Objective To compare the curative effect of nasal intermittent positive pressure ventilation (nIPPV ) and nasal con-tinuous positive airway pressure (nCPAP) method .Methods Retrospective study was took on 196 neonatal infants ,all cases were assigned into nIPPV group (n=102) and nCPAP group (n=94) according to mode of ventilation from January 2011 to December 2012 in a neonatal intensive care unit of this hospital .The ratio of requirement for endotracheal ventilation and the outcome were in-vestigated .Results Infants treated initially with nIPPV need less endotracheal ventilation and ventilation time than those treated with nCPAP (11 .8% vs .24 .5% ,P<0 .05) .Conclusion Compared with nCPAP ,nIPPV decreased the requirement for endotrache-al ventilation in neonatal infants after extubation .
2.Resident dietary exposure of cadmium in Jiangsu province.

OBJECTIVETo investigate the resident dietary cadmium exposure in Jiangsu province and assess its safety.
METHODSCadmium concentration of 229 food items under 12 food groups were obtained from the food surveillance program in Jiangsu province between 2001 and 2006. Food consumption data of 778 food items of 3938 residents who were classified into four age groups (< 7, 7-, 13-, 18-) were got from the Nutrition and Health Status Survey of the Jiangsu resident in 2002 by 24 h dietary recall on three consecutive days. Dietary cadmium exposures for the residents of different age groups were obtained by using both point estimation and simple distribution estimation through integrating the two datasets above. The safety of dietary cadmium exposure was assessed.
RESULTSPoint estimation showed that the average dietary cadmium intakes of different age groups ranged from 5.7 to 8.6 microg/kg, accounting for 567.1% - 857.1% of the provisional tolerable daily intake (PTDI, 1.0 microg/kg). Result of simple distribution method showed mean daily cadmium exposure of different age groups ranged from 0.2 to 0.4 microg/kg, accounting for 20% - 40% of PTDI. Mean weekly cadmium exposure ranged from 1.4 to 2.5 microg/kg, accounting for 20% - 35.7% of the provisional tolerable weekly intake (PTWI, 7.0 microg/kg). The mean daily dietary cadmium exposure for different groups were as follows: < 7, 0.4 microg/kg; 7-, 0.3 microg/kg; 13-, 0.2 microg/kg; 18-, 0.2 microg/kg. Differences of daily dietary cadmium exposures among groups were significant (F = 69.0, P < 0.05). The mean weekly dietary cadmium exposure for different groups were: < 7, 2.5 microg/kg; 7-, 2.0 microg/kg; 13-, 1.4 microg/kg; 18-, 1.4 microg/kg. Differences of weekly dietary cadmium exposures among groups were also significant (F = 41.6, P < 0.05). The P97.5 of daily cadmium exposure for < 7 and 7- were 1.4 and 1.2 microg/kg, respectively, both of which were higher than PTDI. The P99.0 of daily cadmium exposure for 13- and 18- were 1.3 and 1.1 microg/kg, respectively. The daily dietary exposure from cereals for different age groups were 21.5 - 253.4 microg/kg, occupying 42.2% - 47.8% of the total daily exposure. Vegetables were 8.0 - 119.4 microg/kg, occupying 14.6% - 20.1%.
CONCLUSIONThe average level of dietary cadmium exposures for residents in Jiangsu province calculated by simple distribution estimation were much lower than that calculated by point estimation and were considered to be at no risk. P97.5 or P99.0 of daily or weekly dietary cadmium exposure of different age groups exceeded PTWI and PTDI. The main food types of dietary cadmium exposure were cereals and vegetables.

3.Construction and validation of a model for predicting the risk of immune checkpoint inhibitor pneumonitis

