1.INVOLVEMENT OF PERIPHERAL NMDA RECEPTORS IN PERSISTENT NOCICEPTION INDUCED BY SUBCUTANEOUS BEE VENOM INJECTION: A BEHAVIORAL STUDY

The present study was to investigate whether peripheral NMDA receptors were involved in the persistent nociceptioninduced by subcutaneous (s. C. ) bee venom injection in the conscious rat by using quatitative pain scoring methods, a.c. Bee venom injection into one hindpaw resulted in a persistent, monophasic nociceptive response characterized by continuously flinching.lifting and licking the injected paw for more than I h. The non-competitive. NMDA receptor channel blockers, ketamine and ME-801, were administered s.c. 5 or 20 min after bee venom. Local ketamine injection produced a suppression of flinching reflex by 20. 90±2.88% and 45.76±13.9%, while that of lifting/bcking time by 39. 53±10. 05% and 59.94±5.53%, at doses of 25mmol/L and 50 mmol/L respectively without any motor disturbance, Local MK-801 resulted in an inhibition of flinching reflexby 22.84±3.12% and 49.53±5.35%. While that of lifting/licking time by 17. 49±5.67%and 53.49±3. 87%. At doses of 10μmol/L and 100 ,μmol/L respectively also with no motor disturbance. However, s. C. Administration of ketamine and MK-801 inior region symmetrical to the bee venom injection site on the contralateral hindpaw produced no change in the nociceptive behaviors, suggesting that the analgesic actions of keramme and MK-8Ol were not the result of systemic effects. The present resultsuggests that peripheral NMDA receptors are involved in the production of persistent pain.
2.PERIPHERALLY SUPPRESSIVE EFFECTS OF KETAMINE ON SUBCUTANEOUS BEE VENOM-INDUCED PROLONGED,PERSISTENT FIRING OF SPINAL WIDE-DYNAMICRANGE NEURONS IN THE CAT

By using extracellular single unit recording technique, locally suppressive effects of a single dose of ketamine on sub-cutaneous (s. c. ) bee venom-induced increase in firing of wide dynamic-range (WDR) neurons in spinal dorsal horn were investi-gated on urcthane-chloralose anesthetized cats. Injection of bee venom s.c. into the cutaneous receptive field (RF) resulted in asingle phase of prolonged, persistently increased firing of WDR neurons over background activity for more than 1 h. Local pre-treatment with ketamine (100 mM, 0. 1 m l) into the center of RF where bee venom was injected produced a dramatic suppressionof the increased neuronal firing by 60% (3.10± 0.42 spikes/s, n= 5) when compared with saline pre-treated group (7.61 ± 0.17spikes/ s. n = 5 ). Moreover, local post-treatment with the same dose of ketamine also produced a profound suppression of the in-creased neuronal activity by 81% (1.51±0.06 spikes/s, n=5) when compared with the saline post-treated group (7.76±0.15spikes s, n=5). However, s.c. administration with the same dose of ketamine into a symmetrical region on the bee venom un-treated contralateral hindpaw produced no affection on the increased firing of the WDR neurons, suggesting that the suppressiveaction of local ketamine was not the result of systemic effects. The present result suggests that ketamine may exert its localantinociceptive effects mainly through the peripheral NMDA receptors in addition to its partially potential blocking effects onsodium and voltage-sensitive calcium channels.
3.The effect of hyperbaric oxygen on neural stem cell proliferation and differentiation after cerebral ischemia

Objective To study the effect of hyperbaric oxygen (HBO) on the proliferation and differentiation of neural stem cells (NSCs) in rats after middle cerebral artery occlusion (MCAO).Methods Seventy-two adult,male,Sprague-Dawley rats were randomly divided into a control (CON) group,a hyperbaric air (HBA) group,a normobaric oxygen (NBO) group and a hyperbaric oxygen (HBO) group.All were subjected to MCAO.Rats in the CON group did not receive any treatment; those in the other groups were treated with HBA,NBO or HBO daily beginning 2 hours after the operation.Western blotting was applied to detect the expression of nestin,MAP2 and GFAP at 2,3,7 and 14 days after the MCAO.Results The expression of nestin in the HBO group was significantly higher than in the other groups on the 3rd,7th and 14th days.It peaked at day 3 but remained high until day 14.Similarly,expression of MAP2 was significantly higher than in the other groups at least until day 14.GFAP expression was significantly lower than in the other groups.Conclusion HBO can increase neural stem cell proliferation and neuronal differentiation,and inhibit the proliferation of astrocytes.
4.Influence of out-hospital nursing with no gap on the level of HbAlc for patients with diabetes

Objective To know the influence of out-hospital nursing with no gap on the level of HbAlc for patients with diabetes.Methods Divided 80 patients with diabetes into the intervention group and control group randomly,there were 40 cases in the each group.Out-hospital nursing cares with no gap were used in the intervention group,while the routine nursing cares were used in the control group.Fellowed up 1 year.Compared the FPG,2 hPG and HbAlc between the two groups at the time points of the 3rd,6th,9th and 12th month when out of hospital.Results All the indexes which had monitored were better in the intervention group than those of in control group.Conclusions Out-hospital nursing with no gap can remain the indexes in the normal level,and then promote their quality of life,which should be developed in clinical field.
5.Analysis of inhaled and food allergen spectrum of patient with allergic rhinitis in north of Zhejiang.

