3.The Effect of Nerve Root Block on the Patients Who Are Considered as Surgical Candidates with Cervical Radicular Pain.

PURPOSE: This study was conducted to evaluate the effect and usefulness of cervical selective nerve root block in advance for the patients who was scheduled for operation. MATERIALS AND METHODS: Between January 2001 to December 2008, 28 cases were selected from the patients, who underwent selective nerve root block before operation. The mean follow-up period was 30.4 months and the mean age was 59.2 years. The patients were divided into the operation group and the only cervical nerve root block group. The groups were compared for age, gender, the contraction period and the, solidity of the disc. The groups were radiologically evaluated for the existence of cervical spondylosis and the neural compression rate. The clinical results were analyzed using the neck disability index (NDI) and the visual analogue scale (VAS) score. RESULTS: Among the 28 cases that were scheduled for operation, 18 cases did not proceed to operation and 10 cases among them were treated with additional conservative treatment. The average VAS score for the operation group and the cervical nerve root block group was 8.3 and 7.7 at the preoperative state and the preinjection state, respectively. After surgical treatment and cervical nerve root block, the scores were 3.2 and 3.6, respectively. The NDI for the operation group was 40.6 at the preoperative state and this decreased to 11.2 at the last follow up. The NDI for the cervical nerve root block group was 38.4 at the preinjection state and this decreased to 13.6 at the last follow up. The mean contraction period was 3.2 months and the mean age was 51.9 years for the operative group. The mean contraction period was 14.82 months and the mean age was 55.9 years for the root block group. There was no difference of clinical outcomes between the operative group and the cervical nerve root block group. The operation group's mean age was younger and the duration of symptom was shorter than those of the cervical nerve root block group. CONCLUSION: Cervical nerve root block is considered to be an effective treatment for patients with cervical disc herniation with radiating pain and who are scheduled for an operation.

4.Relaxation Effect of Synthetic Ceramide Analogues in Cat Esophageal Smooth Muscle Cells.

Ceramide has emerged as a novel second messenger for intracellular signalling. It is produced from sphingomyelin and is involved in the control of cell differntiation, proliferation, and apoptosis. C2- ceramide, short chain ceramide, plays a role in mediating contraction of cat esophageal smooth muscle cells. We examined the effect of synthesized ceramide analogues on the C2-ceramide and ACh-induced contraction in esophageal smooth muscle cells isolated with collagenase. CY3523, CY3525, or CY3723 inhibited C2-ceramide induced contraction, in a time dependent manne. Each analogue also inhibited the contraction in concentration dependent manners. CY 3523, CY 3525, and CY 3723 had no effect to the contraction induced by PMA. The inhibition with CY3523, CY3525 and CY3723 on the C2- ceramide induced contraction was recovered by PMA. These analogues decreased the density of MAPK bands (p44/42 or p38) in the western blot. These results suggest that ceramide analogues can inhibit C2-ceramide induced contraction via PKC and MAPK dependent pathway.

5.Clinical Outcomes of Cervical Stellate Ganglion Block in Patients with Secondary Lymphedema: A Pilot Study.

OBJECTIVE: To investigate the clinical effect of stellate ganglion block in patients with secondary lymphedema after breast cancer treatment. METHOD: In a prospective clinical trial, the consecutive stellate ganglion block (SGB) was performed every two weeks to 10 patients (Mean age 46.0+/-9.4 (yr)) who was diagnosed as of secondary lymphedema after breast cancer treatment. The parameters were the circumstance of arm and the thickness of skin and subcutaneous tissue of the affected arm measured by ultrasonography. We measured these parameters with baseline value before SGB treatment and repeated the evaluation after each SGB treatment. And the subjective data of satisfaction, softness and improvement were obtained by questionnaires. RESULTS: 10 patients were treated with stellate ganglion block. The mean circumferences of upper arm and forearm after three consecutive SGB treatments reduced significantly: upper arm from 31.39 cm to 29.72, forearm from 25.14 cm to 23.64 cm (p <0.05). And the thickness of subcutaneous tissue of upper arm significantly decreased under no- compression measuring technique after three SGB treatments (p <0.05). The compliance of superficial tissue showed significant change with serial procedure. Almost all of the patients had a satisfaction and feeling of softness in their affected arm after treatments. CONCLUSION: The cervical stellate-ganglion block reduced the circumstance and the thickness of arm with lymphedema and satisfied almost lymphedema patients. So this treatment presents a favorable outcome to the breast cancer-related lymphedema patient clinically.