Objective:To construct and validate a risk prediction model for immune checkpoint inhibitor-associated pneumonia (CIP) using machine learning algorithms and the nomogram, aiming to provide an accurate and intuitive method to assist nurses in screening people at high risk of developing CIP.Methods:This was a retrospective case -control study. A total of 230 oncology patients treated with immune checkpoint inhibitors attending Zhujiang Hospital of Southern Medical University from January 2019 to February 2022 were collected using the hospital's electronic medical record system. The prediction models were built using five machine learning algorithms and nomogram. The models were then validated on a separate test set, and their differentiation and stability were assessed using evaluation indices like AUC and accuracy rate.Results:Underlying lung disease, smoking history, serum albumin≤35 g/L and radiotherapy history were identified as important influencing factors of CIP in all six models. The AUC of K nearest neighbor, support vetor machines (SVM), naive Bayesian, decision tree and random forest models predicted CIP were 0.647, 0.696, 0.930, 0.870, and 0.934, respectively. The AUC of the model created by the nomogram was 0.813, which was lower than the best random forest model in the machine learning algorithm, but with good predictive performance (AUC=0.934).Conclusions:The nomogram model can assess the patient′s risk more intuitively, but the risk prediction model of CIP based on a machine learning algorithm has a higher diagnostic value. It is suggested that the accuracy and usefulness of the prediction model can be increased by combining the nomogram's foundation with the machine learning algorithm.
4.Protective effect of non-mitogenic haFGF on retinal injury induced by N-methyl-N-nitrosourea in Sprague-Dawley rats.

AIMTo study the effect of non-mitogenic human acidic fibroblast growth factor (nm-haFGF) on retinal injury induced by N-methyl-N-nitrosourea (MNU) in Sprague-Dawley rats and its mechanism.
METHODSFemale rats of 50-days-old were injected with MNU (60 mg x kg(-1)) intraperitoneally, and three doses of nm-haFGF (1.25 microg, 2.5 microg and 5 microg in one eye of each rat) were injected, separately, into vitreous body of one eye of each rat twice a day at 0 and 12 h after MNU treatment. 24 h later, apoptotic index of photoreceptor cells was detected by TUNEL labeling and the expressions of Bcl-2 and Bax were analyzed by Western blotting. At the 7th day, retinal injury was evaluated based on retinal thickness.
RESULTSCompared with model group, apoptotic index of photoreceptor cells was significantly reduced in nm-haFGF groups at the dose of 1.25 microg and 2.5 microg in one eye of each rat at 24 h, and the total retinal thickness as well as the outer retinal thickness markedly increased 7 days after MNU, respectively. The expressions of Bcl-2 increased and that of Bax decreased adversely after being injected with different doses of nm-haFGF.
CONCLUSIONnm-haFGF partially suppressed retinal injury induced by MNU in Sprague-Dawley rats. The mechanism could be related to up-regulation of Bcl-2 and down-regulation of Bax.

5.Probabilistic evaluation of dietary exposure to lead and age-related exposure centile for residents in Jiangsu.

OBJECTIVETo establish a probabilistic model for evaluation of dietary exposure to lead and construct age-related exposure centiles for the residents in Jiangsu.
METHODSLead contamination data were obtained from the national food contamination monitoring program during 2001 - 2006 and 2791 samples from 232 food products in Jiangsu were included. Food consumption data were from the national diet and nutrition survey conducted in 2002, including 3938 subjects in Jiangsu. A non-parametric probabilistic model using Monte Carlo simulation was applied to derive the intake distribution. The intake data was then analyzed using the LMS method, which constructs exposure percentiles adjusted for the median (M), the coefficient of variation (S) and the skewness (L) of the intake distribution.
RESULTSThe median and P(99) of the lead exposure for the residents in Jiangsu were 1.02 µg×kg(-1)×d(-) and 9.29 µg×kg(-1)×d(-1), respectively.6.38% of the total population showed to have a lead intake exceeding the tolerable limit, which for the urban and rural population were 4.31% and 7.06%, respectively. The exceeding rate for children of 2 - 10 years old from the urban and rural areas were 13.17% and 17.70%, respectively.
CONCLUSIONThere was a large variation in the lead exposure level of the population in Jiangsu; People in rural areas are in greater risk for higher lead exposure than urban people; The dietary exposure to lead for children and the high-end population was serious.

6.Changes of IFN-gamma, IL-2, IL-6 and IL-10 in the patient with cardiac surgery under combined acupuncture anesthesia.

