Carbon monoxide poisoning is described as “silent killer” because carbon monoxide is an odourless, colourless, tasteless gas and the clinical presentation is non-specific. A two-month-old boy presented with rapid breathing, irritable, and refused to be fed. The symptoms occurred approximately 1 h after waiting with his father in the car with the engine running. The father also presented with symptom of carbon monoxide poisoning. The child had respiratory distress and sinus tachycardia. The child’s carboxyhaemoglobin level was normal, but the father’s level was elevated. Serum lactate and troponin I were raised. He was given 100% normobaric oxygen and was admitted. Subsequently his condition improved and was discharged the next day. Infants are more prone to the effects of carbon monoxide poisoning. When an infant suddenly become unwell, high index of suspicion and detail collaborative history are required so that carbon monoxide poisoning will not be missed out.

There have been multiple reports already regarding acute carbon monoxide/CO poisoning in the Philippines secondary to misuse of portable generators, especially during the times of typhoon. We present a case of unintentional carbon monoxide poisoning in a Filipino household wherein three of its members were found dead and leaving other five unconscious before they were rushed to the hospital. The index patient had increased serum fraction percentage of carboxyhemoglobin level and presented with rhabdomyolysis during admission. Neuroimaging confirmed a hypoxic-ischemic encephalopathy secondary to carbon monoxide intoxication. Even without hyperbaric oxygen therapy, patient improved with adequate hydration, early rehabilitation and trauma-focused psychotherapy.

Our research was carried out on 133 occupational carbon monoxide exposed workers in a metallurgical factory. The results showed that carbon monoxide concentration in some working places was higher than the allowance limit from 1.4 to 5.3 times. 77.2% indoor environment was polluted by substances coming from factory. This pollution had influenced on the health of 35.3% of workers (influence on nervous, circulatory, respiratory, digestive systems and anemia,..) 16.5% of the cases suffered from occupational carbon monoxide chronic clinical poisoning. The occupational acute carbon monoxide poisoned anamnesis at the factory was 7.3%.

The investigation was done in 74 occupational carbon monoxide exposed workers in a coke factory. The result showed that carbon monoxide concentration in some work places was higher than the allowable limit from 1,2 to 5 times. 70,3% workers' indoor environment was polluted by substances coming from factory. These pollution had influence on the health of 47,2% workers (influence on nervous, circulatory, respiratory, digestive systems and anemia etc.). 1,4% of the cases suffered from occupational carbon monoxide chronic clinical poisoning.
