1.Histopathological study of early gastric cancers with lymph node metastasis

Objective To examine the ability of cancer metastasis to lymph nodes by study of the h(?)o,(?)thology of early gastric cancers. Methods Eighty-one cases of early gastric cancers with lymph node(?)etastasis were used as metastatic group. Another randomly selected 81 cases of early gastric cancer without lymph node metastasis were used as control group. Both groups consisted of 11 cases of intramucosal cancer and 70 cases of submucosal cancer. The location of cancers in tissue was further classified as two categories; the superficial lesions and invasive fronts. In addition, the gastric cancers were hislopathologically divided into three subtypes: differentiated, undifferentiated and the mixed type. The histopathological classification and location of cancers in tissue were similar between the two groups. 2.Cloning and expression of Treponema pallidum specific antigens P15 and P47 for clinical application

Objective: To clone and express Treponema pallidum (TP) specific antigens P15 and P47 and use them in clinical examination. Methods: P15 and p47 genes synthesized were inserted into a GST fusion expression vector. The recombinant antigens were purified by affinity chromatography and then coated on microplates to establish an ELISA kit. Using this kit, national TP standards, sera of normal persons and patients with TP and other diseases were tested. Results: The synthesized p15 and p47 genes sequenced were identical with those of GenBank. National TP standards were tested by ELISA kit which coated with these recombinant antigens, and the results accorded with the standards. High sensitivity and specificity was showed when 2 recombinant antigens were used in ELISA to detect the sera of normal persons and patients with TP and other diseases. Conclusion: The recombinant TP specific antigen P15 and P47 are suitable for establishment of ELISA kit in clinical examination.
3.Effect of fish oil emulsion on early stage of experimental severe acute pancreatitis

Objective: To observe the effect of ω-3 fish oil emulsion on the experimental severe acute pancreatitis(SAP)through the morphologic alteration of pancreas,the level of serum amylase(AMS)and the functions of liver and kidney,and explore its possible mechanism. Methods: Rat model of SAP was produced by injecting 5% Sodium Cholate(1 ml/kg)into the biliopancreatic duct.Male Sprague-Dawley rats were randomly divided into 4 groups: normal group(n=6),fish oil emulsion treatment group(FOG,n=18),soybean oil emulsion treatment group(SOG,n=18) and normal sodium treatment group(NSG,n=18).Then fish oil emulsion(FO,10 ml/kg);soybean oil emulsion(SO,10 ml/kg)and normal sodium(NS,10 ml/kg)were intravenously injected respectively 120 minutes.The pancreatitis were confirmed by levels of serum AMS and histopathologic score.ALT,AST,BUN and Cr were tested 24 hours after the treatments.Expressions of IL-1β and IL-10 were tested by ELISA.The activated NF-kappa B was examined in the pancreases. Results: Lower level of serum AMS(P<0.05)and lower histopathology score(P<0.05) appeared in FOG.Compared with NS,FO decreased the levels of serum ALT and BUN significantly (P<0.05).FO significantly attenuated the expression of IL-1β (P<0.05).FO downregulated the activity of NF-kappa B efficiently.Conclusions: By down-regulating the levels of IL-1β together with up-regulating the level of IL-10,FO reduces inflammatory damage at the beginning of AP.
4.Preliminary study of MR imaging in rat with series coil

