2.Clinical observation of inflammation and malnutrition in patients with stage V chronic kidney disease

Objective To explore the variation of the plasma high-sensitive C reactive protein (hs-CRP) levels and analyze the relationship between inflammation and malnutrition in the patients with stage V chronic kidney disease (CKD). Methods Ninety patients with stage V CKD were enrolled. The patients were divided into three groups according to their treatment, no dialysis (ND) group, hemodialysis (MHD) group and continuous ambulatory peritoneal dialysis (CAPD) group. Plasma hs-CRP, albumin and hemoglobin were detected. Results The levels of plasma hs-CRP were significantly increased in the patients with stage V CKD compared with normal individuals (P< 0.01). It showed a significant difference between MHD group, CAPD group and ND group (P < 0.05), while no significant difference was found between MHD group and CAPD group. The plasma hs-CRP levels increased in the patients with lower plasma albumin (< 35 g/L) or lower hemoglobin (< 90 g/L). Levels of plasma hs-CRP showed a negative relationship with plasma albumin and hemoglobin (r =- 0.535, P < 0.01 ;r =-0.220, P < 0.05). Conclusions Plasma hs-CRP levels increase in patients with stage V CKD, and there is a negative relationship between hs-CRP and plasma albumin and hemoglobin. Inflammation may be a factor of malnutrition in patients with CKD.
3.Preparation and biological characteristics of MTX and VCR co-encapsulated erythrocytes

Objective To explore the method and feasibility of preparing MTX+VCR-loaded erythrocytes using healthy adult erythrocytes. Methods We used the modified hypotonic pre-swelling and isotonic resealing technique to prepare MTX+VCR-loaded erythrocytes. The loading parameters and releasing rate were evaluated with high penformance liquid chromatograph (HPLC) and auto-cytoanalysis. We observed the osmotic fragility of MTX+VCR-loaded erythrocytes conventionally. The change in innate immune activity of co-encapsulated erythrocytes was measured by counting the rosette rate of MTX+VCR-loaded erythrocytes adhering to S180. Results The concentration of MTX showed significant influence on the co-encapsulation efficiency (P0.05). Conclusions The preparation and biological characteristics of MTX+VCR-loaded erythrocytes were not different from those of mono-loaded erythrocytes, and erythrocytes were practically capable to be used as carriers for co-encapsulation of MTX and VCR.
4.Reflection and investigation on the current situation of remote fetal monitoring system in primary hospital

Objective To study the feasibility and care mode of remote fetal monitoring at primary hospital. Methods A questionnaire survey was administered to 71 hospitals of different levels in Jiangsu province.Question-naire content included human resources status of midwifery,the remote fetal monitoring system and its clinical applica-tion;anonymous questionnaire survey was conducted to 130 maternal women by random cluster sampling;investigation of remote fetal monitoring device type,operation mode,clinical usage and existing problems was done through the doc-ument retrieval of VIP and Wanfang data platform.Results In grade Ⅱhospital,obstetricians sufficient and insuffi-cient were 10 cases (24.39%),31 cases(75.6%),respectively,and in grade Ⅲ hospital were 8 cases (61.53%), 5 cases (38.46%),respectively,there was statistically significant difference between grade Ⅱ and grade Ⅲ hospital (χ2 =4.572,P <0.05);In grade Ⅱ and grade Ⅲ hospital midwives were obviously insufficient,but there was no sta-tistically significant difference between grade Ⅱ and grade Ⅲ hospital(χ2 =0.219,P >0.05).Different cultural lev-el,place of residence,occupation of maternal demand for the development of remote fetal monitoring were urgent and higher degree of acceptance,but the cognition was not comprehensive,college degree or above,technical personnel and urban pregnant women that their need for remote fetal monitoring were the most urgent.There were significant differ-ences(all P <0.01).The use of remote fetal monitoring equipment was mostly at tertiary hospital nationwide.3G net-work way to transfer data had short time and small scope.Conclusion The existing monitoring system and service mode can not meet the requirements of the implementation of remote fetal monitoring,medical personnel professional quality and maternal comprehensive quality are the main influencing factors.The primary hospitals should actively pro-mote the application of remote fetal monitoring,maternal health care services and health education should arouse the attention of the whole society.

6.Hepatic Effect of Simultaneous Exposure to Nickel and Cadmium in Rats.

