1.The Prognosis of Preganglionic Brachial Plexus Injuries by Electrodiagnosis.

OBJECTIVE: To document the neurological recovery of the preganglionic lesion of brachial plexus, both clinically and electrodiagnostically. METHOD: Sixteen male and three female patients were evaluated. Ages of the patients ranged from 1 to 45 years with the mean age of 25.2+/-14.3 years. We studied retrospectively the improvements of 43 injured nerves and its innervated muscles of 19 patients retrospectively according to the complete or incomplete nature of the injuries by the electromyographic findings. We compred the results of initial and final examinations by the electromyographic and clinical findings including a muscle strength test. RESULTS: In the complete injuries, 84.0% of the cases showed a minimal or no improvement in muscle strength whereas in the incomplete injuries, 88.2% of the cases showed a greater than moderate improvement in muscle strength. The cases in which no compound muscle action potential could be evoked on the initial study, showed the improvements of greater than 3/5 muscle strength only in 20.0%. However 71.5% of cases with the initial amplitude between 0.1 & 0.5 mV and 100.0% of cases with the initial amplitude greater than 0.5 mV showed the muscle strength greater than 3/5. CONCLUSION: The prognosis for the neurological recovery was significantly poor in the complete preganglionic lesions of brachial plexus injuries but in the incomplete lesions, the prognosis was extremely good. It would be crucial for the initial CMAP being greater than 0.5 mV to expect the recovery of muscle strength to the functional level. Rehabilitative or operative management should be instituted based on the electrodiagnostic findings.

2.Comparison of Filtered Back Projection, Hybrid Iterative Reconstruction, Model-Based Iterative Reconstruction, and Virtual Monoenergetic Reconstruction Images at Both Low- and Standard-Dose Settings in Measurement of Emphysema Volume and Airway Wall Thick.

OBJECTIVE: To evaluate the accuracy of emphysema volume (EV) and airway measurements (AMs) produced by various iterative reconstruction (IR) algorithms and virtual monoenergetic images (VME) at both low- and standard-dose settings. MATERIALS AND METHODS: Computed tomography (CT) images were obtained on phantom at both low- (30 mAs at 120 kVp) and standard-doses (100 mAs at 120 kVp). Each CT scan was reconstructed using filtered back projection, hybrid IR (iDose4; Philips Healthcare), model-based IR (IMR-R1, IMR-ST1, IMR-SP1; Philips Healthcare), and VME at 70 keV (VME70). The EV of each air column and wall area percentage (WA%) of each airway tube were measured in all algorithms. Absolute percentage measurement errors of EV (APEvol) and AM (APEWA%) were then calculated. RESULTS: Emphysema volume was most accurately measured in IMR-R1 (APEvol in low-dose, 0.053 ± 0.002; APEvol in standard-dose, 0.047 ± 0.003; all p < 0.001) and AM was the most accurate in IMR-SP1 on both low- and standard-doses CT (APEWA% in low-dose, 0.067 ± 0.002; APEWA% in standard-dose, 0.06 ± 0.003; all p < 0.001). There were no significant differences in the APEvol of IMR-R1 between low- and standard-doses (all p > 0.05). VME70 showed a significantly higher APEvol than iDose4, IMR-R1, and IMR-ST1 (all p < 0.004). VME70 also showed a significantly higher APEWA% compared with the other algorithms (all p < 0.001). CONCLUSION: IMR was the most accurate technique for measurement of both EV and airway wall thickness. However, VME70 did not show a significantly better accuracy compared with other algorithms.

Gossypibomas, retained surgical sponges are prone to creat adhesions and to encapsulate, or to provoke an exudative response, with or without accompanying bacterial infection. Often a process of transmural migration can occur and we experienced a case which was diagnosed by colonoscopy. A 24 year-old female patient who had a history of Cesarean section 4 months ago came to the hospital with a complaint of intermittent right lower quadrant pain. A thread of surgical sponge was found by colonoscopy from ascending colon to cecum. At laparotomy, a transmurally migrating surgical sponge through the colonic fistula at the cecal base with surrounding abscess cavity was found. Adding a case report, the fate of intraperitoneal forgotten surgical sponges is reviewed.