Objective To investigate the effect of MR imaging in rats with series coil. Methods MR scanning was performed on the self-made aqueous solution phantom with the series coil and other three coils respectively, using the same FRFSE- XL pulse sequence. SNR of the same image was obtained in the same position slice for all four coils. Twelve SD rats were divided into three model groups and one normal group. FRFSE-XL FSPGR and Probe-SI pulse sequence were respectively used to show the brain injury,1H-MRS, spinal cord injury (SCI) and abdomen with series coils. Results The SNR of series coil (39. 7)was over six times higher than the SNR of knee joint coil (6. 41 ), which was the best in other three coils.Grey matter and white matte; the brain structure and hematomas were clearly depicted in the rats'brain on T1WI and T2WI. From the second week after injection of 6-OHDA into the rats brain, the ratio of NAA/Cr decreased (before injection 1.24 ,two weeks after injection 0. 781 ). The spinal cord and the configuration of the white matter were clearly showed on T2WI image, and the location and degree of SCI in the rat thoracic spinal cord were also well defined on T2 WI image. The pulse sequence of FSPGR T1 WI was used for rat abdomen scanning. The scan time was only eight seconds. It is helpful to overcome the breathing movement effect and show the structure of rat abdomen. Conclusion It provided a good means for the rat MR in vivo to make use of 3.0T MR combined with series coils.
5.Clinical value of urine liquid-based cytology test in the diagnosis of urothelial carcinoma

Objective To explore the diagnostic accuracy of urine liquid-based cytology(LBC)for diagnosis of urothelial carcinoma(UC).Methods A total of 603 cases in our department from January 2005 to July 2007 were subject to urine liquid-based cytology test(LBC)and followed by histological examination.Using the histological appearance as the gold standard,the accuracy of LBC test was evaluated.Results A total of 436 cases with histological diagnoses were selected.There were 274 cases of UC and 61 cases of other malignant tumors of urinary system.The sensitivity,specificity and accuracy of urine LBC test for malignant tumors were 61.5%,86.5% and 67.1%,respectively.The positive predictive value and negative predictive value were 94.1% and 39.2%,respectively.The sensitivity of urine LBC test for UC and non-urothelial malignant tumors were 68.6% and 29.5%,respectively.The sensitivity for UC Was significantly higher than that for non-urothelial malignant tumors.The diagnostic sensitivity of urine LBC test for UC G1,G1-2;G2,G2-3 and G3 was 53.3%,74.5% and 90.6%,respectively.The diagnostic sensitivity of urine LBC was increased with the increacment of histological grade of UC.Conclusions The sensitivity of urine LBC test for high grade UC is high and has a good clinical value.However,for the diagnosis of low grade UC,the sensitivity of LBC test is low and adjuvant test is needed to improve it.
6.Expressions of injury-related gene in cultured developing neurons following seizures

Objective To study interleukin-1 receptor(IL-1R) and connexin(Cx36) gene expression following Mg 2+-free-induced seizures in cultured developing neuron. Methods Rat embryo cortical neurons cultured for 6 and 17 days were exposed to Mg 2+-free media to induce seizure. At different time after Mg 2+-free treatment, real-time RT-PCR was used to detect IL-1R and Cx36 mRNA expression. Results 1. IL-1R mRNA expression transiently decreased after Mg 2+-free treatment in neurons cultured for 6 and 17 days in vitro. Then the levels of IL-1R mRNA expression recovered in neurons cultured for 6 days, but IL-1R mRNA expression were increased in neurons cultured for 17 days compared with control group and the peak was at 24 hours. 2. In neurons cultured for 6 days in vitro, Cx36 mRNA expression increased after Mg 2+-free treatment compared with control group, the peak was at 24 hours. But in neurons cultured for 17 days in vitro, Cx36 mRNA expression decreased at 6 hours after Mg 2+-free treatment compared with control group, the peak was at 24 hours. Conclusions IL-1R mRNA and Cx36 mRNA expression following Mg 2+-free-induced seizures are different between the neurons cultured for 6 and 17 days in vitro. This is possibly related to the different neuron injury between 6 and 17 days in vitro following seizures.
7.Clinicopathological and biological features of micrometastasis in early gastric cancers.

