1.Embryo carcinoma in a premature teratoma after peritonea (diagnosis and treatment)

A case of a woman aged 42 years old with abdomen pain under the navel and bilateral pelvis tumors indentified by CT scanner as post-peritoneal tumors and AFP level increased was treated at Viet Duc hospital with the diagnosis of embryonary carcinoma based on a mature malignant teratoma. Some aspects of the diagnosis and treatment of post-peritoneal teratoma with embryonary carcinoma were received. Authors concluded that up to now in adults for post peritoneal tumor under the group of germ cell tumors, the treatment of choice is a surgery of through removal of the tumor, then the formula of BEP, PEL or VAC chemiotherapy will be carried.

Primary germ cell tumor of extragonadal origin are rare, accounting for approximately 3 to 5 percent of all germ tumors. Extragonadal germ cell tumors usually originate in midline body structures. We report a case of extragonadal germ cell tumor from the left superficial inguinal area, a nonmidline structure and a case of from the superficial suprapubic area unusual site of extragonadal germ cell tumor-with brief of the literature.

This case reports of nasopharyngeal embryonal rhabdomyosarcoma mainly for a stuffy nose, runny nose with blood, and without typical clinical manifestations. Electronic laryngoscopy tip: nasopharyngeal neoplasm. MRI tip: nasopharyngeal carcinoma. By pathological and immunohistochemical examinations, it finally was diagnosed with nasopharyngeal embryonal rhabdomyosarcoma.

5.A Case Combined Germ Cell Tumor in Testis.

Primary germ cell tumor of the testis is rare, which occupies 1 to 2% of all reported malignant male neoplasms. Combined primary germ cell tumor of the testis composed of embryonal carcinoma and seminoma is more rare tumor. The authors experienced a case of 50-year old male who presented with painless enlargement of right testicle. He has had radical orchiectomy and diagnosed by pathologically as combined germ cell tumor of testis. The testicle is measuring 180 gm in weight and 9×6×5 cm in dimension, and almostly replaced by tumor mass. Grossly the tumor is rubbery solid smooth tumor mass, with variegated cut surface with geographically outlined diffuse necrotic area. Histologically the tumor is composed of two components of tumor, which are solid growth pattern of large round to polyhedral cells with clear or granular cytoplasm and distinct cell border, and anastomosing glandular and papillary arrangement of anaplastic epithelial cells. The former corresponds to seminoma, and the latter to embryonal carcinoma. Each tumor lobule is separated by abundant fibrous stroma.

6.Histopathological Studies on Tumors of Testis in the Taegu Area.

For evaluation on the histopathologic studies and age distribution of the testicular tumors in the Taegu area, the inguinal orchidectomized materials were collected at the Department of Pathology, College of Medicine, Yeungnam University, and the analyzed results were as follows: 1. In total of 11 cases of orchidectomized materials, germ cell tumors are 10 cases (90.9%). In germ cell tumors according to the histologic types, seminoma was 5 cases (45.5%), and embryonal carcinoma, 3 (27.2%). 2. The highest age incidence of the group is 20th and 30th, and the next, 50th and 10th.

7.A Case of Embryonal Carcinoma Developed in a Cryptorchid Testis.

A case of testicular embryonal carcinoma developed in s cryptorchid testis is presented with a review of the literature. According to Campbell's series, the incidence of malignant cryptorchid testis was 11.6%. and his statistical evaluation was similar to Gilbert and Hamilton's series of 7,000 cases of testicular neoplasms where 840 tumors were developed in undescended testes(12%). Both authors agree that the chance of neoplastic development in a cryptorchid testis is approximately 48 times greater then that developing in a normally descended testis. Several cases of testicular tumor were reported sporadically in Korea, but malignant tumors in cryptorchism were rarely seen in the literatures Only two cases of malignant cryptorchid testis were already reported in Korea.

Teratoma is one of germ cell tumor, common neoplasm in women of reproductive age, but it can arise at any age. Its malignant transformation is rare, less than 2%, frequently at older age. Here we present a case of mixed transformation, adeno-squamous cell carcinoma with brief review of the concerned literature.

9.A Case of Squamous Cell Carcinoma Arising in Mature Cystic Teratoma of the Ovary.

Mature cystic teratoma of the ovary is the most common ovarian germ cell tumor and almost benign, but malignant transformation occurs in less than 2% of benign mature teratoma. Of the malignancies arising in teratomas, squamous cell carcinoma is the most common (70-80%). The presentation in stage I disease dose not differ from that of benign cystic teratoma. In the early stage, the treatment is possible through surgical intervention alone. We experienced a case of squamous cell carcinoma of the ovary arising in mature cystic teratoma, which is presented with a brief review of literature.

10.Clinical Experience of Suprasellar Germ Cell Tumors.

The germ cell tumors are classified to germinoma, immature teratoma, mature teratoma, choriocarcinoma, yolk sac tumor and embryonal carcinoma. We analyzed 10 cases of suprasellar germ cell tumors which had been treated at Yonsei Uviversity Hospital from 1980 to 1990. Results of our study were summurized as follows : Common clinical symptoms are diabetes insipidus, visual dysfunction, panhypopituitarism, etc. Therapeutic plan can be based on serum and CSF AFP and HCG level. Pretreatment and posttreatment bormonal evaluation were performed and hrmonal replacement were required in all patients. Germinoma showed good outcome but immature teratoma showed bad outcome by multisystem involvement. More advanced new chemotherapeutic and immunotherapeutic agents must be studied for this tumor.
