3.Effect of intervertebral finite fenestration on lubar intervertebral disc protrusion complicated with spinal stenosis

Objective To investigate the effect of intervertebral finite fenestration on lubar intervertebral disc protrusion (LIDP) complicated with spinal stenosis.Methods Eighty patients with LIDP complicated with spinal stenosis were selected from May 2014 to May 2016 in the 159th Hospital of the Chinese People's Liberation Army.The patients were divided into observation group and control group according to the treatment method,40 cases in each group.The patients in the observation group were treated with intervertebral finite fenestration,lateral recess and nerve root canal decompression.The patients in the control group were treated with total laminectomy,lateral recess and nerve root canal decompression.The lumbar pain of patients was evaluated by the visual analogue scale (VAS),and the lumbar function was evaluated by using Japanese Orthopaedic Association(JOA) scoring system in the two groups before and after operation.The postoperative complications were observed in the two groups.Results There was no significant difference in VAS score of lumbar pain between the two groups before operation (t =1.638,P < 0.05).The VAS score of lumbar pain after operation was significantly lower than that before operation in the two groups(t =3.182,4.303,P < 0.05).The VAS score of lumbar pain in the observation group was significantly lower than that in the control group after operation(t =2.776,P < 0.05).There was no significant difference in JOA score of lumbar function between the two groups before operation(t =5.170,P < 0.05).The JOA score after operation was significantly higher than that before operation in the two groups (t =28.841,45.070;P < 0.05).The JOA score of the lumbar function in the observation group was significantly higher than that in the control group after operation(t =52.952,P < 0.05).The total effective rate in the control group and the observation group was 62.5 % (25/40) and 92.5 % (37/40),respectively;the total effective rate in the observation group was significantly higher than that in the control group(x2 =12.381,P <0.05).The incidence of postoperative complications in the control group and observation group was 25.0% (10/40) and 10.0% (4/40),respectively;the incidence of postoperative complications in the observation group was significantly lower than that in the control group(x2 =11.140,P < 0.05).Conclusion Intervertebral finite fenestration can effectively reduce lumbar pain,improve the lumbar function of patients with LIDP complicated with spinal stenosis;and it has fewer postoperative complications.
4.Impact of 5-fluorouracil on glucose metabolism and pancreatic pathology in rats.

OBJECTIVETo explore the impact of 5-fluorouracil (5-FU) on glucose metabolism and pancreatic pathology.
METHODSTwenty Wistar rats were divided into 5-FU group(n=10, chemotherapy was administered intraperitoneally to animals at a dose of 20 mg/kg daily for continuous 5 days) and control group (n=10, sodium chloride was administered intraperitoneally to animals with the same dose at the same time ). Glucose tolerance was evaluated 2 and 7 days following 5-FU treatment by serial measurement of blood glucose before and after an oral glucose load. Plasma insulin concentration was determined by radioimmunoassay. Pancreatic pathology was examined with morphological method and the ultrastructural changes of β cells were observed by transmission electron microscope.
RESULTSFasting blood glucose level was significantly higher in the 5-FU group than that in the control group [(7.6±0.9) mmol/L vs. (4.6±0.6) mmol/L at day 2; (8.9±1.0) mmol/L vs. (4.7±0.6) mmol/L at day 7, P<0.01]. Insulin releasing test indicated that the early phase insulin response to glucose load was significantly diminished in animals treated with 5-FU at day 2. Insulin level was significantly lower in the 5-FU group than that in the control group at 30 min (P<0.01). The peak secretion time of plasma insulin in 5-FU group was at 60 min, similar to the control group; and plasma insulin level decreased more slowly. Plasma insulin level was higher in 5-FU groups than in control groups on 120 min and 180 min. At day 7, Insulin level was lower in the 5-FU group than that in the control group on 60 min, and the peak secretion time of plasma insulin was delayed to 120 min. Plasma insulin level was significantly increased in 5-FU group than that in control group on 180 min(P<0.01). No gross histopathological damage to the pancreas was observed at day 2 and 7 following administration of 5-FU. The structural changes of mitochondria were mainly the quantities of secretory granule diminished at day 7 under transmission electron microscope. Dilated rough endoplasmic reticula, swollen mitochondria, and the presence of adipose drops in lysosomes were found in few cells.
CONCLUSIONS5-FU-induced hyperglycemia appears to be mediated in part by a relatively deficient insulin secretion to glucose stimulation. A relative deficiency in insulin secretion following 5-FU treatment appears to be related to β cells function impairs with islet cell ultrastructural changes induced by 5-FU.

5.Setting of logos on tobacco control information at outlets for retails and restaurants in 12 cities of China

Objective To explore the setting of logos on tobacco control information at outlets for retails and restaurants in 12 selected cities of China.Methods For all the shops for retail of tobacco,alcohol,food and restaurants under survey in 333 blocks of 12 cities (Beijing,Tianjin,Shanghai,Qingdao,Hangzhou,Shaoxing,Suzhou,Nantong,Zhenjiang,Chengdu,Xining and Harbin),setting and contents of logos on tobacco control information,inside and outside them were examined.Results 45 700 objectives were included in the study.Among all types of retail shops,the identification rate of tobacco control information at the entrance and inside were 3.6% and 4.4%,with an overall identification rate as 7.0%.The overall rate at the entrance of all the restaurants was 4.6% which was larger than the ones at the retail shops.Our result showed that there were differences between cities and types of establishments and higher rates seen in the larger ones.Of all the places that having had placement of information on tobacco control,only 18.5% of them had put them both inside and outside.Slogans or images on "No Smoking" were the main forms of information but less than 10% of them would show signs as ‘exclusive non-smoking'.Conclusion Data from our survey showed that the identification rate of tobacco control information was at a low level in 12 cities,and differences were seen between cities,size of establishment,that called for improvement of the existing tobacco control policies in China.
6.Screening for the microdeletions of azoospermia factor on the Y chromosome in male infertile patients from Guangzhou.

