1.Hypokalemic Familial Periodic Paralysis: A Report of 4 members in a family

Hypokalemic familial periodic paralysis is one of the rare familial disease characterized by recurrent and transient attacks of weakness or paralysis of the somatic musculature. Also, this disease is usually inherited as an autosomal dominant trait in most cases. During an attack, the plasma potassium falls as a rasults of shift of potassium from the extracellular to the intracelluar compartment, but there is no loss of total potassium from the body. We have experienced hypokalemic familial periodic paralysis recently which affected 4 members in a family,and report this disorder.

2.A Study on the Indoor-Outdoor NO2 Levels and Personal Exposures to NO2 with Analysis of Factors Affecting the NO2 Concentrations: Centering on Urban Homes and Housewives.

This study was conducted to establish the control program for preventing unfavorable health effects of nitrogen dioxide(NO2) exposure in homes by preparing the fundamental data for evaluation of relationships between NO2 levels and influencing factors through measurements of indoor-outdoor NO2 levels and personal NO2 exposures for housewives with questionnaire survey on 172 homes in Pusan area from April to June, 1987. NO2 measurements were made by using diffusion tube samplers(Palmes tube NO2 sampler) for one week at 4 sites in homes ; kitchen(KIT), bedroom(BED), living room(LIV), outdoor(OUT) and near the collar housewives(personal exposure livel, PNO). The details of questionnaire were number of household members(FAM), number of regular smokers(SMOKER), daily number of meals eaten(MEAL), type of housing units(HOUSE), location of house with distance from the heavy traffic roads as walking time(DIST), and of kitchen(KAREA), kind of cooking fuels(FUEL), cooking time of each meal(CTIME), usage of kitchen fan for cooking(FAN), type of heating facilities(HEAT) and so on of subject homes. The obtained results were as follows : 1) The mean NO2 level was significantly higher at indoors than outdoors(p<0.01) and the kitchen NO2 level was the highest with 33.7+/-13.6ppb(9.5-81.5ppb). The mean personal exposure level of NO2 for housewives was 20.6+/-8.8ppb(3.1-46.9ppb). 2) The mean indoor NO2 level was significantly higher in the group of household members above 5 than below 4(p<0.05), in detached dwellings than apartments(p<0.001), within 5 minutes of distance than over 5 minutes(p<0.001), in the group of unusing fan(p<0.001), in the group of longer cooking time(p<0.001), and it was in order of coal briquette, gas, electricity and oil by kind of cooking fuels(p<0.05). 3) Variables showing significant correlation(p<0.001) with indoor NO2 level were kitchen NO2 level(r=0.8677), cooking time(r=0.5921), outdoor NO2 exposure level(r=0.4615), usage of kitchen fan(r=0.3573) and location of house(r=-0.2988). 4) As a result of multiple regression analysis, the most significant influencing variable to the kitchen NO2 level was cooking time [KIT=-0.378+/-11.772(CRIME)+0.298(OUT)+3.102(FAN)], it was kitchen NO2 level to the indoor NO2 level [IND = 6.996+0.458 (KIT) + 0.230 (OUT) - 1.127 (KAREA)], and it was indoor NO2 level to the personal NO2 exposure level [PNO=15.562+0.729(IND)-4.542(DIST)-0.200(KIT)]. 5) It was recognized that artificial ventilation in the kitchen, suppression of unnecessary combustion and replacement of cooking fuel, as much as possible, were effective means for decreasing indoor NO2 levels in homes.

3.Intrapelvic Pseudocyst After Total Hip Arthroplasty: A Case Report.

Many uncemented acetabular components use transfixation screws to stabilize the acetabular component until ingrowth of bone occurs. But the use of transacetabular screw fixation in total hip arthroplasty could increase the incidence of intrapelvic complications including neurovascular injury, extrinsic compression and erosion of intrapelvic organs. Metal debris was attributed to corrosion or fretting between screw and metal shell may lead to pelvic osteolysis. A foreign body reaction to metal and polyethylene wear debris in a loose acetahular component eroded the medial wall of the acetabulum and produced an intrapelvic mass. We experienced a case of intrapelvic pseudocyst in a failed total hip arthroplasty. The cyst communicated with hip joint through the transacetabular fixation screw which was placed near the center of the acetahulum.

4.Two Cases of Poland's Syndrome

Polands syndrome is a congenital disorder associated with thoracic anomalies and ipsilateral syndactly. It was described first by Alfred Poland in 1841 year. We has been experienced 2 cases of Polands Syndrome recently. Two cases were male. Each case revealed one side thoracic muscle anomalies, that is, absence of sternocostal head of pectorails major with hypoplasia of nipple and, breast, and ipsilateral syndactyly with partial aplasia and hypoplasia of midphalanges.

5.Large Neurilemmoma of the Lumbar Area (Extradural and Paravertebral): A Case Report

Neurilemmoma is a benign neoplasm arising from the Schwann cell of the nerve sheath. Acoustic nerve is the most common site of neurilemmoma and mandible is the most frequently involved in intraosseous neurilemmoma. Extradurally located dumbbell type neurilemmoma was rarely reported. The histology of neurilemmoma was first described by Verocay in 1908 and elaborated upon by Stout in 1935. Characteristic histologic patterns of neurilemmoma were encapsulation and Verocay bodies, Antoni type A and B areas, palisading nuclei, and lack of malignant characteristics. We experienced such a rare case of neurilemmoma which probably arised from the left first lumbar spinal nerve root, and located extradurally and paravertebrally, and confirmed by radiologic and pathologic findings.

6.Endobronchial Actinomycosis: A report of two cases.

