1.Studies on the Antivacterial Effect of Cephalosporin Derivatives.

The minimal inhibitory concentration(MIC) of cephalosporin derivatives, (cephaloridine, cefazolin sodium, cephradine, cephapirin aodium, cephacetrile sodium, and cephalexin from various Korean drug companies) for Staphykcoccus aureus ATCC 25923 and Escherichia coli ATCC 25922 was measured by the tube dilution method, using Mueller-Hinton broth. The results obtained were as follows: 1) The MIC of each individual cephalosporin derivative, although produced by different manufacturers, was similiar or equal. 2) Against Staphyloeoccus aureus, cephaloridine and cephapirin had the lowest MIC while cephradine and, cephalexin the highest. Against E. coli, cephaloridine and cefazolin had the lowest MIC while cephradine and cephalexin the highest. 3) The MIC against Staphylococcus aureus was l5.6 to 250 times lower than that against E. coli in all cephalosporin derivatives. 4) The MIC against Staphylococcus aureus ranges from 0.0125 to l.0ug/ml and against E. coli from 1. 56 to 25ug/ml in all cephalasporin derivatives.

2.Pathogens of Corneal Ulcer (II).

One hundred and thirty-two cases(132 eyes) of corneal ulcer were analysed on the clinical and etiologic basis. The history of evident ocular trauma was found in 28.8% of the cases and ocular disease was found in 7.6% of the cases. Positive results were 16/55 in smear and 33/61 in culture, and the most common isolate was coagulase negative staphylococcus in bacteriologic studies. In fungal studies, positive results were 1/42 in smear and 5/42 in culture, and Aspergillus was the most common isolate. Cephalexin was the most sensitive antibiotics for Gram positive organism and carbenicillin and gentamicin were the most sensitive antibiotics for Gram negative orgamsm. These results indicate that gentamicin and cephalexin in combination would be recommended as the initial treatment of wide-spectrum antibiotics coverage for bacterial corneal ulcer until the final culture results and senitivity are available.

3.A Case of Herpes Zoster with Generalized Varicella like Eruption.

Generalized varicella-like eruption occurs in 2 to 10 percent of unselected patients with localized zoster, the majority of whom are patients with immunologic defects due to underlying malignanies (particularly rnalignancy of hematopoietic system) or immunosuppressive therapy. A 25-year old male noted onset of pain and burning sensation on the right side of cheet nine days prior to admission. The following day multiple vesicles were noted on right side of the chest. Three day before the admisson erythernatous maculopapules were noted on the abdomen. One day prior to admission vesicular lesions appeared over the entire hody. The patient was treated with topical application of calamin lotion, and with kanamycin and Cephalexin to prevent secondary infection. The secondary vesicles had disappeared by the fifth hospital day, but the initial lesion had not improved. On the fifteenth hosptital day the initia1 lesions has disappeared.

4.A case of bronchial asthma due to cefadroxil in a housewife.

Cephalosporins are the most important -lactams that induce IgE-mediated reactions. Also, cephalosporins have been known as a causative agent for occupational asthma in pharmaceutical workers. To our knowledge, this is the first report of cephalosporin-induced bronchial asthma in a housewife with no history of occupational exposure. We experienced a 30-year old female who had developed shortness of breath, coughing and itching sensation of the skin since 3 years ago whenever she handled drug powder for upper respiratory infections (URI) prescribed for her two sons with bronchial asthma. She had handled drug powder for 7 years because her sons had experienced frequent URI. Skin prick test with cefadroxil (10mg/ml) and cefaclor (10mg/ml) showed positive reactions. Bronchial challenge test with cefadroxil showed immediate asthmatic reaction, and bronchial challenge with cefaclor showed immediate urticaria and angioedema without significant fall in FEV1. We confirmed cefadroxil-induced bronchial asthma sensitized by intermittent inhalation in a non-occupational setting.

5.A Case of Pseudomonas fluorescens Infection after Dacryocystorhinostomy and Silicone Tube Intubation.

PURPOSE: To report a case of Pseudomonas fluorescens infection following endoscopic dacryocystorhinostomy and silicone tube intubation in a healthy patient who was using steroid nasal spray. In addition, a literature review is conducted. CASE SUMMARY: A 72-year-old female patient came to our clinic with tearing and hyperemia in the right eye. Ten months prior, she had undergone endoscopic dacryocystorhinostomy and silicone tube intubation due to nasolacrimal duct obstruction in the right eye. Six months after the first operation, dacryocystorhinostomy revision with silicone tube exchange was performed due to obstruction of the nasal bony orifice. In addition, the patient was using a steroid nasal spray. On slit lamp examination, conjunctival injection, marked inflammation and punctal edema around the tube were observed. The silicone tube was removed and the tube cultured. Pseudomonas fluorescens was isolated from the tube contents. The patients was treated with topical 0.3% gatifloxacin 4 times a day, methylol cephalexin lysinate 1000 mg 3 times a day and the nasal spray was discontinued. Two weeks later, all symptoms were resolved after treatment with antibiotic treatment. CONCLUSIONS: A case of Pseudomonas fluorescens canaliculitis which occurred in healthy patient who was using steroid nasal spray is presented with a literature review. Pseudomonas fluorescens canaliculitis can be treated by using proper antibiotics.

