Chinese Medical Ethics 2017;30(3):367-371
Objectives:To understand the patients' cognition and satisfaction with public welfare in public hospitals. Methods:A questionnaire survey was conducted among 600 patients in Beijing municipal public hospitals and the data was statistically analyzed by SPSS 22.0.Results:Patients' understanding of public welfare was not ideal, the satisfaction was quite well, and the satisfaction of the patients in the experimental hospitals was more satisfactory than those in non-experimental hospitals. The patients have urgent needs on rational drug use, mutual recognition of medical results, smooth channels of patient rights, reduced medical expense and shortened waiting time. Conclusions:It should strengthen the publicity and governance of the public welfare of public hospitals to enhance the patients' understanding, breakthrough the urgent needs of public welfare to improve patients' satisfaction, and intensify the policy linkage and explore diversified implement forms of public welfare to improve service level.

Equity of the essential medicine system involves social justice, being important in health services availability equity. The essential medicine system,in practice in China, has to a certain extent improved the equity of health service availability, yet found with poor equity in the process due to differences in insurance, regional diversity, pricing of essential medicine, and poor implementation of the system at primary medical institutions.Such equity needs to be further improved in the future in such aspects, consistence of different insurance reimbursement percentages, further support to underdeveloped areas, encouragement of market competition, and better compliance of primary medical institutions in implementing the system.

Journal of Medical Postgraduates 2017;30(9):926-931
Objective To investigate the mechanisms of oxygen free radical/JNK signaling pathway on neuronal autophagy after subarachnoid hemorrhage(SAH) in rats. Methods 160 male Sprague-Dawley rats were randomly divided into four groups: sham group, SAH model group, low dose Edaravone group and high dose Edaravone group. The SAH model was established by autologous blood injection into cisterna magna twice, while the rats in the sham group were injected with isotonic saline(0.3mL/time).The high dose of edaravone group and low dose of edaravone group were given 5mg/kg or 10mg/kg of edaravone, respectively, once daily with tail intravenous injection after the models were established. The morphological changes of hippocampus neural cells were detected by light microscope. The malondialdehyde (MDA) level in brain tissue was determined with thiobarbituric acid. The changes of phosphorylated JNK and autophagic biomarkers (Beclin-1 and LC3-II)were detected by immunohistochemical method. The expressions of JNK mRNA,Beclin-1 mRNA and LC3 mRNA in hippocampus was detected by Real time-quantitative PCR.Results The necrotic nerve cells were seen in the hippocampus of SAH group in terms of nuclear dissolution, nuclear fragmentation or nuclear disappearance.Compared with Sham group, the level of MDA and the number of dead neurons, the expression of JNK mRNA, Beclin-1 mRNA and LC3-Ⅱ mRNA were increased in the SAH group (P<0.05).The survival rate of nerve cells in the SAH group was lower than that in the sham group. The immunoreactivity of phosphorylated JNK 、Beclin-1 and LC3-Ⅱ in the SAH group was enhanced than that in the sham group. However the damage of the morphological structure of nerve cells was relatively decreased in both doses groups.Compared with SAH group, the level of MDA and the expression of JNK mRNA in low dose Edaravone group and high dose Edaravone group were decreased. The expression of Beclin-1 mRNA and LC-3 mRNA was higher (P<0.05).Furthermore, the survival rates of nerve cells in both dosesgroups were higher than that in the SAH group (P<0.05).Meanwhile, the immunoreactivity of phosphorylated JNK in both doses groups was weakened than that in the SAH group. The mRNA expression of Beclin-1 and LC3-Ⅱ was increased(P<0.05).Conclusion Oxygen free radical played an important role in process of neuronloss by activating the JNK signaling pathway to regulate Beclin-1 and LC3-Ⅱ expression.

As one of the chronic disease of respiratory system, chronic obstructive pulmonary disease contributes to a progressive decline of lung function,as well as the extra-pulmonary effects.Malnutrition, one of the extra-pulmonary effects, becoming the common problem for chronic obstructive pulmonary disease patients. It has severe harm to chronic obstructive pulmonary disease patients that need us to pay more attention to it.The choice of nutrition assessment tools has important significance for the correct results of nutrition status of the patients.

Objective To explore the effect of Eight-Section Brocadeon improving extrapulmonary performance and lung function in elderly stable chronic obstructive pulmonary disease (COPD) patients. Methods A total of 120 elderly stable COPD patients were divided into experimental group and control group by random digits table method with 60 cases each. The experimental group received Eight-Section Brocade and the control group took a walk both based on the routine health education. The World Health Organization Quality of Life (WHOQOL)-BREF, 6 min walking test and lung function were conducted before and after intervention 12, 24 and 36 weeks. Results After intervention, the score of each field of WHOQOL-BREF were higher than those before intervention, comparison of two groups of physiological fields Ftime=57.112, Fgroup=10.510, Finteractive=13.653, all P<0.01; psychological field Ftime=240.209, Fgroup=6.201, Finteractive=6.902, all P<0.05;field of social relations Ftime=34.957, Fgroup=7.265, Finteractive=11.554, all P<0.01; environmentfield Ftime=66.431, Fgroup=8.197, Finteractive=18.413, all P<0.01, and there were significant differences in time, groups and interactive. Comparison of two groups of the result of 6 min walking test after intervention, Ftime=327.697, Fgroup=37.181, Finteractive=61.420, all P<0.01. Comparison of two groups of the result of lung function indexforced expiratory volume in one second, Fgroup=5.083, P<0.05, there was significant difference;after intervention the index of Forced expiratory volume in 1 second take up of the expected value, Fgroup=4.141, P<0.05, there was significant difference. Conclusions Eight-Section Brocade can effectively improve the elderly stable COPD patients with exercise endurance and quality of life, improve the extrapulmonary performance, delay the progressive decline of lung function.

Objective To investigate the effects of Berberine on growth of Immorto-Min colonic epithelial cell line (IMCE) and explore its possible mechanisms. Methods IMCE cells were treated with Berberine in the absence or presence of epidermal growth factor (EGF) and TNFα. Ki-67 staining and terminal deoxynucleotidyl transferase-mediated dUTP-biotin nick end labeling (TUNEL) assay were used to identify the cell proliferation and apoptosis respectively. Furthermore, Western blot analysis was performed to detect the epidermal growth factor receptor ( EGFR), protein kinase B (Akt) and their phosphorylation. Results ( 1 ) Proliferating activity of IMCE cells was increased after adding EGF and the proportion of cell proliferation was ( 10. 64 ± 1.41 ) %. The proportion was significantly lowed in EGF plus Berberine group [(1.81 ±0. 85)%] compared to the EGF group(P <0. 01 ), while the lowest was the Berberine group [(0.49 ± 0.42) %]. (2) The proportions of cell apoptosis were ( 8.47 ± 2. 52 ) % and (9. 39 ± 2. 13 ) %in the Berberine group and TNFt group respectively which were significantly higher compared to the normal control [(0. 27 ± 0. 30)%], both P < 0. 01. (3) The phosphorylation of EGFR was significantly increased after adding EGF and p-EGFR was decreased in EGF plus Berberine group at a concentration-dependent manner. (4) Moreover, the phosphorylation of Akt was enhanced after addition of TNFα , while the phosphorylation in the TNFα and Berberine group was inhibited compared to the TNFα group. Conclusions Berberine may suppress the proliferation and promote the apoptosis of IMCE cells. The mechanisms may relate to the inhibition of the phosphorylation of EGFR and Akt.