Actinomycosis causes a chronic suppurative infection most commonly involving the cervico-facial region, thorax, and the abdomen. Thoracic infection results from an aspiration of contaminated material from mouth or oropharynx. Actinomyces was known to have a predilection for the periphery of the lung and the endobronchial lesion is extremely rare. We report two cases of actinomycosis presenting as an endobronchial mass which arose in 53-year-old and 44-year-old women. The first case presented with cough, sputum, weight loss and had a history of pulmonary tuberculosis 24 years ago. Chest CT revealed a right lobe collapse simulating mass and suggested carcinoma. The second case presented with hemoptysis for 2 years. Chest CT revealed cystic bronchiectasis of both lungs and intracystic soft tissue mass in the anterior segment of the right upper lobe which suggested aspergilloma. The gross features of them were similar to those of aspergilloma. Characteristic sulphur granules consisting of a granular basophilic center surrounded by a radiating zone of eosinophilic, hyaline, club-shaped projection were histologically confirmed in both cases. Granulomas containing P. westermani eggs were present in the second case. Staphylococcus, true fungal organism, nocardia, and streptomyces shoud be distinguished by analysing their morphologic characteristics in the appropriate stains. Actinomycosis should be included in the differential diagnoses of an endobronchial mass.

7.Study on Awareness of Suicide and Suicide Prevention Among Community Youth

Objectives:
South Korea has the highest suicide rate among Organisation for Economic Co-operation and Development countries; there is an increasing trend in suicide attempts among middle and high school students. Various factors contribute to the risk of suicide among adolescents, and the perception of suicide prevention has emerged as a significant factor. This study aimed to investigate the association between emotional and behavioral difficulties among middle and high school students and their perceptions of suicide prevention and to explore differences in suicide perception according to age.
Methods:
A survey was conducted among community middle and high school students, including 530 participants, between 2020 and 2021. Emotional and behavioral difficulties were assessed using the Strengths and Difficulties Questionnaire-Korean version, and participants were asked to complete a questionnaire on the importance and possibility of suicide prevention. A correlation test and analysis of variance were used to examine the relationships between the variables, and suicide awareness was compared according to age.
Results:
The participants who displayed higher strength or lower difficulty were more likely to respond positively to suicide prevention measures. They also exhibited high strength and low difficulty levels, thus agreeing with the importance of suicide prevention. Regarding age-related perceptions of suicide, adults aged 20–29 years reported the lowest probability of suicide prevention.
Conclusion
Suicide perceptions influence the incidence of suicide. Therefore, active societal engagement through suicide prevention campaigns and related education is essential to improve such perceptions. Continuous attention and support are required to address this issue.
8.Effects of Mianserin on Negative Symptoms of Chronic Schizophrenia.

OBJECTIVES: Patient's behavior features are important factors which influences the clinical judgement including diagnosis. However, most psychiatrists build up a picture of patients' behavior from an amalgamation of their own brief observations and nurses' reports, which often lack in the objectiveness. Several behavioral scales have been developed to alleviate this difficulty, but the poor efficiency and reliability of these scales have made them less useful. The recently developed Ward Daily Behavior Scale is an objective tool for evaluating all the daily noteworthy behaviors of patients, and is easily applicable to wide ranges of diagnoses and ages. This study tried to prove the reliability and validity of the Ward Daily Behavior Scale-Korean version. METHODS: The 112 patients, 63 males and 49 females, at a chronic psychiatric inpatient ward were selected as subjects. Experienced and unexperienced nurses rated patients' behaviors independently with the Ward Daily Behavior Scale-Korean version, after observing behaviors of subjects for 8 hours during day duty time. And then we tested the inter-rater reliability, internal consistency, and concurrent validity of this scale. RESULTS: The Ward Daily Behavior Scale-Korean version proved to be both reliable and valid for measuring of behaviors of psychiatric inpatients. CONCLUSIONS: The Ward Daily Behavior Scale-Korean version will be a valuable tool to observe and quantify patients' behavior in psychiatric wards.

9.Antibiotic-Associated Diarrhea in 3 to 6 Month Old Infants with Febrile Urinary Tract Infections.

PURPOSE: This study attempted to investigate the frequency, duration, and risk factors of antibiotic-associated diarrhea (AAD) in infants hospitalized due to febrile urinary tract infection (UTI). This is a basic research on the probiotics used in the prevention and treatment of AAD in infants. METHODS: Medical records of the infants aged 3-6 months hospitalized in Gachon University Gil Hospital from January 2008 to September 2010 due to the febrile UTI were retrospectively reviewed. The episodes of loose or watery stool were investigated for frequency, onset, and duration. Those who had AAD and those who did not (non-AAD) were compared. The antibiotic regimens and the episodes of diarrhea were investigated in AAD group. RESULTS: Total 147 infants were included. Fifty-four (36.7%) showed AAD. Intravenous third-generation cephalosporin (3rd CS) single therapy was used for 102 patients (69.4%), the 3rd CS and non-3rd CS combination therapy for 24 (16.3%), and non-3rd CS combination therapy for 21 (14.3%). There was no significant difference in the dose of cefotaxime between AAD and non-AAD group (P=0.601). According to the antibiotic therapies above, in AAD group, there was no significant difference in the onset and duration of diarrhea respectively (P=0.717, P=0.830). Although the frequency of diarrhea was higher for the 3rd CS and non-3rd CS combination therapy subgroup with 9.25+/-5.30 times/day than the other two subgroups (7.58+/-2.97 times/day in 3rd CS single therapy subgroup, 6.75+/-4.40 times/day in non-3rd CS combination therapy subgroup), there was no statistical significance (P=0.078). CONCLUSION: AAD seems common to the infants aged 3-6 months with febrile UTI, regardless of regimen and amount of antibiotics in usual dosage. Further research on the effects of probiotics used in the prevention and treatment of AAD in infants is warranted.