Pancreas divisum is the most common congenital anomaly of the pancreas. Whether pancreas divisum is the cause of chronic abdominal pain, acute pancreatitis, or chronic pancreatitis is a matter of controversy yet. We recently experienced a case of acute pancreatitis due to an impacted stone at the orifice of the minor papilla in a 58 year-old man with pancreas divisum who showed clear radiologic evidences of dorsal chronic pancreatitis. The bout of acute pancreatitis recovered rapidly after endoscopic treatment (minor papilla sphincterotomy, stone removal, and endoscopic nasopancreatic drainage). We herein report an unusual manifestation of pancreas divisum in which endoscopic treatment was effective.

7.Long-term Clinical Outcomes and Prognostic Factors After Endovascular Treatment in Patients With Chronic Limb Threatening Ischemia

Background and Objectives:
Endovascular therapy (EVT) first strategy has been widely adopted for the treatment of chronic limb threatening ischemia (CLTI) patients in realworld practice. This study aimed to investigate long-term outcomes of CLTI patients who underwent EVT and identify prognostic factors.
Methods:
From the retrospective cohorts of a Korean multicenter endovascular therapy registry, 1,036 patients with CLTI (792 men, 68.8 ± 9.5 years) were included. The primary endpoint was amputation-free survival (AFS) defined as the absence of major amputation or death. Secondary endpoints were major adverse limb events (MALE; a composite of major amputation, minor amputation, and reintervention).
Results:
Five-year AFS and freedom from MALE were 69.8% and 61%, respectively. After multivariate analysis, age (hazard ratio [HR], 1.476; p<0.001), end-stage renal disease (ESRD; HR, 2.340; p<0.001), Rutherford category (RC) 6 (HR, 1.456; p=0.036), and suboptimal EVT (HR, 1.798; p=0.005) were identified as predictors of major amputation or death, whereas smoking (HR, 0.594; p=0.007) was protective. Low body mass index (HR, 1.505; p=0.046), ESRD (HR, 1.648; p=0.001), femoropopliteal lesion (HR, 1.877; p=0.004), RC-6 (HR, 1.471;p=0.008), and suboptimal EVT (HR, 1.847; p=0.001) were predictors of MALE. The highest hazard rates were observed during the first 6 months for both major amputation or death and MALE. After that, the hazard rate decreased and rose again after 3–4 years.
Conclusions
In CLTI patients, long-term outcomes of EVT were acceptable. ESRD, RC-6, and suboptimal EVT were common predictors for poor clinical outcomes.

BACKGROUND: This study compared the bronchodilator efficacy and safety of tiotropium inhalation capsules (18microgram once daily) with a ipratropium metered dose inhaler (2 puffs of 20microgram q.i.d.) in patients with chronic obstructive pulmonary disease (COPD). METHOD: After the initial screening assessment and a two-week run-in period, patients received either tiotropium 18microgram once daily or ipratropium 40microgram four times daily over a period of 4 weeks in a double blind, double dummy, parallel group study. The outcome measures were the lung function, the daily records of the peak expiratory flow rate (PEFR), the patients' questionnaire, and the use of concomitant salbutamol. The forced expiratory volume in one second (FEV1) and the forced vital capacity (FVC) were measured 5 minutes before inhalation, and 0.5, 1, 2 and 3 hours after inhaling the study drug on days 0, 14 and 28. RESULT: In 16 centers, 134 patients with a mean (SD) age of 66 (7) years and a predicted FEV1 of 42 (12)% were analyzed. The trough FEV1 response was significantly higher in the tiotropium group than in the ipratropium group after a four-week treatment period. The weekly mean morning PEFR of the tiotropium group was consistently higher than that of the ipratropium group during the 4-week treatment period with differences ranging from 12.52 to 13.88 l/min, which were statistically significant. Tiotropium was well tolerated by the COPD patients during the 4-week treatment period and had a similar safety profile to ipratropium. CONCLUSION: This study shows that tiotropium administrated once daily has a superior bronchodilator effect with a similar safety profile in treating COPD patients compared with ipratropium, inhaled four times daily.