6.Lower Levels of Urinary Nerve Growth Factor Might Predict Recurrent Urinary Tract Infections in Women.

PURPOSE: To investigate the changes in urinary nerve growth factor (uNGF) levels after acute urinary tract infection (UTI) and to assess the role of uNGF in predicting UTI recurrence in women. METHODS: Women with uncomplicated, symptomatic UTIs were enrolled. Cephalexin 500 mg (every 6 hours) was administered for 7-14 days to treat acute UTIs. Subsequently, the patients were randomized to receive either sulfamethoxazole/trimethoprim 800 mg/160 mg daily at bedtime, or celecoxib 200 mg daily for 3 months and were monitored for up to 12 months. NGF levels in the urine were determined at baseline, 1, 4, and 12 weeks after the initiation of prophylactic therapy, and were compared between women with first-time UTIs and recurrent UTIs, sulfamethoxazole/trimethoprim and celecoxib-treated women, and no UTI recurrence and UTI recurrence that occurred during the follow-up period. Twenty women free of UTIs served as controls. RESULTS: A total of 139 women with UTI and 20 controls were enrolled in the study, which included 50 women with a first-time UTI and 89 women with recurrent UTIs. Thirty-seven women completed the study. Women with recurrent UTIs (n=23) had a trend of lower uNGF levels than women with first-time UTIs (n=14). During follow-up, 9 women had UTI recurrence. The serial uNGF levels in women with UTI recurrence were significantly lower than those in women who did not have UTI recurrence during the follow-up period. CONCLUSIONS: The lower levels of uNGF in women with recurrent UTI and the incidence of UTI recurrence during follow-up suggest that lower uNGF might reflect the defective innate immunity in women with recurrent UTI.

7.In Vitro Activities of Cefatrizine-Clavulanic Acid against Gram-Negative Bacilli Isolated from Community-acquired Urinary Track Infection.

BACKGROUND: A high proportion of currently isolated gram-negative bacilli are resistant to beta-lactams by producing beta-lactamases. beta-lactam and beta-lactamase inhibitor combinations have been successfully used to overcome the resistance. In this study, in vitro antimicrobial activity of a new combination, cefatrizine-clavulanic acid, was determined against gram-negative bacilli isolated from community-acquired urinary track infections. METHODS: Nonduplicate strains of Enterobacteriaceae, isolated in 2003 from urine specimens of outpatients and inpatients of less than 3 hospital days at Severance Hospital, were tested by the NCCLS agar dilution method. RESULTS: Of a total of 204 isolates, 144 (71%) were Escherichia coli and 30 (15%) were Klebsiella spp. MIC50 and MIC90 of cefatrizine for E. coli were 2 microgram/mL and 16 microgram/mL, respectively. MIC90s of both cefaclor and cefoxitin were also 16 g/mL. MIC50 and MIC90 of cefatrizine-clavulanic acid for E. coli were 1 microgram/mL and 4 microgram/mL, respectively, which were 1/2-1/4 of those of cefaclor and cefoxitin. For Klebsiella spp., MIC90 of cefatrizine was 4 microgram/mL with an MIC range of 1->128 microgram/mL, whereas that of cefatrizine-clavulanic acid was 2 microgram/mL with an MIC range of 0.5-32 microgram/mL. In vitro activity of cefatrizine-clavulanic acid was higher than that of cefatrizine. CONCLUSIONS: Improved in vitro activity of cefatrizine-clavulanic acid against isolates of E. coli and Klebsiella spp. from community-acquired urinary track infection suggested that the combination is useful for an empirical treatment of the infection.

8.Study on a selection of domestic standard for distributing delta3 - cefaclor to quantify and quality the cefaclor by high performance liquid chromatography

System suitability tests are an integral part of liquid chromatographic technique. They are used to verify the resolution (R) and reproducibility of the analysis to be done. HPLC method for assaying the related substances test of Cefaclor in raw material as well as in pharmaceutical dosage forms introduced to some pharmacopoeia such as EP, USP need to use Delta- 3 cefaclor as the internal standard to verify the suitability of chromatographic system. In case of lacking it to use, Cephalexin can be used as the internal standard in place of Delta-3 cefaclor.