OBJECTIVE:
To explore the inhaled and food allergen distribution of patient with allergic rhinitis in north of Zhejiang and to analyze the difference of allergen distribution among different age groups.
METHOD:
One thousand and forty eight patients in north of Zhejiang diagnosed with allergic rhinitis in our outpatient department were tested with skin prick test (SPT). The positive rate of inhaled and food allergens were calculated. To analysis the difference of positive rate between children and adult.
RESULT:
Nine hundred and eighty-eight cases (94.3%) had the positive reaction. Dermato phagoides farinae and Dermatophagodies pteronyssinus had the highest positive ratio (72.1%, 71.8%) in inhalation group, followed cockroach (14.1%). In food group, Shrimp and peanuts had the highest positive ratio (18.3%,14.2%). Between children group and adults group, positive rate of food allergen was significantly different (P < 0.05), but of inhaled allergen was not significantly different.
CONCLUSION
The distribution of some allergens in children group and adults group was variable. The study shows that Dust mite was the mostly responsible common allergen in north of Zhejiang.

6.Rapid rehabilitation nursing mode reduces postoperative complications in elderly patients with appendicitis

Objective To investigate the effect of rapid rehabilitation nursing mode on postoperative complications in elderly patients with appendicitis. Methods Forty-eight elderly patients with appendicitis underwent surgery from July 2013 to June 2014 were set as the control group, and another fifty-three elderly patients from July 2014 to June 2015 as observation group. The patients in the control group were treated with routine care while the patients in the observation group with rapid rehabilitation nursing. Operative complications, gastrointestinal tumor recovery and postoperative hospitalization time were compared within one week after the operations between two groups. Result The rates of constipation, abdominal distention, chills and dysuria in the observation group were significantly lower than those of the control and the time of anal first exhaust, defecation and postoperative hospitalization in the observation group were significantly lower or shorter than those in the control group (all P<0.05). Conclusion The rapid rehabilitation nursing mode based on evidence can reduce surgery complications in elderly patients, promoter recovery and shorten hospitalization time.
7.Change regulation before and after treatment of snake bite for routine blood test and hypersensitive C-reactive protein

Objective To probe into the change regulation before and after treatment every snake bite patients for the routine blood test and serum hs‐CRP .Methods The study objects were selected in the hospitalized patients for clear diagnosis belong to what kind of snakes in the past two years .The indexes of routine blood test and serum hs‐CRP were determined before and after treatment various periods in these patients .The test results were made to statistical analysis according to kind of snakes ,periods and disease condition .Results WBC was obvious .rise before treatment only the viper snake bite patients .WBC was all significant .rise after treatment first day and second day for 5 kinds snake bite patients (P<0 .01) .This index had all reduced trend after treatment fourth day but determined value was still high in contrast to the normal reference scope .RBC and HGB all were normal level and had not obvious change before and after treatment for 5 kinds snake bite patients .PLT was reduced before treatment for the trime‐resurus gramineus bite patients ,before and after treatment for the viper snake bite patients (P<0 .05) .The hs‐CRP content was higher before treatment for the viper snake bite patients and was highest after treatment for the cobra snake bite patients ,the inter‐comparison had significant difference (P<0 .05) .Conclusion 5 kinds snake bite patients before and after treatment not basely ane‐mia symptom .But most of the patients appear the inflammatory response .PLT decrease is more serious for the trimeresurus grami‐neus bite patients ,and for the viper snake bite patients especially .
8.Comparison of pericardial devascularization with modified Sugiura procedure in management of portal hypertension

Objective To compare the effect of pericardial devascularization with that of the modified Sugiura procedure in management of portal hypertension. Methods From 1990 to 2008, 236patients with portal hypertension underwent operations including pericardial devascularization in 147and modified Sugiura in 89 in our hospital. Results There were 12 perioperative deaths (8.2 % ), and 2 rebleedings (2 % ) in the pericardial devascularization group, and 7 perioperative deaths (7.9 % ) and 2 rebleedings(3.4 % ) in the modified Sugiura group. The follow-up rate was 91.9 % in the pericardial devascularization group and 87.8% in the modified Sugiura group respectively, in a period from 6 months to 19 years. The 1-, 3-and 5-year rebleeding rates were 5.7%,15.2% and 25.5% in the pericardial devascularization group and 6.9%, 16.3%, 29.5 % in the modified Sugiura group, respectively. The 1-, 3- and 5-year survival rates were 87.8% ,79.1% and 69.7% in the pericardial devascularization group and 95.8 %,85.0%, 76.9 % in the modified Sugiura group, respectively. Conclusion Modified Sugiura procedure and pericardial devascularization have differences in perioperative mortality as well as rebleeding and survival rates.
9.Effects of etomidate on local synaptic transmission in substantia gelatinosa neurons of the adult rat spinal cord.

