BACKGROUND:Intracranial and extracranial arterial stent implantation is the main therapy for cerebrovascular diseases, especialy for some patients who cannot tolerate surgical operation, but the postoperative rehabilitation and biocompatibility problems restrict the further development of the technology. OBJECTIVE:To explore the rehabilitation efficacy and biocompatibility of acupuncture after degradable cerebrovascular stent implantation. METHODS: A total of 70 patients with cerebral arterial stenosis aged 43-79 years were enroled, including 41 males and 29 females, who were al treated with drug-eluting stent implantation, carotid artery stent implantation in 31 cases, vertebral artery stent implantation in 21 cases, subclavian stent implantation in 5 cases, and middle cerebral artery stent implantation in 13 cases. After stent implantation, al the patients were randomly divided into two groups: control group with routine rehabilitation therapy and acupuncture group with routine rehabilitation therapy+electroacupuncture. After 3 months of treamtnet, neurologic impairment scores, Fugl-Meyer score for motor function, Barthel index, degree of stent stenosis, thromboxane A2, prostacyclin, thromboxane A2/ prostacyclin levels were detected. RESULTS AND CONCLUSION:At 3 months after treatment, the neurological impairment scores were significantly lower in the acupuncture group than the control group (P < 0.05), but the Fugl-Meyer score for motor function andBarthel index were higher in the control group (P < 0.05). After treatment, the thromboxane A2 and thromboxane A2/prostacyclin levels were significantly decreased in the two groups (P < 0.05), but the prostacyclin levels were increased than those before treatment (P < 0.05). In addition, there were no differences in the thromboxane A2, prostacyclin, thromboxane A2/prostacyclin levels between the two groups. Compared with the control group, the rate of restenosis was significantly lower in the acupuncture group (P < 0.05), and there were no significant difference in the reference vessel diameter, blood vessel stent diameter, decreased vessel diameter index, re-intervention rate between the two groups (P > 0.05). Taken together, acupuncture can promote the rehabilitation efficacy folowing degradable cerebrovascular stent implantation, reduce the incidence of restenosis, and show good biocompatibility.

Objective To investigate the efficiency and safety of laparoscopic hepatectomy (LH) and open hepatectomy (OH) in patients with hepatocellular carcinoma (HCC). Methods A total of 45 HCC cases undergoing hepatectomy from January 2008 to December 2010 in our hospital were divided into LH group ( n =21 ) and OH group ( n =24 ).Their clinical efficiency and safety were compared and analyzed. Results The amount of intraoperative blood loss and blood transfusion in LH group were significantly higher than that of OH ( respectively x2 =3.973,x2 =4.862,all P < 0.05 ).LH is associated with less postoperative complications and duration of hospital stay, compared with OH ( respectively x2 =4.746,t =-2.717,all P <0.05).Cumulative survival rates in both groups were not statistically significant ( x2 =0.172,P > 0.05 ).However tumor-free survival rate in LH was significantly higher, superior to that of OH (x2 =4.543,P < 0.05). Conclusions LH and OH are all safe and effective hepatectomy for HCC.LH has the advantage of less complication and shorter hospital stay.

Objective To investigate the prevalence of resistant Neisseria gon orrhoeae and plasmid-mediated resistant strains in Guangzhou from 1996 to 2001. Methods The resistant N.gonorrhoeae and plasmid-mediated resistant strains to tetracycline (TRNG) were determined using agar dilution method, and penicillinas e-producing N. gonorrhoeae (PPNG) by acidometric method. Results A total of 793 gonococcal isolates were tested from 1996 to 2001. The resistant rate for penic illin increased from 57.2%to 81.8%and PPNG from 2%to 27.2%, respectively, du ring the six years. Resistance to tetracycline remained high and stable over the years, while the rates of TRNG were increased from 1.5%to 27.2%. Conclusions The prevalence of plasmid-mediated resistant strains of N. gonorrhoeae increase s year by year in Guangzhou. These results suggest that the clinical isolates of gonococcal strains in this city are highly resistant to penicillin and tetracyc line.

