1.Acute lumbar sprain treated with massage combined with acupuncture at different distal acupoints: a randomized controlled trial.

OBJECTIVETo observe treatment efficacy of acupuncture at different distal acupoints for acute lumbar sprain after massage.
METHODSOne hundred and fifty patients with acute lumbar sprain were randomly divided into a Yaotongdian (Extra) group, a Houxi (SI 3) group, a Weizhong (BL 40) group, a Chengshan (BL 57) group and a Shuigou (GV 26) group, total 5 groups, 30 cases in each one. After the same massage treatment and based on groups divided, different distal acupoints above-mentioned were selected to be acupunctured. Visual analogue scale (VAS) scores, lumbar activity and treatment efficacy of patients in 5 groups were evaluated.
RESULTSWith acupuncture at distal acupoints after massage,VAS scores of patients in 5 groups were decreased compared with those after massage treatment (all P<0.05); lumbar activity was all obviously improved (all P<0.01); and cured and markedly effective rates were all increased in comparison with those after massage (all P<0.01). But among 5 groups the differences of VAS scores, lumbar activity and cured and markedly effective rates were not statistically significant(all P>0.05).
CONCLUSIONBased on massage treatment, acupuncture at distal acupoints could further improve the treatment efficacy for acute lumbar sprain, but there is no obvious effective difference among every distal acupoint. As long as choosing acupoints with lower pain threshold and stimulating enough, good efficacy could be acquired for acute lumbar sprain.

2.Interventional therapy of advanced and/or recurrent breast cancer

Objective To evaluate the clinical efficacy of intraarterial infusion chemotherapy in patients with advanced and/or recurrent breast cancer. Methods From February 2000 to March 2003, 18 patients with advanced and/or recurrent breast cancer were treated with interaarterial chemotherapy (IAC). The Seldinger's technique was used in all patients. IAC was administered for 2-3 courses every 3-4 weeks for each patient. Results The proceduce was successfully performed in all 18 patients including one with a complete response, 12 of a partial response, none in 3, and with progression in 2. The overall response rate was 72.2%. The frequent adverse effects were fever, leukopenia, nausea, and vomiting but no severe complication occurred. Conclusion Intraarterial infusion chemotherapy is a safe, simple, complication-free and effective in the patients with advanced and/or recurrent breast cancer.
3.Intravesical instillation resiniferatoxin for the treatment of patients with idiopathic overactive bladder:a randomized,placebo controlled clinical trial

Objective To evaluated the clinical outcomes of intravesical instillation resiniferatoxin(RTX) for the treatment of patients with idiopathic overactive bladder(IOAB).Methods 26 cases with IOAB were randomly divided into test(A,14 cases) and control(B,12 cases) groups.The patients in group A were treated by intravesical instillation with 40ml of 0.5% idocain retained within the bladder for 3 minutes at first and with 100ml of 100nmol/L RTX retained within the bladder for 30 minutes late.The patients in group B were treated by the same method,however,the furacilin solution(placebo) at a dilution 1∶5000 was used instead of 100nmol/L RTX.The efficacy(daily voiding frequency,urgent uresis,FDV,MCBC,Qmax) of the 2 groups were evaluated before,1 month and 3 months after treatment.Results The pre-treatment comparion with prost-treatmen at 1 month and 3 months of A group was significant(P0.05).14 patients(54%) had slightly stimulating symptom in the urethra or bladder,and were otherwise generally well tolerated.Conclusion Single administration of RTX is safe and effective in patients with IOAB.
4.Control Study of Cervical Spondylotic Radiculopathy Treated by Loosening Posterior Tubercle of Cervical Transverse Process with CT-guided Insertion of Small Needle Knife and Blind Insertion of Small Needle Knife

Objective To investigate the successful rate and the therapeutic effect of loosening posterior tubercle of cervical transverse process with CT-guided insertion or blind insertion of small needle knife for cervical spondylotic radiculopathy. Methods Ninety patients with cervical spondylotic radiculopathy were divided into CT-guided insertion group and blind insertion group in the admission order using the random number table method, 45 cases in each group. CT-guided insertion group adopted the insertion of small needle knife following by the guide of CT, and the blind insertion group adopted the insertion of small needle knife following by cervical bony landmarks and muscles. After insertion, the position of the small needle knife in patients of the two groups was located by CT scan. If the needle knife did not arrive to the foci, it will be inserted again following by the guide of CT till successful insertion and location. The successful rate for the two times of insertion was calculated, and the pain scores were evaluated by visual analogue scale(VAS). Results(1) The successful rate for the first insertion of CT-guided insertion group was 73.3%, and that of the blind insertion group was 47.8%, the difference being significant(P < 0.01). The successful rate for the second insertion of CT-guided insertion group was 92.2%, and that of the blind insertion group was 80.0%, the difference being significant (P<0.01). (2) After treatment for one week, pain VAS scores of the two group were decreased (P < 0.01 compared with those before treatment) , and the decrease in the CT-guided insertion group was more obvious than the blind insertion group (P < 0.05). Conclusion CT-guided insertion makes the small needle knife therapy be more accurate and safer, ensures much better therapeutic effect, and brings less pain in the patients.
5.Optimization of Ultrahigh Pressure Extraction of Forsythoside from Forsythia Suspensa by Box-Behnken Experimental Design

Objective: To optimize the ultrahigh pressure extraction ( UPE) process for forsythoside from Forsythia suspensa by Box-Behnken experimental design. Methods:On the basis of single factor screening, a three-factor and three-level Box-Behnken ex-perimental design was employed with liquid/material ratio( X1 ) , extraction pressure ( X2 ) and extraction time ( X3 ) as the independent variables. The response variable was the extraction yield of forsythoside. Results:The optimal UPE conditions for forsythoside were as follows:the ratio of solvent to material (ml·g-1) was 70, the extraction pressure was 151 MPa, and the extraction time was 114 s. With the optimal extraction process, the extraction yield of forsythoside was 13. 15 mg·g-1 . Conclusion:As a novel extraction tech-nology for Chinese herbal medicines, UPE procedure has higher extraction yield, lower extraction temperature, shorter extraction time and less power consumption, which provides a brand-new method for the extraction of forsythoside from Forsythia suspensa.