OBJECTIVETo observe the effect of electroacupuncture (EA) on cytokines in the cardiac surgical patient and to evaluate the application of combined acupuncture anesthesia to cardiac surgery.
METHODSThirty patients with atrial septal defect were divided into 3 groups, general anesthesia group (A), acupuncture anesthesia group (B) and combined general anesthesia and EA group (C). Peripheral blood samples were collected before anesthesia, before cardiopulmonary bypass (CPB), 30 min after CPB and 24 h after operation to determine the levels of interferon-gamma (IFN-gamma), interleukin-2 (IL-2), IL-6 and IL-10.
RESULTSThe levels of IFN-gamma and IL-2 decreased in the 3 groups after CPB and further decreased 24 h after operation, and in the group C were higher than those in the group B. The levels of IL-6 and IL-10 significantly increased 24 h after operation in the 3 groups with no significant difference among the 3 groups.
CONCLUSIONThe general anesthesia combined with EA can not completely improve the decrease of IFN-gamma and IL-2 induced by CPB, indicating that the good response of the general anesthesia combined with EA to stress can partially improve the immunosupression induced by CPB. Acupuncture does not have significant effect on inflammatory cytokine reaction induced by cardiac surgery.

7.Biochemical tests of rat cardiac injury induced by citreoviridin toxin in rats with joint deficiency of selenium and protein

ObjectiveTo clarify the causative effect of citreoviridin toxins(CIT) as well as nutritional deficiency of selenium and protein on rat myocardial injury at biochemical level.Methods According to 2 × 2 factorial design,48 healthy male Wistar rats aged 4-week were randomly divided into four groups:exposed to CIT along with nutritional deficiency of selenium and protein,nutritional deficiency of selenium and protein,exposed to CIT and control groups.After the rats in each group began to be fed with selenium and protein deficiency and(or) adequate fodder for 3 months,8 mg/kg body weight of CIT was fed daily to the rats in the two CIT toxin groups for two months.After that the CIT dose was raised up to 10 mg/kg body weight each day within the final 2 weeks.At the experimental endpoint,all the rats were sacrificed by femoral artery bleeding after ether anesthesia,and serum and heart specimens were collected for biochemical analysis by detecting serum Tn-Ⅰ and albumin,serum activities of CK and GSH-Px,myocardial SOD and T-AOC.ResultsThe interactions between Se & protein and CIT in rat final body weight,serum albumin,and Tn-Ⅰ was observed(F=8.186,6.160,19.183,all P< 0.05),whereas interactions in rat body weight of 12 weeks,serum GSH-Px,CK as well as myocardial SOD,T-AOC activity were not found (F=1.633,1.987,0.075,0.474,1.145,all P > 0.05).Under the nutritional deficiency of selenium and protein,serum albumin and Tn-Ⅰ level in the groups with CIT toxin[ (42.88 ± 1.19) g/L,(668.6 ± 55.8) ng/L,respectively]were lower than that of the group without CIT toxin[ (47.59 ± 1.05)g/L,(989.3 ± 49.2)ng/L,respectively,all P <0.05].The main effects of selenium and protein on rat body weight of 12 weeks,serum GSH-Px,myocardial SOD and T-AOC were statistically significant between different groups (F =96.860,58.086,4.475,25.485,all P < 0.05).Rat body weights of 12 weeks in the two groups of nutritional deficiency of selenium and protein[ (186.33 ± 7.89),(197.83 ± 7.89)g] were all lower than others[ (274.08 ± 7.89),(265.42 ± 7.89)g,all P < 0.05]; serum GSH-Pxs in the two groups of nutritional deficiency of selenium and protein[ (317.5 ± 102.6),(296.9 ± 90.5)U/L] were all lower than others[ (926.1 ± 110.9),( 1181.7 ± 85.9)U/L,all P < 0.05] ; myocardial SODs in the two groups of nutritional deficiency of selenium and protein [ (65.22 ± 5.91 ) × 106, (62.68 ± 5.61 ) × 106 U/kg] were all lower than others [(74.07 ± 7.24) × 106, (80.07 ± 5.91) × 106 U/kg,all P< 0.05]; myocardial T-AOCs in the two groups of nutritional deficiency of selenium and protein[ (1.138 ± 0.086) × 106,(0.806 ± 0.081 ) × 106 U/kg] were all lower than others[(1.688 ± 0.105) × 106,(1.163 ± 0.086) × 106 U/kg,all P < 0.05].Conclusions Animal model with selenium and protein deficiency is successfully established.However,the results of biochemical index tested in the experimental rats show no regularity effect,which needs to be checked again in the future study.
8.Morphological changes of rat cardiac tissue caused by citreoviridin combined with selenium and protein deficiency