BACKGROUND: T-type or linear type external fixator is applied in transcervical fracture in the elderly. Because its fixing force is weak, so it is difficult to avoid the aggravation of complication due to long term lay up in severe osteoporosis. Based on this reason, percutaneous penetrating arch tri-claw external fixator is designed to treat elderly transcervical fracture with strong fixing force for the realization of early restoration to the active status before injury to reduce complication.OBJECTIVE:To design pereutaneous penetrating arch tri-claw external fixator for the treatment of elderly transcervical fracture for the investigation of its feasibility.DESIGN: A self-controlled study by employing patients as subjects.SETTING: Department of orthopedic surgery of the first affiliated hospital of a universityPARTICIPANTS: Thirty-one cases of elderly transcervical fracture including 18 males and 13 females aged between 71 and 86 years old with an average age of 79 years were admitted by the Department of Orthopedics of the First Affiliated Hospital of Nanhua University between December 2002 and June 2004.METHODS: Percutaneous penetrating arch tri-claw external fixator was applied in the treatment of 31 cases with elderly transcervical fracture for the observation of its clinical effects, and the force-bearing situation and stability of the external fixator during application were analyzed by theoretical mechanics.MAIN OUTCOME MEASURES: ① The functional effects of percutaneous penetrating arch tri-claw external fixator applied in the therapy of transcervical fracture(sub-femoral head type, transcervical type and femoral basilar type); 2② The force-bearing situation and the stability of the fixing point on the fixator; ③ The restoration of activity in hip joint and fracture union time RESULTS: ① Patients could sit and stand immediately after the applicationof the fixator, and its fixing reinforce was analyzed by theoretic mechanics during standing(the force on upper fixing point A, B or C was 1/22, 1/2 or 1/2 of the lower fixing point) . The unidirectional flexion and extension activity of the hip joint overcame hip-inversion and rotational disposition at the distal end of the fracture. The hip-joint axial compression board caused compact embedment and insertion of the fracture end and vertical crush to simulate fracture union by stability and compressive strain. ② The stability of percutaneous penetrating arch tri-claw external fixator: No disposition was found during the application except rotational disposition during activity in sub-femoral head type fracture. The fixation was stable, the operative trauma was less, and the operation was simple with good accommodation. No complication was found due to long-term lay up and no re-disposition due to the loosening of the external fixator was found. ③ The hip-joint flexion-extension function was restored within 7 days averagely and fracture healing time was about 2 to 3 months, average of 2.4 months.CONCLUSION: No re-injury is found dter the application of percutaneous penetrating arch tri-claw external fixator, which has small force bearing at upper fixing point with strong stability. There is strain stimulation on the cross section of the fracture to promote fracture healing. The articular function status before injury could be restored at early stage.

Objective This retrospective study was to explore the efficacy and determine the risk factors of survival for recurrent hepatocellular carcinoma ( HCC) treated by repeat hepatectomy. Methods From January 1995 till December 2010, 60 patients with recurrent HCCs, were treated by repeat hepatectomy. The significance of seventeen clinical or pathological variables in the risk factors of overall survival were assessed. Results The overall survival 1,3, and 5-year survival rates were 76. 3% , 40.7% and 25. 0% (from repeat hepatectomy), and 95. 0% , 62. 6% and 43. 3% ( from initial hepatectomy) , respectively.Univariate analysis indicated that tumor size at initial hepatectomy, recurrence interval from initial hepatectomy, serum albumin(ALB) level, resection margin, diameter of largest recurrence tumor and rumor vascular invasion were significant prognostic factors(P <0. 05, Kaplan-Meier Log-rank test). Multivariate analysis showed recurrence interval from initial hepatectomy, resection margin, diameter of largest recurrence tumor and rumor vascular invasion were significant prognostic factors(P<0.05, Cox proportional hazards model). Conclusion Repeat hepatectomy is effective for recurrent HCC. Recurrence interval from initial hepatectomy, resection margin, diameter of largest recurrence tumor and rumor vascular invasion were significant prognostic factors.

BACKGROUND:Transplantation of mesenchymal stem cels to prevent and treat degeneration of the intervertebral disc is a feasible method. Mesenchymal stem cels co-transfected by SRY-related high mobility group-box gene 9 (SOX-9) and growth differentiation factor-5 (GDF-5) can differentiate into nucleus pulposus cels, in order to obtain greater effect of induction and proliferation of nucleus pulposus cels. OBJECTIVE:To investigate the effect of SOX-9 and GDF-5 co-transfection on the differentiation of rabbit bone marrow mesenchymal stem cels into nucleus pulposus cels. METHODS: We separated and cultured bone marrow mesenchymal stem cels from the bone marrow of rabbit aged 4 months. Passage 3 cels were divided into five groups andin vitro induced to differentiate into nucleus pulposus cels: non-transfected group, empty vector transfection group, SOX-9 transfection group, GDF-5 transfection group, SOX-9 and GDF-5 co-transfection group. At 14 days after transfection, RT-PCR was employed to assay SOX-9, GDF-5 and colagen type II mRNA expressions in bone marrow mesenchymal stem cels. The marker of nucleus pulposus cels-KRT19 expression was also detected by immunohistochemical staining. RESULTS AND CONCLUSION:In the co-transfection group, the mRNA expressions of SOX-9, GDF-5, and colagen type II were significantly higher than those in the SOX-9 transfection group, GDF-5 transfection group, and both these two groups, respectively (P < 0.05). Cels were positive for KRT19 in the SOX-9 and GDF-5 groups, and strongly positive for KRT19 in the co-transfection group. These findings indicate that double gene-transfected bone marrow mesenchymal stem cels are better than single gene-transfected cels with regard to differentiation into nucleus pulposus cels and secretion of extracelular matrix.
