1.Four Cases of Intrapulmonary Hamartoma: An ultrastructural study.

Tumors of the lung and bronchi containing cartilage were known by a variety of names, chondroma, adenochondroma, chondromatous hamartoma and mixed tumor. This variation in nomenclatures explain the difference of illustration on the nature of these tumor. The concept pulmonary harmatomas are benign neoplasm and not developmental malformations, has gained wide acceptance in recent years. We have experienced four cases of intrapulmonary hamartoma which were all discovered during routine chest film check up for certificate of health and evaluation of other disease. One case is added further detailed histologic examination by electron microscopy. The age at time of the detection were 53 (male), 23 (male), 39 (female), and 56 (female) years old. The mean size is 4.3x3.7x3.4 cm. The locations were three left upper lobes and one right upper lobe. Lobectomy and wedge resecions were done. Cut surface showed promiment lobular structures, papillary configuration and multiple cleft like spaces. Predominant cellular components were cartilage but fat tissue in one of the four cases. Microscopic findings showed abundant hyaline cartilages bearing lobular configuration and overlying pseudostratified ciliated columnar and cuboidal epithelium. Fibromyxoid and undifferentiated cells were seen in myxoid and fatty tissue. Electron microscopic findings revealed stellate, undifferentiated mesenchymal cells bearing collagen formation, stellate smooth muscle and transition areas between undifferentiated mesenchymal cells and mature cartilage. Epithelial components were similar to terminal bronchiole and alveolar epithelium. These findings suggest the concept that intrapulmonary hamartoma represent a histologic specturm of benign mesenchymal neoplasms, which originate in peribronchial connective tissue.

2.Metastatic Ovarian Carcinoma: Clinicopathologic analysis of 33 cases.

Thirty three cases of metastatic ovarian carcinoma were analysed cilincopathologically. The patient's age ranged from 20 to 68 years, the average being 44.3. Most ovarian tumors were bilateral and asymmertically enlarged, firm and occsaionally multicystic. Histologic examination revealed adenocarcinoma in 31 cases, of which Krukenberg tumors in 14; squamous cell carcinoma in 2. The most common primary site was gasrtointestinal(75.8%), followed by genital, breast and gallbaldder. Most Krukenberg tumors were originated from poorly differentiated adenocarcinoma of stomach. The ovarian and primary carcinomas were synchronously diagnosed in 15 cases, while in 3 cases the primary carcinoma was not round until the ovarian tumor had been removed.

3.Diagnostic Significance of Cold Agglutinin and Antimycoplasma Antibody for Mycoplasma pneumoniae Infection.

A study to evaluate the diagnostic significance of M. pneumoniae Infection by measurements of cold agglutinin and antimycoplasma antibody titers is performed with 191 pediatric patients who have visited Yeungnam University Hospital during the period through January to July, 1987. Forty eight of 191 cases made follow up tests feasible. The results obtained are as follows: 1. It is necessary to perform routine combined measurements of cold agglutinin and antimycoplasma antibody titers for the all pediatric pneumonia caser since a large proportion of pneumonia in children is caused by M. pneumonia. 2. For the diagnosis of M. pneumoniae Infection, measurements of cold agglutinin titer alone seems to be less significant than to check both cold agglutinin and antimycoplasma antibody titers. 3. The measurement of antimycoplasma antibody titer appeared to be more specific than cold agglutinin test in the diagnosis of M. pneumoniae Infection. 4. The present study urges the necessity of follow up study of cold agglutinin and antimycoplasma antibody titer for those who initially presented with normal titers in both tests, but are clinically suspected for M. pneumoniae Infection.

5.Economic Analysis of Electronic Patient Record.

This study was carried out to analyse the economic feasibility of EPR. A hospital with 500 beds under the construction was selected for this study. The economic effect was performed with cost-benefit analysis for 5 years from the year 1998 to 2002. The results of this study were as follows; The economic effect of EPR was classified into direct benefit, value acceleration, and value linking. Each factor was applied and analyzed step by step. In the analysis including only direct benefit, the net present value and the ratio of benefit to cost were minus 82,920,000 won and 0.8. With this traditional cost-benefit analysis alone, the EPR in hospital can be regarded as very unprofitable. However, when value acceleration was added, the net present value and the ratio of benefit to cost rose to 748,637,000 won and 2.7 respectively. Moreover, when value linking was added, the net present value and the ratio of benefit to cost were 1,473,162,000 won and 4.3, respectively. In the results of this study, as EPR has only direct benefit, EPR can be regarded as unprofitable project. However, when value acceleration and value linking were added, EPR can be profitable project.

6.Carcinosarcoma of the gallbladder: Report of a case.

Carcinosarcoma, often referred to as "malignant mixed tumor" is a rare neoplasm, but this tumor occurs in various organs, including the gallbladder. We report herein a carcinosarcoma of the gallbladder which consisted of adenocarcinomatous and undifferentiated sarcomatous ares, the latter differentiating towards abundant osteoid and calcified but nonlamellated bony trabeculae formation. There are 18 reported cases of this type tumor of the gallbladder in the literature.
7.Ovarian Pregnancy: 12 cases analysis.

From January, 1972 to August 1983, 12 cases of ovarian pregnancy out of 1202 total ectopic pregnancy were studied and the incidence constituted 0.9%. The gross and microscopic findings were reviewed according to gross description and gross photographs, and microscopic description and reviewing all microscopic slides. All cases were shown to meet Spiegelberg's criteria for primary ovarian pregnancy.

9.A Case of Combined Ectopic Gestation with Cervical and Tubal Components.

Ectopic pregnancy is a common medical problem that is difficult to diagnose and potentially may lead to significant mortality or morbidity. The incidence of ectopic pregnancy is definitely increasing due to the rise in pelvic inflammatory disease (PID), pelvic surgery, intrauterine device (IUD), and assisted reproductive technologies, such as in vitro fertilization and embryo transfer (IVF-ET). Combined ectopic gestations are much rare and their true incidence is unknown. Multiple ectopic gestations may occur in a variety of locations. The majority involve one or both fallopian tubes. We report a case of combined tubal and cervical pregnancies, and discuss their management.

10.Multiple Primary Malignant Tumors: 39 cases.

Thirty nine cases of multiple primary malignant tumors examined at the Department of Pathology, Keimyung University School of Medicine during the period from November 1974 through March 1986 were analysed clinicopathologically. The findings are as follows: 1) Thirty nine cases had synchronous lesion and six cases metachronous lesion. The second primary malignancy occurred mean 4.7 years after the first malignancy in patients with metachronous lesion. 2) The age at diagnosis of the first malignancy ranged from 26 to 76 years, the average being 51.8 years. The male-female ratio was slightly less than 1:1.2. 3) The most commonly involved site in multicentric origin was gastrointestinal tract (44.4%) with all adenocarcinoma and the second urogenital system (29.6%). In cases of different tissues or organs (11 cases), ovary was the most frequent site (5 cases), followed by cervix (4 cases). 4) Multiple primary malignant tumors of urogenital system were all eight cases and six cases among them (75%) had smoking and drinking habits. 5) The detailed informations about possible environmental, familial and host factors necessary for the investigation of multiple primary tumors were lacking in most cases. Therefore, the history taking with special attention to these factors must be carefully recorded. And also, we suggest that in the high-risk individuals, careful follow-up and thorough search for early cancer detection or prevention should be taken.