By using blind spinal slice whole-cell patch-clamp technique, we observed the influence of etomidate (ET) on synaptic transmission in substantia gelatinosa neurons of the adult rat spinal cord. Male adult Sprague-Dawley rats (7~8 weeks old) were anaesthetized with urethane (1.2 g/kg, i.p.), and then lumbosacral laminectomy was performed. The lumbosacral spinal cord (L1~S3) was removed and placed in preoxygenated Krebs solution at 1~3 degrees C. After cutting all of the ventral and dorsal roots, the pia-arachnoid membrane was removed. The spinal cord was mounted on a vibrating microslicer and then a 500 microm thick transverse slice was cut. The slice was placed on a nylon mesh in the recording chamber, and then perfused at a rate of 15~20 ml/min with Krebs solution saturated with 95% O2 and 5% CO2, and maintained at 36+/-1 degrees C. Substantia gelatinosa neurons were identified by their location. Under a binocular microscope and with transmitted illumination, the substantia gelatinosa was clearly discernible as a relatively translucent band across the dorsal horn. The resistance of patch clamp electrodes was 8~12 Msigma. Signals were gained by using an Axopatch 200B amplifier with low-passfiltered at 5 kHz, and digitized at 333 kHz with an A/D converter. The results are as follows. (1) To see whether or not ET has any effects on the local miniature excitatory postsynaptic currents (mEPSC), the holding potential was set up at -70 mV. Under such a condition extracellular superfusion was made with 1 micromol/L TTX for 2 min first, which was followed by consistent application of 500 micromol/L ET and 1 micromol/L TTX for 1 min. It was shown that ET did not influence the decay time, frequency and amplitude of mEPSC, when compared to the control. (2) To see whether or not ET has any effects on the local miniature inhibitory postsynaptic currents (mIPSC) mediated by GABA(A) receptor, the holding potential was set up at 0 mV. Under this condition extracellular superfusion was made with 1 micromol/L TTX and 1 micromol/L strychnine, an antagonist of glycine receptor, for 2 min, and then with consistent application of 50 micromol/L ET, 1 micromol/L TTX and 1 micromol/L strychnine for 1 min. ET prolonged the decay time of GABAergic mIPSC by 45.57+/-12.46% (P<0.05), but did not influence the frequency and amplitude of GABAergic mIPSC, when compared with the control. (3) To see whether or not ET has any effects on the local mIPSC mediated by glycine receptor, the holding potential was also set up at 0 mV, and under this condition extracellular superfusion was made with 1 mmol/L TTX and 10 mmol/L bicuculline, an antagonist also set up at 0 mV, and under this condition extracellular superfusion was made with 1 micromol/L TTX and 10 micromol/L bicuculline, an antagonist of GABA(A) receptor, for 2 min, and then with consistent application of 50 micromol/L ET, 1 micromol/L TTX and 10 micromol/L bicuculline for 1 min. ET had no effects on decay time, frequency and amplitude of glycinergic mIPSC. The above-mentioned results show that ET plays anesthetic or analgesic roles by modulating the decay time of GABAergic mIPSC, i.e. by prolonging the mean open time of GABA(A) receptors, however, ET has no direct effect on local excitatory synaptic transmission in substantia gelatinosa neurons of the adult rat spinal cord.

10.Feasibility of transplanting human bone marrow mesenchymal stem cells into manganese poisoning rats

BACKGROUND: Studies have shown that bone marrow mesenchymal stem cells (BMSCs) can treat central nervous system diseases. BMSCs have the function of self-renewal and differentiation into a variety of neural cell types. BMSCs with self-renewal and multi-directional differentiation abilities can successfully differentiate into dopaminergic neurons after transplantation into an animal model. OBJECTIVE: To observe the effect of transplanted BMSCs on behavior and dopaminergic neurons in rats with manganese poisoning. METHODS: Rat models of manganese poisoning were constructed by intraperitoneal injection of MnCl2?4H2O into Sprague-Dawley rats. The model rats were then randomly divided into two groups, BMSCs and phosphate buffered solution (PBS) control group, and 5 μL of passage 3 human BMSCs suspension or equivalent PBS was transplanted into the right striatum of the manganese poisoning rats. One month after transplantation, the rats were subjected to behavioral assessment. The differentiation of BMSCs was observed by immunofluorescence. The contents of dopamine, brain-derived neurotrophic factor, glial cell-derived neurotrophic factor in the right striatum of rats were detected by ELISA. RESULTS AND CONCLUSION: The behavioral score of the BMSCs treated group was significantly lower than that of the PBS control group after transplantation (P < 0.05). Double-labeled positive cells for human-specific nuclear antigen/tyrosine hydroxylase (hNUC/TH) and human-specific nuclear antigen/glial cell-derived acidic protein (hNUC/GFAP) were observed in the BMSCs treated group after transplantation. Meanwhile, hNUC/TH and hNUC/GFAP double-labeled positive cells were undetected in the PBS control group after transplantation. The expression levels of dopamine, brain-derived neurotrophic factor, glial cell-derived neurotrophic factor in the BMSCs treated group were higher than those in the PBS control group. This suggests that BMSCs can improve the behavior of manganese poisoning rats and can differentiate into dopaminergic neurons and astrocytes.
