2.A Case of Leukemia Cutis at the site of Invasive Cutaneous Candidiasis: Wolfs Isotopic Response.

The isotopic response defined by Wolf et al. describes the occurrence of a new skin disorder at the site of another, unrelated and already healed skin disease. In most of the reported cases, the first diseases were herpes zoster. We report a case of leukemia cutis developed on the exact same site of previous invasive cutaneous candidiasis, regarded as Wolfs isotopic response.

3.Isolation and Identification of Candida Species in Superficial Cutaneous Candidiasis.

BACKGROUND: There have been few clinical and mycological studies of superficial cutaneous candidiasis in Korea. OBJECTIVE: The purpose of the study was to investigate the recent clinical and mycological features of superficial cutaneous candidiasis and correlation between underlying diseases and candidiasis. METHODS: Among 233 patients, 233 strains of Candida(C.) species were isolated and identified by Sabouraud's dextrose agar culture, germ tube test and sugar assimilation test using API 20C kit. RESULTS AND CONCLUSION: 1. The incidence of superficial cutaneous candidiasis was 4.0% out of a total of 5,809 out-patients. 2. The ratio of male to female patients was 1.1:1. 3. Among the age groups, the incidence rate was high in the first decade and above sixth decade. 4. Candidal intertrigo was the most common type of infection(56.7%). 5. Among 233 Candida species C. albicans(90.6%), C. parapsilosis(4.7%), C. guilliermondii(2.1%), C. tropicalis(1.7%), C. glabrata(0.9%) were identified. 6. Neurological disease was the most common underlying disease(56.4%).

4.Identification of Candida Species Using CHROMagar Candida in Superficial Cutaneous Candidiasis.

BACKGOUND: CHROMagar Candida is a new differential culture medium that allows selective isolation and identification of clinically important Candida species. However, no study of CHROMagar Candida in superficial cutaneous candidiasis has been reported in Korea. OBJECTIVE: The purpose of this study was to evaluate the use of CHROMagar Candida to identify Candida species isolated from patients with cutaneous candidiasis. METHOD: A total of 95 strains isolated from 92 patients with candidiasis (70 Candida albicans, 9 Candida parapsilosis, 7 Candida guilliermondii, 1 Candida krusei, 1 Candida glabrata, 1 Candida tropicalis, 2 C. albicans plus C. parapsilosis, 1 C. albicans plus C. krusei) were subcultured to CHROMagar Candida (KOMED, Korea) and incubated for 48 hours. Colony appearance on CHROMagar Candida was assessed by two observers. RESULTS: Expected colony appearance on CHROMagar Candida was 100% for C. albicans, C. krusei, C. glabrata and C. tropicalis, respectively but 85.7% for C. guilliermondii and 77.8% for C. parapsilosis. Three mixed cultures of Candida species, not detected by conventional methods, were detected by CHROMagar Candida. CONCLUSION: CHROMagar Candida is a useful isolation medium capable of a rapid presumptive identification of Candida species and more reliable detection of mixed cultures in clinical specimens.

5.A Case of Congenital Cutaneous Candidiasis.

We report, a 1-day-old male with congenital cutaneous candidiasis who had generalized erythematous papules and pustules on the whole body including palms and soles from birth. At 5th month of pregnancy, his mother had a history of vaginal candidiasis for a month KOH mount of pus from pustules of several different sites showed budding spores and pseudohrphae and fungus culture revealed the growth of Candida albicans We believe this to he the first documented case of congenital cutaneous candidiasis in Korean literature.

6.Superficial Cutaneous Candidiasis.

Candidiasis is an acute or chronic infection caused most commonly by Candida albicans and occasionally by other species of the genus Candida. The superficial cutaneous candidiasis is varied; oral Candidiasis, perieche, candidal vulvovaginitis, candidal balanitis, candidal intertrigo, erosio interdigitalis blastomycetica, perianal candidiasis, candidal paronychia, candidal onychia, chronic mucocutaneous candidiasis, congenital cutaneous candidiasis, and candidid. Diagnosis can be made by typical appearance of the clinical lesions and the presence of satellite vesicopustules. This can be confirmed by KOH examination and culture of skin scrapings. Treatment is the correction of predisposing factors, topical therapy of imidazoles and/or systemic administration of itraconazole or fluconazole.

