported in Korea until now. Herein we report a case of JSE occurring in a 17-year-old man who has suffered from a recurrent pruritic erythematous papulovesicular eruption of both helix, followed by crusting and healing without scarring within one to two months early each spring for six years.

8.Common bile duct and Gall Bladder Varices: Findings of ERCP and Doppler ultrasonography.

After portal vein occlusion, portal to portal collaterals (hepatopetal) develop from preexisting periportal vessels or recanalization of the thrombosed portal vein, undergo compensatory enlargement, bypass the obstructed extrahepatic occlusion and reconstitute the intrahepatic portal branches. Angiographically, collateral veins are seen as multiple tortous winding veins in the porta hepatis and are described as a cavemous transformation of the portal vein. When the common bile duct or gall bladder is compressed by collateral veins, a cholangiogram demonstrates multiple smooth intramural defects and jaundice can develop due to the partial obstruction of the bile duct. Demonstration of the cavernous transformation of the portal vein can be done by ultrasonography, abdominal computed tomography and nuclear magnetic resonance, but Doppler ultrasonography and direct or indirect portography are needed to evaluate its hemodynamic change. We present a 35-year-old female patient complaining repeated jaundice, in whom common bile duct and gall bladder varices accompanied by cavernous transformation of the portal vein and intrahepatic stones were diagnosed by ultrasonography, abdominal computed tomography, ERCP, and Doppler ultrasonograpy and confirmed by surgery. ERCP demonstrated the irregular contour of the common bile duct and gall bladder due to multiple smooth intramural defects. Doppler utrasongraphy revealed the unique flow signal of portal vasculature from the tortous vessls in the porta hepatis and from the vascular structures on the wall of the gall bladder.

9.Operative Treatment of Fracture of the Tibial Condyle

The fracture of the tibial condyle involving the articular surface can produce some disability of the knee joint because it is frequently accompanied by soft tissue injury to the ligaments and menisci. Accurate anatomical reduction and rigid internal fixation with early mobilization is known to provide good results. The authors analysed 50 cases of the fracture of the tibial condyle which were treated with recent method(anatomical reduction, rigid fixation and early joint motion) at the Dae Han Hospital from January, 1988 to May, 1993. The results were summerized as follows: 1. The associated injuries were ligamentous injury in 24 cases(48%), fibular fracture in 21 cases(42%), femur fracture in 7 cases(14%) and meniscal injury in 7 cases(14%). 2. According to Hohl's classification, the split compression type was most common in 17 cases(34%), and there were 14 comminuted type(28%), 8 local compression type(16%), 7 total condylar compression type(14%) and 4 split type(8%). 3. Method of internal fixation were plate & screw in 32 cases(64%), tibial bolt & screw in 18 cases(36%), bone graft in 31 cases(62%), ligament repair in 24 cases(48%) and menisectomy in 4 cases(8%). 4. The clinical result by Insall's criteria were excellent in 37 cases(74%), good in 8(16%), fair in 3(6%), poor in 2(4%) among 50 cases.

10.Treatment of Colles' Fracture with Percutaneous Pinning

Colles' fracture is a common injury encountered in emergency departments. Despite the frequent experience of every orthopedic surgeon with Colles' fracture, considerable variation remains in the treatment of fracture of the distal radius. Everyone nearly agress that reduction of the fracture is not difficult, but maintenance of reduction is the problem. Eighty patients with Colles' fracture were treated with closed reduction and percutaneous pinning under the C-arm fields and followed up more than 1 year, from January 1990 to May 1993 at the Department of Orthopedic Surgery, Dae Han Hospital. The results of this study were as follows: 1. The fractures were classified according to the system of Gartland and Werley. Of the 80 cases, 12(5%) were type 1; 25(31%) type 2; and 43(54%) type 3. 2. Complications were such as; temporary neuropathy of median nerve 2 cases, redisplacement 2 cases, radiocarpal arthritis 1 cases, stiff hand 1 case and shoulder hand syndrome 1 case. 3. The overall assessment showed exellent in 29 patients(36.2%), good in 44 patients(55,0%) and poor in 7 patients(8.8%) under the subjective and objective criteria of Gartland and Werley and the objective criteia of Scheck. 4. The results were mainly correlated with maintenance of an anatomical reduction. 5. The percutaneous pinning was suitable for anatomical restoration and maintence, had less com plication and more advantage such as the early range of joint and simple technique. Conclusively, it was considered to be one of ideal method for the treatment of Colles' fracture in selected cases.