OBJECTIVE:To improve the related standards in China Pharmacopoeia and to enhance its operation.METH_ ODS:Issues on“general rules on preparations”,“bacteria endotoxin test”,assaying of glucose injection in China Pharmacopoeia were analyzed and discussed.RESULTS&CONCLUSION:The dosage forms recorded are limited and some varieties of preparations are hard to be classified.How long exactly the gel formed by tachypleus amebocyte lysate and bacteria endotoxins remained cemented should be regarded as positive,which should be clearly defined.In determining the optical rotation by po?larimetry,ammonia test solution should be added to the semi-products to achieve balance but not in the finished products for differentiation.

AIM:To study the reversal effects of multidrug resistance by transfecting tumor necrosis factor ?(TNF-?) cDNA and multidrug resistant 1(MDR1) gene antisense RNA into multidrug resistant breast cancer cell line MCF-7/ADR.METHODS:The recombinant vector of enhanced green fluorescent protein(EGFP) with MDR1 antisense RNA and recombinant vector of red fluorescent protein(DsRed2) with TNF-? cDNA were constructed by RT-PCR and DNA recombinant techniques.The recombinant vectors were transfected into multidrug resistant breast cancer cell line MCF-7/ADR.The cell growth curves,cell apoptosis rates,MDR1 gene expression at mRNA and P-gp levels,and the sensitivity to ADR were determined before and after the transfection.RESULTS:After the transfection,cells showed lower growth rate,higher apoptosis rate,lower MDR1 expression at mRNA and P-gp levels,and the sensitivity to ADR increased significantly.CONCLUSION:Transfection of TNF-? cDNA and MDR1 antisense RNA into multidrug resistant breast cancer cells may have good effects on reversal of multidrug resistance.
8.Expression and clinical significance of p63 in nodular lymphocyte predominance Hodgkin lymphoma and classical Hodgkin lymphoma

Purpose To investigate the expression and clinical significance of p63 in nodular lymphocyte predominant Hodgkin lym-phoma (NLPHL) and classical Hodgkin lymphoma (CHL). Methods 15 cases of NLPHL and 54 cases of CHL were stained for CD45, CD30, p63, PAX5, CD20, CD15, Oct-2, BOB1, MUM1, EMA, EBV-LMP1 and Ki-67 by immunohistochemical methods of EliVision. EBER were detected by in situ hybridization method in 12 cases of CHL. Results The expression of p63 in NLPHL (53. 3%, 8/15) was significantly higher than that in CHL (0, 0/54) (P<0. 05). Conclusions p63 protein is frequently expressed in NLPHL and helpful in the differential diagnosis between NLPHL and CHL.
9.The Inhibitory Effect of Recombinant Solube KDR and Its Antibody on Endotheliocyte Proliferation

Objective: To investigate the inhibitory effect of soluble KDR(sKDR) and its antibody on endotheliocyte(EC) proliferation.Methods: The binding of sKDR with VEGF was detected by ELISA. The KDR262 antiserum was prepared from rabbit and the specificity was defined by Western blot. The inhibition of EC proliferation was analyzed by 3H-TdR up-take, MTT and cell counter. Results: The sKDR could bind to VEGF165 specially, the ability was enhanced by heparin. The specific binding of KDR262 antiserum to KDR was also detected. The inhibitory rate of sKDR(10,2,0 4?g/ml) on EC proliferation was 56%,44%,32% respectively. The inhibitory rate of KDR262 antiserum (1∶50,1∶200,1∶800) was 70%, 56%,43% respectively. The cell counter also showed that EC proliferation was inhibited significantly in sKDR group and KDR262 antibody group, vs VEGF165 group, GST group and PBS group. The inhibitory effect was characterized by concentration-dependent and was higher in KDR262 antibody group than that in sKDR group. Conclusion: sKDR expressed by E.coli and its antibody had significant inhibitory influence on EC proliferation.
10.HPLC/ELSD assay of cholic acid compounds by single standard substitute

AIM: To study the determination of 5 kinds of cholic acid content. METHODS: Using single standard substitue (SSS), we assayed 5 kinds of cholic acid, such as cholic acid, deoxycholic acid, hyodexycholic acid, chenodeoxychoilc acid and ursodeoxycholic acid by HPLC/ELSD. and studied the influence of the additive substitutes (such as rabbit Serum, urine and Qingkailing extract) on the measure values. RESULTS: By using any one of the 5 kinds of cholic acid standard, the content in all the 5 kinds of cholic acid could be determined. CONCLUSION: Simply as well as credibly, the SSS method could be used in quality analysis, especially in complex matrix. Also, it proved a feasible way in quality control and research of fingerprint chromatography of TCM.