ObjectiveTo observe the morphological feature of myocardial changes in adult rat exposed to citreoviridin(CIT) with selenium and protein deficiency.MethodsAccording to 2 × 2 factorial design,48 healthy male Wistar rats aged 4-week were randomly divided into four groups:group Ⅰ (Se-Pro-CIT+,low selenium and low protein plus CIT group),group Ⅱ (Se-Pro-CIT-,low selenium and low protein without CIT group),group Ⅲ (Se+Pro+CIT+,adequate selenium and adequate protein plus CIT group),and group Ⅳ (Se+Pro+CIT-,adequate selenium and adequate protein without CIT group),12 rats in each group.After one week of normal adaptive feeding,all the rats were fed with selenium and protein deficiency feed for 2 months,and then the animals in group Ⅰ and group Ⅲwere fed with 8 mg·kg-1 ·d-1 of CIT for 2 more months,after that the CIT dose was increased to 10 mg·kg-1 ·d-1 for the final 2 weeks.At the end of the experiment,all the rats were sacrificed by femoral artery bleeding,and body and heart weight were measured.Heart weight index was calculated and histopathological changes were observed under light microscope.Results Heart weight indexes of the 4 groups(Se-Pro-CIT+,Se-Pro-CIT-,Se+Pro+CIT+ and Se+Pro+CIT- groups) were (3.65 ± 0.45) × 10-3,(3.05 ± 0.19) × 10-3,(3.83 ± 1.06) × 10-3 and (3.31 ± 0.52) × 10-3,respectively.The results of factorial analysis showed that the effects of CIT on heart weight index were statistically significant(F =8.524,P < 0.05 ),the effects of Se + Pro were not statistically significant(F =1.347,P > 0.05),and there were no interactions between the two factors (F =0,048,P > 0.05).Morphologically,tissue fibrosis around branch coronary artery in group Ⅰ rats,plenty of cardiocyte pycnosis in group Ⅱ,and myocardial scattered necrotic foci in group Ⅲ were observed,accompanied by inflammatory cell infiltration in group Ⅲ,and normal myocardial structure in group Ⅳ rats.Conclusions Citreoviridin plays a major role in causing myocardial injury( degeneration and necrosis) and CIT combined with selenium and protein deficiency can aggravate the

OBJECTIVETo investigate into the changes of dietary patterns and their impacts on health of urban and rural residents in Jiangsu Province, China during the 1990s.
METHODSOn the basis of the results of food consumption survey, the calorie intake was calculated according to the balanced diet method and the quality of diet was assessed by scores of desirable dietary pattern (DDP).
RESULTSIt was found that food consumption and dietary patterns changed remarkably during the 1990s. Grain consumption was decreasing year by year, but the consumption of animal food was markedly increasing. Although the score of desirable dietary pattern (DDP) in urban residents was more than 90, the deducted score due to over-consumption of animal food increased. The mortality from infectious diseases evidently decreased in the whole province, while the death rates of some chronic diseases, such as diabetes, hypertension, colorectal cancer and breast cancer, were increasing rapidly in urban areas.
CONCLUSIONSSufficient attention should be paid to the negative effects of change in dietary patterns on people's health, especially in the urban residents.

10.Establishment of database for food classification and coding in Chinese dietary exposure assessment.

OBJECTIVETo establish the basis for Chinese dietary exposure assessment database by classifying and coding the data from the national dietary survey and pollutant surveillance.
METHODSThe method, which combined CODEX food classifying and coding of Codex Alimentarius Commission (CAC) with Chinese food classification of food composition table, was applied to classify and code the data of 1 810 703 Chinese dietary consumption and 487 819 pollutant surveillance. The coding system was according to the first two letters of the respective food group that represent the type or source of foods, the last four digits represent the serial number of the food in the CAC food classification. If the foods can be found in CAC food code system, its original food code is used. The new codes corresponding with the foods which are not exist in CAC food code system, is added according to CAC coding methods.
RESULTSDietary consumption data were divided into 6 major categories, 19 types, 75 groups, the agricultural products of pollutant surveillance corresponding to 499 codes. Comparing with CAC food coding system, Chinese dietary consumption data have added F (candy snacks) and G (beverages) 2 major categories, 4 types, 33 groups, 302 new codes. The additional groups most were the processing food groups with Chinese characteristics, such as canned, beverages, candy, meat products.
CONCLUSIONThe foundation of data communication to dietary exposure assessment has been established, and the connection of Chinese food classifying and coding with CAC data have been achieved.