7.Mural Folliculitis and Alopecia with Cutaneous Candidiasis in a Beagle Dog.

A one-year-old male Beagle dog showed dermatitis, alopecia and scales. Examination of the affected dog revealed generalized alopecia, patchy erythema, and superficial erosions with histological evidence of mural folliculitis. External tests for parasites in scraped skin samples were negative. However, fungal culture tests and polymerase chain reaction revealed the existence of Candida in the lesion. These results suggest that cutaneous candidiasis may induce mural folliculitis and alopecia in dogs.

8.A Clinical and Mycological Study of Cutaneous Candidiasis.

This survey was done on 79 out-patients in order to investigate the present status of cutaneous candidiasis in clinical and mycological aspects in dermatologic clinic of Korea University Hospital from January, 1982 to July1983. Among the 79 specimens, 56 strains of candida species were identified by KOli mount, Sabourauds dextrose agar culture, sugar assimilation test, sugar fermentation test a.nd corn meal agar with Tween 80 culture. Following results were obtained. 1. The incidence of cutaneous candiasis was l. 6A (79 cases among 4, 963 outpatients). 2. The cutaneous candidiasis was more frequent in fcmale than in male(1: 1. 7) 3. The cutaneous candidiasis had its highest in.cidence during first decade (45 .6%). 4 Candidal intertrigo was the commonest type of the infection (51. 9%,) 5. Thrush, diaper candidiasis and candidal intertrigo were common during first. decade, and candidal paronychia and candidal onychia were common during third decade. 6, Positive rate on KOH mount was 60. 8% Nail (38. 5%) and periungal area (42.9%) should low positive rate. 7. Positive culture rate on Sabourauds dextrose agar of Candida species was 70. 9% (56 cases). 8 Candida species isolated from cutaneous lesions were Candida albicans (52 case) and Candida trapicalis(2 cases). Two species could not be identified.

9.In vitro Antifungal Susceptibility Testing of Clinically-isolated Trichophyton Species to Luliconazole.

BACKGROUND: Luliconazole (LLCZ) is an imidazole antifungal agent widely used in Japan for treatment of tinea, cutaneous candidiasis, and pityriaisis versicolor, etc. Theoretically, its potent antifungal activity should make it a viable option for various conditions of dermatophytoses, but there has been no studies to support the notion thus far. OBJECTIVE: This prompted the authors to investigate the distribution of minimum inhibitory concentration (MIC) of luliconazole by carrying out in vitro antifungal susceptibility test on Trichophyton (T.) species clinically isolated from Korean subjects with tinea pedis. METHODS: In vitro antifungal susceptibility test, with broth macrodilution method of luliconazole and terbinafine hydrochloride (TBF) as reference drug on two clinically isolated Trichophton species (22 strains of T. rubrum, 5 of T. mentagrophytes), was carried out. RESULTS: The range of MIC for LLCZ on the total of 27 strains was 0.0110~0.0140 microg/mL (geometric mean: 0.0027 microg/mL), and the range for TBF was 0.0120~0.0250 microg/mL (geometric mean: 0.0042 microg/mL). Additionally, the geometric mean for MIC of LLCZ on T. mentagrophytes was 0.0125 microg/mL, and on T. rubrum, it was 0.0019 microg/mL. No strain showed resistance in vitro. CONCLUSIONS: The authors hence demonstrated that antifungal activity of LLCZ in vitro against Trichophyton species clinically isolated from Korean subjects is quite superior. These findings emphasize the usefulness of LLCZ, a promising new imidazole, for the topical management of dermatophytoses.

10.Diagnoses and Differential Diagnoses of Superficial Mycoses.

The diagnoses of superficial mycoses such as dermatophytosis, cutaneous candidiasis and Malassezia infections are relatively easy, and direct smear with KOH and culture with Sabouraud's dextrose agar media are the most useful and valuable methods. Recently, the incidence of onychomycosis has been increasing and some other diagnostic methods, especially for onychomycosis, and their significance has been introduced and investigated. This review was focused especially on the findings and significance of histopathologic and KONCPA methods (KOH Treated Nail Clippings Stained with PAS) for the diagnosis of onychomycosis. Summaries and brief comments were also made for the findings and significance of other diagnostic methods as well as for the differential diagnoses of superficial mycoses.