OBJECTIVETo investigate the relationship between microdeletion of azoospermia factor (AZF) and male infertility.
METHODSMultiplex PCR was used to detect Y chromosome microdeletion in AZFa, AZFb and AZFc in 103 cases of idiopathic azoospermia, 72 cases of severe idiopathic oligozoospermia, and 60 healthy male controls.
RESULTSNo microdeletion was found in 60 controls. Y chromosome microdeletion was found in 19 of 175 azoospermia patients, the total prevalence rate of microdeletion was 10.9%. There were 15 cases (11 for azoospermia, 4 for severe oligozoospermia) in AZFc (8.6%), 3 cases (1 for azoospermia, 2 for severe oligozoospermia) in AZFb+c (1.7%), 1 case (azoospermia) in AZFa+b+c (0.6%). According to statistics, the difference of microdeletion rate between two groups was significant(P < 0.01).
CONCLUSIONY chromosome microdeletions is an important reason of azoospermia. Screening of Y chromosome microdeletions for azoospermia patients before intracytoplasmic sperm injection treatment is essential.

Embryo technology, including super-ovulation, embryo transfer and embryo nucleus transfer, was a well-developed technology in 20th century. It has been widely used in animal science and veterinary and boomed livestock industry in China. Chinese livestock producer could gain high grade breeds in short time through embryo technology by the reason that embryo technology could make outstanding gene spread out in herd. With the help of embryo technology, rare animals could have a chance to extend their progeny in fast changing world. In this article, we briefly introduced the process of embryo transfer and application of embryo technology in China.

Embryo development is the main stage of the formation of various tissues,organs and systems.Abnormal embryo development can lead to embryo deformity and even death.Many factors are involved in this process.It was found that dual-specificity phosphatase(DUSP)could regulate the growth and development of different cells through a variety of signaling pathways,and it has been confirmed that the DUSPs family has an important association with embryonic development.A large amount of evidences showed that DUSPs and its downstream mitogen-activated protein kinase(MAPK)pathway played a key role in the regulation of gametogenesis,digestive system development and embryonic ear development.dusp27 is another extremely important gene whose role in early embryonic development remains to be studied,but it plays an important role in the development of embryonic muscle tissue.In this paper,the development of the above systems in DUSPs family was briefly reviewed in order to provide theoretical basis for the treatment of embryonic developmental diseases.
9.The reconstruction of neovascularization of posterior pharyngeal flap: an experimental study in dogs.

OBJECTIVETo elucidate the reconstruction of neovascularization that occurred in the superiorly and inferiorly based posterior pharyngeal flaps in different time postoperatively.
METHODSTen mongrel dogs were randomly divided into two experimental groups, which were performed superiorly or inferiorly based posterior pharyngeal flap surgery respectively. Each group was then subdivided into five subgroups, and were sacrificed immediately after operation or on 3, 7, 30, 90 day postoperative respectively. Microangiography was used to exhibite the vessel.
RESULTS1. The blood vessel reconstruction of the superiorly based posterior pharyngeal flap was more rapid compared with the inferiorly based flap. The 3-day flap has established an axial vascular network, which was mature on the 30-day flap. The superiorly based posterior pharyngeal flap was mainly supplied by the pedicle. 2. The blood vessels reconstruction of the inferiorly based posterior pharyngeal flap was firstly occurred in the pedicle and apex of the flap, which grew slowly to the middle of the flap. The inferiorly based posterior pharyngeal flap was supplied by the pedicle and the soft palate. A mature axial vascular network was exhibited on the 90-day flap, which was not mature on the 30-day flap.
CONCLUSIONBoth superiorly and inferiorly based posterior pharyngeal flap can establish an axial vascular network and gain ample blood supply.

10.Brachiocephalic artery-sparing aortic arch repair combined with stent-graft elephant trunk technique for Stanford type A aortic dissection: analysis of 23 cases.

OBJECTIVETo assess the clinical outcomes of brachiocephalic artery-sparing aortic arch repair combined with stent-graft elephant trunk technique for treatment of Stanford type A aortic dissection.
METHODSTwenty-three patients with Stanford type A aortic dissection requiring arch replacement underwent brachiocephalic artery-sparing aortic arch repair combined with stent-graft elephant trunk technique. The operations were performed within 72 h (20 cases) or 3-14 days (3 cases) after the onset of aortic dissection.
RESULTSThere was no perioperative death in these cases. The mean extracorporeal circulation time was 209∓52 min, the aortic cross clamp time was 85∓21 min, and the mean chest tube output within the first 24 h after the operation was 570∓263 mL; none of the patients required chest reopening for management of bleeding. Postoperative acute renal failure requiring hemodialysis occurred in 3 cases, transient neurologic dysfunction in 2 cases, paraplegia in case and hematosepsis in 1 case. No such complications as permanent neurologic deficit or postoperative visceral malperfusion occurred in these cases. All the patients survived and were discharged from hospital without experiencing severe complications in the follow-up for 6-18 months.
CONCLUSIONBrachiocephalic artery-sparing aortic arch repair combined with stent-graft elephant trunk technique is a safe and simple procedure with controllable bleeding and can serve as an optional procedure for aortic arch replacement.