OBJECTIVES: This research was intended to verify hepatic effect of simutaneously combined exposure of nickel chloride and cadmium chloride with IPRL(Isolated Perfused Rat Liver) method. METHODS: AST(aspartate aminotransferase), ALT(alanine aminotransferase), LDH(lactate dehydrogenase) and perfusion flow rate were used as the indicator of hepatotoxicity and oxygen consumption rate were used as the indicator of viability. 300(+/-50) g weighted rats were allocated randomly to each group (control group, 50 microM 200 microM separately exposure group of NiC1, and CdC1, simultaneously combined exposure group of NiC1 and CdC1) by 5, totally 35. Buffer which got out of liver was sampled and then biochemical indicator of hepatotoxicity was measured. In order to verify difference among groups, two way repeated ANOVA was used. With comparing mean summation of separate exposure group with mean of simultaneous exposure group, possibility that combined effect could be synergistic effect was verified. RESULTS: AST, ALT, LDH increased in both of separate exposure group and simultaneous exposure group. Perfusion flow rate and oxygen consumption rate decreased but statistically significant difference among groups was not found, In each exposure group, AST, ALT, LDH, oxygen consumption rate of simultaneous exposure group increased more than sum of separate exposure group after any sampling time. CONCLUSIONS: It was found that simultaneous combined effect could be synergistic effect through the biochemical indicator of hepatotoxicity.

7.Influence of Gleevec on expression of miR-146a,miR-29b and DNMT in K562 cells

Objective To investigate the changes of microRNA (miR)-146a ,miR-29b expression levels and the 3 kinds of meth-ylase DNMT1 ,DNMT3a and DNMT3b levels in K562 cell lines after BCR/ABL inhibitor Gleevec treatment .Methods The half maximal inhibitory concentration(IC50 ) of Gleevec on K562 cells was detected by the MTT method .The stem loop primers method and the fluorogenic quantitative PCR were adopted to detect miRNAs and the methylase gene level .Results IC50 of Gleevec acting on K562 cells was 40 .85μmol/L .After Gleevec action ,miR-29b showed the increasing trend ,but 3 kinds of methylase expression level were decreased to some extent .Gleevec could significantly increase the miR-146a level in K562 cells(P<0 .05) .Conclusion Gleevec can influence the expression of miR-146a ,miR-29b and DNMTs levels in K562 cells .
8.Effect of Rehabilitation Nursing on Activities of Daily Living of Patients with Stroke in Midway-training Course

Objective To observe the effect of rehabilitation nursing on activities of daily living (ADL) of patients with stroke in Midway-training Course. Methods 51 stroke patients were divided into control group (n=26) and supervision group (n=25). The control group received routine rehabilitation service, and the supervision group received rehabilitation nursing supervisor in addition. They were assessed with Barthel Index (BI) before and after the course. Results The BI increased after course in both groups (P<0.05), and increased in the supervision group (P<0.05) after 30 d, but not in the control group (P>0.05). Conclusion Rehabilitation nursing during Midway- training Course can improve recovery of the ADL of patients with stroke.
9.A Case of Lymphocytic Infiltration of the Skin ( Jessner ).

Lymphocytic infiltration of the skin(LIS), first described in 1953 by Jessner and Kanot is relatively uncommon disorder, characterized clinicaily by the presence of erythemat.ous peripherally enlarging papules or plaques which may show central clearing. Histologically, the lesions show patch, perivascular and periadnexal lymphocytic infiltrates. We have experienced a case of 30-year old male who had multiple erythematous nodules and plaques on the face and dorsum of the right wrist with 5 years history. He had first noted erythematous plaques on the cheek and these lesions were gradually increasing in size and spreading to the forehead and forearm. He had transiently improved by medication at the drug store, but recurred after discontinuation.

10.An Immunolhistochemical Study of Extramammary Paget's Disease: Adenokeration , Carcinoembryonic Antigen and Involucrin Expression in Extramammary Paget's Disease.

The origin of Paget cells in extramammary Pagets disease is uncertain, however, some ultrastructural surveys revealed differentistion of Paget cells toward sweat gland apparatus. This is supported by recent immunohistochemical studies that Paget cells are irnmunohistochemically shown to react with antikeratin monoclonal antibodies specific to simple epithelia. In this siudy, seven cases of extramammary Pagets disease were immunohistochemically investigated with adenokeratin, carcinoembryonic antigen(CEA), involucrin by peroxidase-antiperoxidase(PAP) techniques. The results were as follow : l. Epidermis, hair follicies and sebaceous glands revealed negative immunoreacativity for adenokeratin in all cases. Sweat glands(6/7), Paget cells(6/7) and underlying adenocarcinoma revealed positive immunoreactivity for adenokeratin. 2. Epidermis, hair follicles and sebaceous glands(6/7) revealed negative immunoreactivity for CEA. Sweat glands, Paget cells and underlying adenocarcinoma revealed positive immunoreactivity for CEA in all cases. 3. Lower epidermis, sweat glands(6/7), Paget cells and underlying adenocarcinorna revealed negative irnmunoreactivity for involucrin. Upper epidermis, hair follicles and sebaceous glands(5/7) revealed positive immunoreactivity far involucrin. Based on these results, Paget cells of extramammary Pagets disease are considered to be derived from the sweat glands.
