Isolated situs in versus of the stomach with otherwise normal position of the thoracic and abdominal vescera isan extremely rare anomaly occurring in two distinct forms. Majority of cases are associated with eventration of the diaphragm and are reported as being confused with spontaneous pneumothorax of pyopneumothorax at base of the right lung. The right sided stomach may produce interesting and confusing changes in liver scan. We have experienced 2 cases of the isolated dextrogastria.

4.Effects of Early Dexamethasone Therapy in Neonatal Respiratory Distress Syndrome.

PURPOSE: To examine the clinical effects of early dexamethasone treatment from postnatal age of 1 week to ventilated preterm infants with respiratory distress syndrome. METHODS: 14 one-week-old ventilator-dependent infants were enrolled. Seven received dexamethasone and another 7 did not receive. The doses of dexamethasone used were 0.25mg/kg twice daily for 3 days, 0.125mg/kg twice for 1 day, then 0.125mg/kg once for 1 day. Blood gases were measured and ventilatory efficiency index (VEI) and arterial/alveolar oxygen tension ratio (a/A PO2) were calculated every 12 hours from 48hours before to 72 hours after administration. Mean arterial blood pressures were noted and urine output was measured every 24 hours during the study. RESULTS: Following dexamethasone, the values of PaO2 improved significantly during 72 hours of treatment (P<0.05) and the values of PaCO2 tended to decrease during the study but those were not significant changes. VEI and a/A PO2 ratio improved significantly for 48 hours following dexamethasone (P<0.05). After receiving dexamethasone, the urine output increased significantly during 72 hours of treatment (P<0.05). The average extubation time in dexamethasone-treated group was significantly earlier than that seen in control group (P<0.05). Also the average duration of oxygen therapy and mechanical ventilation of dexamethasone-treated infants were significantly shorter compared with those of control infants. CONCLUSION: I conclude that early dexamethasone therapy from postnatal age of 1 week has beneficial effects in ventilated preterm infants with respiratory distress syndrome.

6.A Study on the Systolic Time Intervals in Korean Hypertensive Patients.

The aim of this study was to quantitatively estimate the effect of hypertension upon the left ventricle, using the systolic time intervals. The subjects for this study consist of 72 hospitalized hypertensive patients including 38 males and 34 females. The measurements of the systolic time intervals were obtained from simultaneous high speed recording(100mm/sec) of an electrocardiographic lead best displaying the onset of left ventricular depolarization, a carotid pulse tracing, and a phonocardiogram best displaying the initial high frequency vibrations of the aortic valve closure sound. All data were corrected for heart rate and sex using the regression equations of Weissleretal. The results were follows: 1) As the diastolic blood pressure increased, shortening of left ventricular ejection time index and prolongation of preejection period index and PEP/LVET ratio were significant. 2) As the electrocardiographic findings related to hypertension became severe shortening of left ventricular ejection time index and prolongation of preejection period index and PEP/LVET ratio were significant. 3) As the hypertensive retinopathy became severe, shortening of left ventricular ejection time index and prolongation of preejection period index and PEP/LVET ratio were significant. It was suggested that the measurement of the systolic time intervals are useful in assessing the effects of hypertension upon the left ventricular function and in detecting early recognition of cardiac dysfunction in hypertension, even though not necessarily associated with overt heart failure.

8.Comparison between Dothiepin-Sertraline Combination and Dothiepin Alone Therapy in the Treatment of Depressive Disorder.

The dysfunction of either or both noradrenaline and serotonin system are important in the pathophysiology of depression. Previous reports have suggested that there may be an important interaction between these two systems. Recently, some investigators have suggested that the combination of tricyclic antidepressants(TCAs and selective serotonin reuptake inhibitors(SSRIs would produce a rapid synergistic effect on down-regulation of either or both of these two systems and that this combination may produce a more rapid and absolute antidepressant effect. We compared the treatment efficacy, treatment associated side effects, treatment satisfaction, and the quality of life between the combination therapy of dothiepin-sertraline as well as the therapy of dothiepin alone in the treatment of major depressive disorder and dysthymic disorder. In our study, the combination therapy of dothiepin and sertraline produced a more rapid and absolute antidepressant effect than dothiepin alone. And the patients with combination therapy experienced relatively high treatment satisfaction than the patients with dothiepin therapy. The patients quality of life improved more rapidly in the combination therapy, especially, in the health perception, social behavior, and life satisfaction, that dothiepin alone. These results support the hypothesis that the combination of TCA an SSRI may produce a rapid synergistic effect on either or both norepinephrine and serotonin system, and more rapid antidepressant effect and high treatment satisfaction.

9.Accessory Hepatic Vein: MR Imaging.

PURPOSE: To evaluate the MR appearance of the accessory hepatic veins. MATERIALS AND METHODS: The study included 87 consecutive patients for whom abdominal MR images were obtained. The subjects who had liver lesion or hepatic vascular abnormalities were excluded. Couinaud classified accessory hepatic veins into inferior and middle right hepatic veins. Our major interests were evaluation of the incidence, morphology, and location of the accessory hepatic vein. RESULTS: Inferior right hepatic vein was demonstrated in 43 out of 87 patients(49%). The morphology was linear in 35 patients(80.5%), and V-shaped in 8 patients(19.5%). In 40 patients(93%), the inferior right hepatic vein was located in the posteroinferior aspect of the right lobe. Middle right hepatic vein was demonstrated in 7 out of 87 patients(8%). All were single linear in morphology, combined with the inferior right hepatic vein, and located between the right hepatic vein and inferior right hepatic vein. CONCLUSION: The accessory hepatic vein was demonstrated in 49% among the Korean adult population, and was located in posteroinferior portion of the liver, in 93%.

10.Impact of Intolerance of Uncertainty on Brain Structural Changes in Panic Disorder

Objective:
This study investigated the impact of intolerance of uncertainty (IU) on structural changes in the brain and symptom severity in patients with panic disorder.
Methods:
This study included 90 participants diagnosed with panic disorder. The IU Scale, Panic Disorder Severity Scale (PDSS), Beck Depression Inventory-II (BDI-II), Penn State Worry Questionnaire (PSWQ), Self-Forgiveness Scale (SFS), and Short Form 36 Health Survey (SF) were used. A voxel-wise correlation analysis was conducted to investigate the structural differences in the gray matter.
Results:
As IU increased, the cortical thickness of the right lingual gyrus decreased significantly, while the gray matter volume of the right pars triangularis increased. The cortical thickness of the right lingual gyrus showed a significant negative correlation with the BDI-II score and a positive correlation with the SFS. Additionally, the gray matter volume of the right pars triangularis was positively correlated with the PDSS, PSWQ, and BDI-II scores and negatively correlated with the mental health domain of the SF.
Conclusion
According to our findings, elevated IU in participants with panic disorder was associated with cortical thinning in the lingual gyrus and increased gray matter volume in the pars triangularis. These structural alterations may also have an impact on perceived quality of life, as well as high levels of depression and anxiety.