OBJECTIVETo investigate the clinicopathological and biological features of micrometastasis in early gastric cancers.
METHODSEleven cases of early gastric cancer with micrometastasis (micrometastatic, MM group) and 46 cases of early gastric cancer with lymph node metastasis (control group) were included in the study. Immunochemical staining of ssDNA, bcl-2, p53, E-cadherin, Ki-67, CD34 was performed. The superficial lesions, invasive fronts and lymph nodes were examined in both groups.
RESULTSPositive rate of ssDNA at the superficial lesions in MM group was higher than that in control group. In MM group the positive rate of ssDNA in micrometastasis was higher than that at invasive fronts and in lymph nodes. Positive rate of bcl-2 at the superficial lesions in micrometastasis was higher than that at invasive fronts and lymph nodes. Positive rate of c-myc at the superficial lesions in MM group was higher than that in control group. Positive rate of E-cadherin and the percentage of microvascular areas at the lymph nodes in MM group was lower than those in control group. Proliferative ability of cancer cells at superficial lesions and lymph nodes in MM group was lower than those in control group. Lymph nodes <3 mm in micrometastasis accounted for 27.3%.
CONCLUSIONThe pathological and biological features of micrometastasis in early gastric cancer show low positive rate of ssDNA, E-cadherin, Ki-67 and low percentage of microvascular areas at the lymph nodes.

9.Experimental study on the inflammatory and immune responses of xenogenic acellular dermal matrix transplantation combined with thin split-thickness skin autograft.

OBJECTIVETo investigate the dynamic process of the inflammatory response and the profile of Th1/Th2 cytokines after xenogenic acellular dermal matrix (ADM) transplantation with thin split-thickness skin autograft overlay.
METHODSSD rats were used in the study. In the control group, thin split-thickness skin autograft (STSG) was transplanted in the full-thickness skin defect of the SD rats; in the experimental group, the xenogenic acellular dermal matrix combined with thin split-thickness skin autograft was transplanted. The inflammatory response was examined histologically and Th1/Th2 cytokine mRNA expression in skin grafts was determined by reverse transcription-polymerase chain reaction.
RESULTSInflammatory reaction was induced by ADM at the early stage of transplantation and decreased gradually. Th2 cytokine mRNA expression was higher in the ADM group than that of the control group whereas the Th1 cytokine mRNA expression was undetected in both groups.
CONCLUSIONXenogenic acellular dermal matrix is immunogenic. The increased expression of Th2 cytokines may be related to the humoral immune responses and the absence of ADM graft rejection.

10.Clinical research of thread-dragging through fistula method in treating patients with simple anorectal fistula

OBJECTIVE: To evaluate the efficacy and safety of thread-dragging through fistula method in treating patients with simple anorectal fistula. METHODS: In this multi-centered, prospective, and randomized controlled clinical trial, 244 patients with simple low or high anorectal fistula were randomly divided into study group (with the method of thread-dragging through fistula) and control group (with the method of incision or thread-drawing). The healing time and curative rate of anorectal fistula, and the integral calculus of clinical symptom and life quality evaluations before and after treatment were all examined. The maximal anal canal squeeze pressure was measured to compare the therapeutic safety between these two groups. The health economical benefits were also assessed to determine which therapeutic method was more economical. RESULTS: The curative rate of simple low and high anorectal fistula were of no significant differences between the study group and the control group. The healing time of simple low anorectal fistula in the study group and the control group were (22.26+/-8.67) d and (31.41+/-11.39) d respectively, while the healing time of simple high anorectal fistula in the study group and the control group were (24.73+/-8.15) d and (32.20+/-12.60) d respectively, and there revealed significant differences between these two groups. Each integral calculus of clinical symptom evaluation in the study group was not obviously different from those in the control group besides the integral calculus of anal sphincter function. The integral calculus of life quality between the study group and the control group of simple low anorectal fistula had no significant differences. The integral calculus of anal sphincter function and confidence in treatment in the study group of high anorectal fistula were better than those in the control group. The hospitalization expense of the study group was remarkably lower than that of the control group. The maximal anal canal squeeze pressure in the study group after treatment was not reduced obviously as compared with that in the same group before treatment, while it was decreased significantly in the control group after treatment as compared with those in the same group before treatment and in the study group after treatment. CONCLUSION: The method of thread-dragging through fistula in treating simple low and high anorectal fistula can shorten the course of the disease, save the hospitalization expenses, improve the life quality of the patients, and protect the anal sphincter function.
